Primary antiphospholipid syndrome evolving into systemic lupus erythematosus.

Since 1983 we have followed a total of 165 patients with antiphospholipid syndrome (APS). During the median followup period of 78 mo (range 12-336 mo), 3 of 80 patients with primary APS subsequently developed features of systemic lupus erythematosus (SLE) or lupus-like disease. One patient developed lupus-like disease 4 yrs and the other 2 developed full blown SLE more than 10 yrs after initial presentation of primary APS. Tissue typing in patients who developed SLE showed HLA antigens A2, A3, B35, Bw6, Cw4, DR7, DRw53, and DQ2 (Case 2); and A1, A3, B7, B8, Bw6, Cw7, DR4, DR15, DR51, DRw53, and DQ1 (Case 3). We report clinical features and genetic associations of these 3 patients.

Characteristics of early onset esotropia.

Some 115 patients with a reported onset of esotropia within the first six months of life were studied retrospectively to assess ocular findings and the evolution of any changes with time. All patients had been treated by one ophthalmologist and one orthoptist since 1972. Of the 115 patients, 27% had an accommodative element associated with the esotropia. Patients with an accommodative esotropia had higher levels of hypermetropia when compared to the non-accommodative group, but there were no other distinguishing characteristics. The groups exhibited a large variation in angle size, ranging from four to 45 degrees at the initial visit. The presence of fusion, amblyopia and the degree of hypermetropia had no significant effect on the course of the squint. Amblyopia was noted in 67% of cases and appeared to stabilise at a later-than-expected age, with 66% of patients with amblyopia stabilising between eight and 11 years. Surgical intervention had no effect on the incidence of amblyopia at the initial or final visits.

Single channel and monolayer studies of acylated gramicidin A: influence of the length of the alkyl group.

Three different gramicidin A analogues bearing acyl chains of various length on the ethanolamine moiety have been studied by investigating their single channel behaviour and their monolayer properties. It is shown that the single channel conductance does not depend on the substitution of the ethanolamine OH group and that the channel lifetime is roughly proportional to the length of the alkyl chain. The monolayer study indicates that acylation of gramicidin A produces compounds which have medium-dependent conformations. These acylated compounds are miscible with lipids, while GA is not, and the surface potential is not modified by the esterification of the alcohol group.

Identification of residues in VP2 that contribute to poliovirus neutralization antigenic site 3B.

Amino acid substitutions were placed at residues 74, 243, and 246 in capsid protein VP2 of poliovirus serotype 1, using site-specific mutagenesis methods. The proximity of these residues to those previously identified in neutralization site 3B suggests that these residues may also contribute to neutralization site 3B and potentially be under antibody selective pressure to mutate. However, sequence analyses of independent serotype 1 isolates indicate high sequence conservation at these residues, suggesting selective pressures are present within the virus to maintain sequences within these loop regions. All viable mutants display partial or complete resistance to neutralization by the site 3B neutralizing monoclonal antibodies. Cross-neutralization data with the site-specifically generated viral mutants confirm that these residues do indeed contribute to forming neutralization site 3B and also identify the participation of a new loop region within site 3B. However, many amino acid substitutions generate nonviable virus mutants and even conservative amino acid substitutions produce growth-compromised virus mutants. These data suggest that previous definition of neutralization antigenic sites by isolation of neutralization resistant mutants favors detection of viable mutant viruses with more normal growth characteristics and is inherently biased against detection of neutralization antigenic sites formed by residues critical for other stages of virus replication.

Sclerosing lymphocytic lobulitis of the breast--evidence for an autoimmune pathogenesis.

We describe 13 cases of inflammatory lesion of breast lobules in young and middle-aged women, presenting as breast lumps, with, in five cases, associated breast pain. The patient with the most florid bilateral disease subsequently developed Hashimoto's thyroiditis. This prompted us to consider an autoimmune pathogenesis for all the breast lesions. We confirm a previously documented association of such breast lesions with diabetes mellitus and review the evidence for a possible HLA association. Increased HLA-DR expression by breast epithelial cells was observed in cases available for study. Of the seven patients screened for circulating autoantibodies, three had none, one had smooth muscle antibodies, one parietal cell, one parietal cell and thyroid microsomal, and the seventh had the thyroid autoantibodies expected in Hashimoto's disease. Five of seven patients whose HLA-status was determined were HLA-DR3, 4 or 5 positive, either singly or in combination. Immunophenotypic analysis of the mammary lymphoid infiltrate showed that the majority of infiltrating lymphocytes were B-cells.

Primary health care in public health nursing.

Despite similarities between the goals of primary health care and public health nursing, several obstacles stand in the way of public health nursing's ability to perform its role in achieving an acceptable level of health for all by the year 2000. The purpose of this paper is to describe the essential components of primary health care and public health nursing, discuss obstacles in the way of public health nursing fulfilling its primary health care role, and to pose solutions at the levels of education, practice, and research for overcoming these obstacles.

A system for on-line detection and resolution of radiolabeled DNA molecules and its application to automated DNA sequence analysis.

We describe a system for the real-time detection of radioactively labeled DNA molecules in gel matrix, and we demonstrate the application of this system to DNA sequence analysis. DNA sequencing reactions prepared by the Sanger chain termination method are resolved by electrophoresis on 8% polyacrylamide gels. During electrophoresis the 32P-labeled DNA fragments are detected by solid state detectors positioned 22 cm from the top surface of the gel. This system is able to resolve a DNA sequence of 300 bases or greater. Optimized protocols that allow sequence information to be obtained from single stranded and double-stranded templates are described. A linear relationship exists between the input dpm and the integrated peak values over a 20-fold range indicating that accurate DNA quantitation is also possible using this system.

