We report a successful outcome on an acute adenovirus myocarditis treated with a 24-hour high-dose intravenous immunoglobulin (24-HDIVIG) in a 4.5-year-old girl. A postviral etiology of acute myocarditis was assessed on the basis of the polymerase chain reaction technique. Among other early markers of cardiac injury, cardiac isoform of troponin-I (cTnI) was significantly correlated to the left ventricular ejection fraction (r = -0.86, p < 0.0001). Follow-up of cTnI, which might also be correlated to the short-term outcome, allows fast, easy, and noninvasive estimation of response to the aggressive treatment with 24-HDIVIG in acute adenovirus myocarditis in children.
In Experiment 1 unilateral injection of the GABA agonist, muscimol (20 ng in 0.1 microliter saline) into the substantia nigra (SN) of rats elicited contraversive turning and a correlated transient asymmetry in responsiveness to tactile stimulation of the mouth area. On the side of the face contralateral to the injected SN they responded stronger than on the ipsilateral side, and, furthermore, they responded to tactile stimulation with a withdrawal of the lip, followed by a vigorous biting of the probe. In Experiment 2, systemic injection of apomorphine also sensitized the perioral biting response to tactile stimulation. Unilateral injection into the SN of 8 micrograms 6-OHDA blocked this response to stimulation of the side of the face ipsilateral to the lesion. The experiments demonstrate: (1) pharmacological control of sensory-motor neglect, the asymmetry being determined by the direction of turning; and (2) a possible neuropharmacological basis of the perioral biting reflex, which may be related to certain types of aggressive behaviors.
An unusual papillary meningioma in a cat with progressive cerebellar ataxia is described. Computed tomography revealed the presence of a primary tumour. Microscopically, the tumour was seen to arise from the cerebellar pia mater, forming perivascular structures composed of a single layer of cuboidal to columnar epithelial cells. By immunolabelling, the neoplastic cells appeared positive for cytokeratin, vimentin and S-100 protein. Ultrastructural examination, together with the immunocytochemical findings, indicated the mesenchymal and epithelial nature of the tumour.
Two cases of canine papillary meningioma are reported. The first animal, an 11-year-old male Boxer, presented with vestibular ataxia and the tumour was located in the left pontomedullary region. The second animal, a 15-year-old female cross-bred Miniature Poodle, presented with dementia, truncal ataxia and hypermetria, and the tumour was located in the right semilunar ganglion of Gasser. In the first case, histopathological examination showed that the papillary pattern merged with a typical syncytial meningioma, with extensive areas of necrosis. In the second case, the multilobular tumour had lobules with a purely syncytial, fibrous or transitional histological pattern, and lobules with intermingling papillary and secretory histological patterns. Both tumours exhibited histological features of malignancy, but evidence of remote metastasis was not found.
The age at the different stages of pubertal development, including menarche, was assessed by the status quo method in 1366 middle-class girls living in Athens. The initial pubertal event in the average Athenian girl was the appearance of pubic hair along the labiae (P10:8.85, P50:10.47), followed shortly by breast budding (P10:9.39, P50:10.62), and later on by axillary hair growth (P10:10.55, P50:11.63). The age at menarche was 12.58 +/- 0.07 years. It seems tht Athenian girls of 1979 have one of the earliest ages of menarche reported. Recollected age data on menarche when compared to previous data from the Greek population show a decrease of 0.46 years/decade from 1935 to 1964 and no appreciable change from 1964 to 1979. Although these data are not absolutely comparable from the methodological point of view, they may indicate that the trend in menarcheal age in Athens is levelling off.
Sternal and costal metastases from ovarian cancer are extremely rare. We present here a case of a 47-year-old woman with thoracic wall metastasis from serous-papillary ovarian carcinoma that occurred 3 years after the initial diagnosis, although the patient had received various regimens of intense platinum-based chemotherapy. Special emphasis is given to the effects of alkylating agents, such as cisplatin and carboplatin, on the pattern of tumor spread. We also discuss the possible mechanisms through which the biologic and metastatic behavior of this tumor is expressed.
The aim of this work was to record the metabolic status of the brain (pH, PCO(2) and PO(2)) during carotid endarterectomy (CEA), with the use of an intravascular multiparameter sensor (Paratrend 7) via retrograde catheterization of the contralateral jugular vein. Twenty-four patients with ASA grades II and III scheduled for CEA were included in the study. After induction of anesthesia, the contralateral internal jugular vein was punctured retrogradely and the sensor was introduced. During clamping, pH became persistently more acidotic (7.34-7.31; p < 0.05), PCO(2) was elevated (43.2-46.8 mm Hg; p < 0.05) while most of the patients showed a non-significant decrease in PjvO(2)/SjvO(2) (jv = jugular venous). Correlation with clamping time or stump pressure was not significant. Unclamping was followed by a short period (5- 9 min) of decrease in pH and elevation of PCO(2) (7.30-7.22; p < 0.05, and 48.0-52.5 mm Hg; p < 0.05, respectively). PjvO(2) was significantly elevated (51.8-58.0 mm Hg; p < 0.001) after the restoration of flow. The study suggests that local CO(2) creates conditions for compensation of flow after the application of a carotid clamp. We consider that this monitoring technique, after further validation, may provide useful information.
