[Air tissue expansion, as an alternative to physiologic serum expansion: report of 30 cases].

There has been a renewed interest in cutaneous expansion as a plastic surgery procedure. Complications and failures remain considerable despite a more adequate and standardized technique. Based on a study of 30 cases of cutaneous expansion for burns sequelae, the authors emphasize the value of using air instead of physiological saline as filling medium, to reduce the negative repercussions on quality and quantity of cutaneous gain, related to the increasing weight of the prosthesis, even for large and possibly repeated expansion volumes.

[Expanded flaps on the head and neck].

The authors study the practical aspects of head and neck expansion: choice of material, choice of flaps obtained by skin expansion. They then examine the specific features of expansion in this anatomical region, where it is frequently used, with emphasis on preservation of aesthetic units and the specificities of skin hair and natural orifices. Scalp expansion in the context of baldness is discussed in a separate article.

[Facial paralysis. Treatment of upper eyelid occlusion by inclusion of a concealed gold plate].

Inclusion of a gold plate to oppose the action of the levator muscle has been proposed for a long time in the treatment of superior palpebral paralysis. Long-term ulceration of the overlying skin has generally led to the abandonment of this technique. The simultaneous insertion, in front of the gold plate, of a conchal cartilage graft, which is subsequently reinhabited, overcomes this disadvantage. This is a simple, rapid, reversible, effective procedure which is indicated essentially in longstanding or recent permanent facial paralysis, but also as temporary treatment while waiting for the results of suture, graft or anastomosis of the facial nerve.

[Forum on tissue expansion. Conclusion. How to master expansion...].

The author presents several reflections based on more than ten years of experience with skin expansion. In order to master expansion, a number of rules must be respected at each step of the procedure:--start by defining the treatment plan,--choose appropriate material,--apply the expander(s) according to a rigorous approach,--prevent and treat all complications,--ensure sufficient expansion to eradicate the entire lesion. The authors views are illustrated by particular clinical examples using expanded transposition flaps: treatment of extensive baldness, ear reconstruction, repair of major sequelae of thoraco-abdominal burns.