Microsurgery of ovarian endometriosis.

A long-term follow-up study of 92 patients with ovarian endometriosis treated with microsurgical techniques showed that although endometriosis may persist in many patients, only seven patients required repeat ovarian surgery. Microsurgical techniques are to be recommended for the treatment of ovarian endometriosis and the prevention of postoperative adhesive disease of the ovaries.

Imipenem versus standard therapy in the treatment of serious soft tissue infection.

Thirty-five patients with 36 episodes of serious soft tissue infection were entered in a prospective randomised trial to receive treatment with either intravenous imipenem or standard therapy, most commonly i.v. cloxacillin and benzylpenicillin. Eighteen patients received imipenem (Group 1) and eighteen standard therapy (Group 2). In Group 1, fourteen patients were cured, one improved and three failed therapy. The three failures comprised two where imipenem had to be stopped because of side-effects and one changed to standard therapy after 48 h because of lack of response. In Group 2, fourteen patients were cured, three improved and one failed therapy. This last case was due to lack of clinical response. Tolerance of both drugs was good, although eight patients in Group 1 versus none in Group 2 developed infusion phlebitis. All significant bacterial isolates were sensitive to imipenem, although several were resistant to cloxacillin and/or benzylpenicillin. No development of bacterial resistance was noted in isolated pathogens. There was no significant difference in rapidity of response or length of treatment between Groups 1 and 2. Imipenem is a safe and effective alternative to standard regimens in the treatment of serious soft tissue infection, although its side-effects and broad spectrum make it difficult to justify its use for uncomplicated cellulitis.

Pattern of injuries in survivors of a helicopter crash.

The pattern of injuries caused when a helicopter crashed after an uncontrolled descent of approximately 15 m is described. Eleven patients were admitted to hospital for further treatment and observation, and 7 were treated in the Accident and Emergency Department. One-third of the victims sustained injuries to the dorsolumbar spine. Some conditions which could improve flight safety are discussed.

Teaching in medicine: an elective course for third-year students.

An elective course titled 'Teaching in Medicine' was given to eight third-year medical students in response to the policy of the University of British Columbia medical school to expand its elective offerings. Course objectives focused on the skills that doctors need to fulfil their role of teacher of patients, students or colleagues. Instructional methods included directed reading, group discussions, microteaching, evaluation of videotaped samples of teacher behaviour, role play, demonstration and practice in developing and using audiovisual materials, and analysis of research in teaching and learning in medicine. The course culminated in each student presenting a major teaching session which was videotaped and assessed by the student and course teachers. All students rated the course as excellent. This paper describes the course and the teacher and student perceptions of it. The experience of this medical school is that a course of this nature is extremely worthwhile.

In vitro transcription of RU, a middle repetitive element of the rat genome.

Repetitive DNA sequences have been found within as well as adjacent to the rat growth hormone (rGH) gene (1,2). In vitro transcription of the rGH gene yields transcripts predominantly from a tandemly duplicated repeat (RU) in the second intron of the gene. The relative alpha-amanitin sensitivity of this transcription indicates that it is carried out by RNA polymerase III. Transcription initiates within a 5' flanking 15 base-pair repeat, and terminates within a stretch of A residues at the end of each repeat, a termination site that is notably unlike most of those used by polymerase III. Approximately 75% of the transcripts stop at the end of the first repeat; the remainder "read-through" to a homologous termination region at the end of the second repeat.

Genetically-engineered microorganisms: I. Identification, classification, and strain history.

We have argued that accurate identification of the microorganism will form a cornerstone of the assessment of potential hazard. Appropriate methodology for identification exists, and is continually under development and refinement. Organizations such as the American Type Culture Collection will perform certified identifications for relatively low cost. Thus there appears to be little reason that an organism should not be identified insofar as current microbiology allows prior to submission for PMN review. We suggest that a complete microbiological characterization be considered an essential element of an acceptable PMN. To accomplish this, however, current institutional arrangements for the protection of trade secret information needed in the process of identification may need to be improved. An accurate identification of the strain will often provide access to important information with which to evaluate its ecology, pathogenicity, biochemistry, and genetics. Specialized texts, the scientific literature, and professional consultation are ready sources of such information. However, a major effort should be made to establish a data base that can specifically address the needs of biohazard evaluation. This could be done, in part, by collecting information about the construction, and about the behavior in the environment of genetically-engineered microorganisms that are now under development and will soon be tested or used. Identification information may also eventually be useful for the formulation of hypotheses about possible modes of harm or about relative safety, based on phylogenetic relationships. This is a very difficult undertaking at present, however. Microbial taxonomy is currently in a process of radical reevaluation as new macromolecular sequence information reveals previously unsuspected phylogenetic relationships, and disturbs categorizations based on older types of traits such as morphology, etc. This means that both inferences about relative safety and about possible modes of harm from taxonomic relationships must be highly tentative based on current information. Regulatory authorities may wish to consider requesting confirmatory DNA hybridization data or other macromolecular sequence comparisons in cases where strong arguments related to safety must be made from taxonomic information in relatively poorly studied groups of organisms. Detailed strain histories would provide valuable information for safety evaluations.(ABSTRACT TRUNCATED AT 400 WORDS)
