Tracheo-bronchial mucociliary clearance in patients with primary and secondary Sjögren's syndrome.

We determined the tracheo-bronchial mucociliary clearance (MCC) in order to evaluate a possible impairment of this function in patients affected by Sjögren's syndrome (SS) with or without overt clinical symptoms of xerotrachea. The MCC was expressed as flow rate (mm/min) and studied in 22 non-smoking SS patients (10 pSS and 12 sSS) and in 8 control subjects by specifically adapted ventilation lung scintigraphy (VLS). The MCC in the control group was 5.9 +/- 1.1 mm/min. No values were produced for MCC in 16 SS patients (8 pSS and 8 sSS) in the time interval considered and were reduced in the remaining 6 SS patients (3.3 +/- 1.2 mm/min). In all nine cases with clinical evidence of xerotrachea no values for MCC were obtained. A significant correlation was found between the MCC values and the rate of stimulated salivary excretion determined by dynamic scialoscintigraphy in the same patients (p < 0.001). These preliminary data show that the majority of SS patients studied presented with MCC impairment, always found when clinical symptoms of xerotrachea were present.

Abnormalities of magnetic resonance imaging of the central nervous system in patients with systemic lupus erythematosus correlate with disease severity.

Forty randomly selected patients with systemic lupus erythematosus (SLE) were studied by clinical and serologic parameters and magnetic resonance imaging (MRI). Abnormal MRI was found in 15/40 patients (37.5%): all 15 cases showed multiple widespread small-sized areas of increased signal in T2 in the white matter; in one of these patients MRI also displayed a large area with a reduced signal in T1 and an increased signal in T2 involving both the white and the gray matter. Among the 15 patients with abnormal MRI, only 7 had neuropsychiatric symptoms. The presence of MRI changes was highest in patients with organic type symptoms and was associated to the highest disease severity scores. A long-term follow up of asymptomatic patients would be useful to establish whether the application of MRI is appropriate for the assessment of CNS involvement in SLE.

HLA-DP genotyping in patients with systemic lupus erythematosus: correlations with autoantibody subsets.

In order to ascertain whether the HLA-DP locus plays a role in the genetic predisposition for systemic lupus erythematosus (SLE), 42 patients with SLE were typed for 17 different DPBeta haplotypes by locus specific amplification followed by allele specific oligonucleotide hybridization. Sera from the same patients were assayed for the presence of autoantibodies to Sm, RNP, Ro, La and of anticardiolipin antibodies (aCL). DPB1*0301 and DPB1*1401 were increased in patients compared with 107 healthy controls, mainly in those anti-Sm/RNP positive and aCL positive. Remarkably, DPB1*1401 and DPB1*0301 have a nearly identical nucleotide sequence, suggesting that an epitope shared by their membrane products is of major importance for the DP related susceptibility to SLE.

Prevalence and clinical associations of anticardiolipin antibodies in systemic lupus erythematosus: a prospective study.

The main objectives of the present study were to determine the prevalence of IgG and IgM anticardiolipin antibody (aCL) isotypes in systemic lupus erythematosus (SLE) in order to analyze their possible association with the clinical manifestations of the disease. Clinical features of 64 consecutive and unselected SLE patients were prospectively studied. Sera from the same patients taken during each clinical manifestation were tested for the presence of aCL. The prevalence of aCL was 43.75% for the IgG isotype and 9.4% for the IgM isotype. A strong linkage between the presence of these antibodies and the occurrence of both thrombosis and abortions was found: a weaker association with neurological events and thrombocytopenia was also demonstrated. The titre of aCL appeared to be linked with the probability of having the clinical manifestations associated with these autoantibodies. Our results suggest that thrombosis and abortion, and possibly thrombocytopenia and central nervous system involvement, may be associated with the presence of aCL at the time when these clinical events develop in SLE patients.

[Familial Mediterranean fever. Description of a case observed by us].

Familial mediterranean fever (FMF) is an hereditary disorder characterized by attacks of febrile serosal inflammation involving pleura or peritoneum and synovium, followed usually by insidious onset of amyloidosis. In other patients amyloidosis of AA-type is the only finding of the disease. This disorder is common in Jews of Sephardi and Ashkenazi ancestry, Arabs, Armenians and Turks. In this work the clinico-biological features and the therapeutical aspects of a patient, suffering from FMF, of Italian ancestry are presented.

Iron overload and lysosomal stability in beta zero-thalassaemia intermedia and trait: correlation between serum ferritin and serum N-acetyl-beta-D-glucosaminidase levels.

Increased iron storage represents a characteristic condition in patients with beta zero-thalassaemia intermedia. Iron overload is an important factor in cellular damage. Recent studies have shown an enhanced lysosomal fragility due to increased iron storage. 13 patients with beta-thalassaemia intermedia, aged 17-44 years, were studied. Both serum ferritin and serum N-acetyl-beta-D-glucosaminidase levels were evaluated in all subjects studied. A significant linear correlation (P less than 0.05) between serum ferritin and serum N-acetyl-beta-D-glucosaminidase levels were found.

Safety of flurbiprofen in subjects with G-6-PD deficiency. In vivo and in vitro results.

The safety of flurbiprofen, a new non-steroidal anti-inflammatory drug was tested in individuals with G-6-PD deficiency. The study was carried out in vitro evaluating the erythrocyte levels of reduced glutathione under basal conditions and after incubation with the drug or acetylphenyl-hydrazine. In the red cells marked with 51Cr, survival was evaluated in vivo before and during flurbiprofen administration. The study shows that flurbiprofen has no oxidating activity in carriers of the G-6-PD Mediterranean variant.

Tolmetin and G-6-PD erythrocyte deficiency: research in vitro.

In thirty-two patients, divided into four groups: normal, G-6-PD deficient, beta-thalaxemic heterozygote and carriers of both anomalies, the variations in erythrocyte GSH were studied after incubation with APH and a non-steroidal anti-inflammatory drug: tolmetin sodium. The results obtained show a considerable fall in GSH after incubation with APH above all in the G-6-PD deficient patients and in the carriers of both anomalies. On the other hand, after incubation with the drug under examination there were no significant variations in the erythrocyte GSH.
