Monocyte-to-macrophage transformation is a phenomenon which correlates with the absolute number of mononuclear cells, principally lymphocytes, contained in the culture. The addition of cimetidine to cultures of mononuclear cells from the peripheral blood of healthy volunteers enhances monocyte transformation (probably by way of blocking the H2 receptors of suppressors T lymphocytes) regardless of the absolute number of monocytes in the culture or of basal transformation rates. Removal of the lymphocytes from the cultures lowers the basal transformation rate and prevents the effect of cimetidine. The addition of histamine to the cultures causes significant depression of the monocyte transformation rate; this effect is partly offset by the concurrent addition of cimetidine. In the case of cancer patients, mononuclear cell cultures from peripheral blood fail to respond to cimetidine even though basal transformation rates are not significantly different from those of healthy controls; this suggests some intrinsic impairment of monocyte function, possibly mediated by blocking factors produced by the tumor.

The authors have studied insulin receptors on peripheral blood monocytes and insulin sensitivity, evaluated by simultaneous infusion of glucose, insulin and somatostatin in 10 control subjects and in 20 obese patients with normal glucose tolerance. The obese patients have been divided into two groups, normo (NO) and hyperinsulinemic (HO), according to the total insulin response during OGTT. We considered HO patients with insulin response higher than M + 2DS of controls. Obese patients showed, in comparison to the controls, a lower specific binding and higher degree of insulin resistance. The subdivision of obese patients allowed us to distinguish two groups. The first was characterized by basal hyperinsulinemia, normal insulin response to the stimulus, reduced number of insulin receptors and normal or slightly reduced sensitivity. The second group showed high basal and after stimulus insulinemic values, reduced number of insulin receptors and high level of insulin resistance. When we compared the two groups of obeses we found that the first has a shorter duration of obesity and lower blood glucose values after OGTT. However both groups show the same reduction of insulin bound and the same degree of basal hyperinsulinemia. These data suggest that a reduction of insulin receptors is not the main factor responsible for insulin resistance in obesity. Furthermore, the presence of basal hyperinsulinemia and normal insulin sensitivity in our first group suggests that the modification of basal insulin concentrations is not dependent on the presence of insulin resistance.
We investigated the psychoneuroendocrine and emotional correlates of the natural stress situation of human labor. State anxiety, subjective pain, plasma ACTH, peripheral plasma beta-lipotropin (Beta-LPH), beta-endorphin (Beta-EP), and met-enkephalin (Met-Enk) were serially evaluated at six predetermined time points before, and after labor in a sample of 14 women with normal pregnancies. State anxiety and subjective pain showed a progressive increase during labor, with a levelling during the final stage. Plasma Beta-EP and ACTH showed a similar progressive increasing from baseline until the end of labor. Beta-LPH showed no significant modification. Met-Enk remained at nearly baseline values throughout labor, with a marked progressive rise in the postpartum stage. The findings of this study seem to confirm the role of plasma Beta-EP as a stress hormone. Possible relationship between pain and anxiety curves and plasma Beta-EP are discussed in light of psychobiological studies on stress, the opioid system and analgesia. Plasma Met-Enk, according to our findings, should probably not be regarded as a stress hormone. Its rise in the postpartum stage might be as one of the psychoneuroendocrine mechanisms maintaining elevated prolactin levels during lactation.

Serum glycerol and NEFA content variations are examined before and after labor in obese and normal weighing women (35 subjects). Blood glycerol and NEFA are shown to increase before delivery. Glycerol values are shown to drop to normal immediately after delivery, while NEFA values diminish to a lesser extent. Statistical analysis shown that blood glycerol increase could be pregnancy-dependent in both normal weighing and obese women, but that NEFA increase could be pregnancy-dependent in normal weighing women only. Obesity increases blood glycerol and NEFA concentration considerably, thus masking the effects of pregnancy.
Hydroxyurea is a well known enzyme urease inhibitor. Bacteria strains producers of enzyme urease can be the cause of struvite calculosis. Clinical studies have shown that treatment with hydroxyurea can produce acid urine in patients suffering from struvite calculosis. In this work antiurease and antibacterial activity of hydroxyurea was valued in vitro against Proteus mirabilis strains. Our results seem not to agree with the ones obtained in vivo.
Gentamicin and adriamycin were tested against eight strains of Proteus mirabilis: using the cellophane transfer technique, combinations of these antibiotics were screened for bactericidal activity. On mice bearing Ehrlich ascites carcinoma, the antitumoral effect of a combination of gentamicin and adriamycin was determined. Antagonism occurred with the combination in bactericidal activity and not in antitumoral activity.
A selected case file of 32 patients with carcinoma of the lung which had passed the stage of surgical cure was randomized in 3 arms and treated with different schedules: regimen A, adriamycin plus cyclophosphamide; regimen B, the BACON combination, slightly modified; regimen CVP, cyclophosphamide plus vincristine plus prednisone. Response rate and survival curves were analyzed. Statistical evaluation showed a significant increase in survival of B versus A, but not versus CVP, while other factors such as the initial performance status and the response rate seemed to have a marked influence on survival time. No significant correlation was observed with different histiotypes within each regimen. All three regimens compare favorably with patients who received only supportive treatment.
