[Effect of acipimox on diabetes mellitus-associated hyperlipoproteinemia].

The authors give summary on the pathogenesis of hyperlipoproteinaemia in patients with diabetes mellitus. They investigated the effects of acid nicotinic derivate acipimox on lipid metabolism in these patients. They treated with acipimox 15 patients who suffered from non-insulin-dependent diabetes mellitus, as well as II/B, IV type hyperlipoproteinaemia by Fredrickson. They investigated the effect of acipimox on the lipoprotein content and quality in the sera and uric acid and carbohydrate metabolism. The authors found decreased cholesterol, triglyceride, apolipoprotein-B, glycosilated haemoglobin, glucose and uric acid levels in the sera and they found that the atherogenic index decreased too. During the treatment they could not recognise a change in the low-density lipoprotein level of the sera while the high density lipoprotein and apo-A1 level increased.

Atypical clinical features of hypo- and hyperthyroidism in elderly age.

The clinical features of senile hypothyroidism and hyperthyroidism differ from the general characteristics of these conditions. This may be explained primarily by the changed response of peripheral tissues to thyroid hormones. Because of the atypical clinical symptoms the recognition of these conditions meets difficulties and, in general, occurs late. Therapy raises further special questions considering primarily cardiac complications and other accompanying diseases (for instance arteriosclerosis, osteoporosis, diabetes mellitus.). The authors analyse data of 171 hypothyroid and 219 hyperthyroid patients as a function of age, primary disease, and the most often occurring impulse generation, impulse conduction disorders.

Ultrasonographic test of subcutaneous arteriovenous Cimino fistula.

A total of 168 chronic dialysed patients were subjected to echography; 48% of them presented aneurysmal alterations of the fistula, with a tendency to increase in proportion to the length of the dialysis period. Two examination methods, the audial and the visual, furnish information on the flow within the fistula, the quality and lumen of the created anastomosis, blood yield, formation and position of collateral circulation. The authors emphasize the advantage of the non-invasive technique as against examination with contrast medium, the early performance of anticoagulation, and the significance of thrombectomy to spare a reoperation of the fistula.

Activation of the lipoxygenase pathway in the methionine enkephalin induced respiratory burst in human polymorphonuclear leukocytes.

In comparative studies of f-met-Leu-Phe (FMLP) and methionine enkephalin (ME) induced polymorphonuclear leukocyte (PMNL) stimulation the following results were obtained: (i) both FMLP and ME increased the intracellular killing (IK) capability of human PMNLs probably through NADPH oxidase activation, (ii) the ME-induced respiratory burst (RB) differed from the chemotactic peptide FMLP-triggered superoxide generation because the former was not accompanied by the activation of the glutathione system and the duration of the superoxide production was prolonged. The reaction was dependent on lipoxygenation, was potentiated by indomethacin (IM) and was inhibited by nordihidro-guairetic acid (NDGA), (iii) both 14C-arachidonic acid (14C-AA) release and leukotriene B4 (LTB4) synthesis of ME-treated PMNLs were elevated as compared to those of FMLP triggered cells. Our results suggest that lipoxygenation and even an increased LTB4 synthesis are involved in the ME-induced RB of leukocytes.

Effect of elastin peptides and N-formyl-methionyl-leucyl phenylalanine on cytosolic free calcium in polymorphonuclear leukocytes of healthy middle-aged and elderly subjects.

The effect of elastin peptides (kappa-elastin, KE) and N-formyl-methionyl-leucyl-phenylalanine (FMLP) on cytosolic free calcium in polymorphonuclear leukocytes (PMNLs) of healthy middle-aged (35-45 years) and elderly (greater than 60 years) patients with normal and high serum cholesterol level was investigated. The cytosolic free calcium [( Ca2+]i) elevation after stimulation with these compounds was decreased in PMNLs of the aged groups compared to the healthy middle-aged group. The guanine nucleotide binding regulatory Gi protein inhibitor, pertussis toxin (PT) prevented the enhancing effect of KE and FMLP on PMNL free calcium of healthy middle-aged subjects, but could not completely abolish the [Ca2+]i elevation in PMNLs of aged subjects.

