Oocyte and embryo donation: evaluation of 412 consecutive trials.

A total of 199 patients (412 consecutive cycles) were treated by oocyte-embryo donation in 336 replacement cycles. Of these, 296 involved intra-uterine embryo transfers, 38 zygote intra-Fallopian transfers (ZIFT) and two gamete intra-Fallopian transfers (GIFT). Of the 336 replacements, 244 (73%) constituted transfers of fresh concepti and 92 (27%) of frozen-thawed ones. A total of 85 pregnancies were achieved of which 16 ended in preclinical abortions, giving a clinical pregnancy rate of 34.7% per patient, 20.5% per transfer and a take-home baby rate of 29.1% per patient. The pregnancy rate was significantly higher (P less than 0.05) following fresh gamete or embryo replacement (23%; 56/244) than following that of frozen-thawed embryos (14.1%; 13/92). No significant difference was observed when intra-Fallopian replacement was applied (27.5%; 11/40) as opposed to intra-uterine (19.6%; 58/296). Ovarian function was not found to be of significant importance to the achievement of pregnancies after oocyte donation since comparable pregnancy rates per replacement and per started cycle were obtained in patients with ovarian failure and in those with functional ovaries (19% and 15.4%; 24.2% and 20.2% respectively). Comparison of the implantation and abortion rates between these two groups did not reveal any significant difference (11.1% and 11.1% versus 14.8% and 16.6%). The highest pregnancy rate among patients with ovarian failure was observed in those with primary ovarian failure (26.4%; 14/53), while the lowest was among women who had received chemotherapy and/or radiotherapy (9%; 1/11).(ABSTRACT TRUNCATED AT 250 WORDS)

Embryo Transfer

Laparoscopic application of interceed (TC7).

Interceed (TC7) is a surgical adjuvant which has been shown to be efficacious in reducing adhesion formation in gynaecological pelvic surgery by laparotomy. In view of the increasing number of laparoscopic interventions and the concern with reducing post-operative adhesions, we evaluated the possibility of Interceed delivery through the ancillary trocars. Twenty-four patients underwent operative laparoscopy followed by the application of Interceed. No difficulties were noticed. Material sized from 1.9 x 2.5 to 3.8 x 5 cm was successfully delivered into the peritoneal cavity, while the duration of the procedure ranged from 2.5 to 6 min for each piece. In conclusion, it is feasible to consider application of Interceed during laparoscopic interventions. This approach to delivery requires a short operative time, minimal instrumentation and enhanced ease of application.

[The anti-lipidemic effect of Minipress].

Prazosin (Minipress) monotherapy was given to 152 patients with essential hypertension for one year in a multi-center study involving 13 hospitals and university clinics. In three centers serum levels of total cholesterol, HDL-cholesterol and triglycerides were also determined in 32 patients with hypertension and hyper/dys-lipoproteinemia. As a consequence of Minipress monotherapy significant decreases were found in serum level of cholesterol (after three months and also after one year), triglycerides (after one year), while the serum concentration of HDL-cholesterol increased. Atherogenic index (a ratio of total cholesterol over HDL-cholesterol) was significantly decreased by Minipress. As new data showing a causative correlation between hypertension and hyperlipoproteinemia were published in the literature authors, on the basis of their results, suggest to determine lipid profile in every patient with hypertension. They regard Minipress as the first line drug in young patients with "familial dyslipidemic hypertension". When choosing an antihypertensive drug metabolic side effects should be taken into consideration.

Changes of some lipid and lipid peroxidation characteristics in obese people as a result of a low energy diet.

In blood samples of 54 obese persons (13 men, 41 women) some lipid components, malondialdehyde (MDA), and the activities of free radical protecting enzymes superoxide-dismutase (SOD) and glutathione-peroxidase (GSHpx), were determined before and after a seven-day slimming diet of 2.1 MJ/day. Body weight, triglycerides, HDL-cholesterol, and HDL-3-cholesterol all decreased, total cholesterol increased after the seven days hospitalization. After continuing the slimming diet (5.0-6.3 MJ/day) at home, HDL-2-cholesterol increased and total cholesterol returned to the initial value after three months. MDA decreased in the groups of hyperlipoproteinemia II/a and II/b and also in the group with high cholesterol and triglyceride levels (above 5.7 and 2.7 mmol/l). Correlation coefficients refer to the dominant role of high TG in the intensity of lipid peroxidation. The activity of SOD fell after the slimming, independent of lipid parameters. GSHpx activity remained unchanged except a drop in the male participants having high cholesterol levels. The behavior of the free radical protecting enzymes needs further examination.

Lipid characteristics and malondialdehyde level in the sera of obese people.

In the blood sera of 70 obese persons (26 men, 44 women) some lipid components and malondialdehyde (MDA) level were determined before slimming. The people were classified into groups of hyperlipoproteinaemia on the basis of laboratory results, according to Fredrickson and Lees. In 32 people with high blood cholesterol level (above 5.7 mmol) there were negative correlations between MDA and high-density lipoprotein (HDL) fractions. Correlation coefficients were the greatest in the group IIa of hyperlipoproteinaemia (HDL-C. r = -0.74; HDL-2-C. r = -0.54; HDL-3-C. r = -0.78). Correlations were not found in subjects with a normal cholesterol level. The results were attributed to the formation of oxidized cholesterol products, caused by lipid peroxidation, which may decrease the HDL synthesis. It seems that in hypercholesterolaemia coupled with obesity, lipid peroxidation can contribute to the reduction in HDL levels, which is an important protective factor against cardiovascular diseases.