Human model for studying in vitro the regulating function of specific and scavenger LDL receptors.

Measurements of the specific and scavenger LDL receptor activities in different disorders do not reveal all the causes and origin of hypercholesterolaemia. Studying the failure of the intracellular regulation of these receptors can give us some new information about the background of atherosclerosis. Therefore, we have tried to develop an in vitro model which is suitable for studying the specific and scavenger LDL receptor activities and their most important regulating functions, i.e. the inhibition of HMG-Co-A reductase by native LDL and the apo E secretion induced by acLDL. MDMs cultured for three days have turned out to provide a good model to test these functions.

Immunomodulating effect of low density lipoprotein on human monocytes.

Low density lipoprotein (LDL) isolated from sera of healthy volunteers in 50 micrograms protein/ml concentration induced an early adenylate cyclase activation in human monocytes followed by elevation of cGMP level. In addition, a rapid 45Ca2+ influx was also detected on addition of 25-100 micrograms protein/ml concentrations. The monocyte activating effect of LDL under in vitro circumstances was characterized by an enhanced O2 consumption, H2O2 generation and by the increased release of lysosomal enzymes such as beta-glucuronidase and elastase like protease (ELP). On the other hand, LDL diminished markedly the Fc gamma receptor (Fc gamma R) mediated rosette formation, phagocytosis and the antibody dependent cellular cytotoxicity (ADCC) of monocytes without a significant decrease in the IgG binding capability of cells. High levels of serum LDL may play a significant role in the arterial wall injury by elastase like protease as well as biologically active oxygen species released from monocytes of patients suffering from arteriosclerosis.

Age related variations of some polymorphonuclear leukocyte functions.

The age-related alterations of some biochemical processes in polymorphonuclear leukocytes (PMNL) obtained from 20 healthy aged male and 20 healthy aged female (age: 60-94 years) subjects were investigated. The basic level of luminol dependent chemiluminescence (LDCL) was increased, whereas the basal value of reduced/oxidized glutathione (GSH per GS-SG) was decreased in the aged groups. The enhancement of LDCL was more significant by PMNLs of aged males than of females. The change of both GSH and GS-SG levels during phagocytosis were diminished in the PMNLs of aged subjects. The L-alanine beta-naphthylamide specific elastase like protease (ELP) activity measured in living cell suspension was markedly increased with aging in male subjects. Therefore, it was concluded that the aging of PMNLs is partly a sex related process. The intensive release of both beta-glucuronidase (beta-G) and ELP by the PMNLs of aged subjects after in vitro treatment with calcium ionophore A23187, Cytochalasin B or low density lipoprotein (LDL) suggests that they could play an important role in some age-related disorders.

Some aspects of hypothyroidism in elderly.

The frequency of hypothyroidism does not diminish with age but there is a considerable change in its clinical pattern. The data of 169 patients treated with primary hypothyroidism were processed by means of computer analysis. They were put, in accordance with their basic diseases, into 5 groups: idiopathic (50.3%); chronic thyroiditis (26.6%); as a result of the ablative treatment of Graves' disease (17.2%); nodular goitre (4.7%) and thyroid malignancy (1.2%). According to age, the patients fell into 4 groups. III patients were between 50 and 90 years of age. In most cases the identification of the disease was belated. The usual symptoms that might lead to the consideration of hypothyroidism are edema not reacting to diuretics, anemia, and high blood lipid values. In 18 patients with a previous case history of immunothyroiditis, the cultured monocyte monolayers were examined for the activity of the specific and the scavenger LDL receptors and for the intracellular breakdown of LDL.

Lipid abnormalities in uraemic patients on chronic haemodialysis.

Patients kept on haemodialysis because of chronic renal insufficiency were investigated for lipid profiles. The cholesterol level did not differ as compared to the age-matched control, while the triglyceride level was elevated. The correlation was found between the lipid parameters, period spent in dialysis programme and level of serum creatinine and urea. In renal failures of different origin the lipid levels are in relationship with the underlying disorders.
