[Optical-acoustical analogous biofeedback conditioning in the treatment of fecal incontinence in childhood].

This is a report on the treatment of anal incontinence by biofeedback conditioning. Since August 1982 9 children with incontinence (8 rectoanal agenesia, one Morbus Hirschsprung) were treated by simultaneous optical and acoustical analogous biofeedback conditioning. Training of sensitivity and voluntary contractions were performed and controlled electromanometrically. According to the pressure waves electrical alterations were transformed and feedback visually and acoustically. Results of the training of the maximal voluntary contraction are presented and the superiority of this method for the coordination of the anorectal sphincter function is stressed.

[Routine sonography as a screening method for occult abdominal diseases in children].

At the department of pediatric surgery of the University of Mainz each child was routinely sonographically examined between September 1981 and April 1982. 582 children were examined. Pathological findings were detected in 4.8% of all cases, leading to a change in operative treatment in 45.4%. To determine more precisely the diagnosis of a disease in children undergoing surgery, routinely sonographically examination are recommended as screening method.

Sonographic diagnosis of intussusception in childhood.

The diagnosis of intussusception was established in 26 children by sonography alone. In 23 cases barium enema confirmed the diagnosis; two cases because of longstanding intussusception and one case after intestinal anastomosis were confirmed by surgery alone; in two additional cases barium enema ruled out the sonographically suspected intussusception. No positive finding was missed by sonography, which proved to be an accurate method for the diagnosis of intussusception. The sonographic findings of idiopathic intussusception and intussusception caused by lymphosarcoma are presented.

[Rectal carcinoma in childhood].

The authors report on the case of a 12-year-old girl on whom surgery was performed for primary carcinoma of the rectum with a case history of one year. The tumour had already infiltrated the vagina. Abdominoperineal amputation of the rectum was performed. An extended tumour relapse and metastases in the liver were found on relaparotomy which had to be performed three years later The prognosis was, therefore, unfavourable indeed. Numerous publications prove that carcinoma of the rectum must be given differential diagnostic consideration even in childhood, especially if there are unclear deposits of blood in the faeces in conjunction with abdominal pain and loss of weight. Usually, the "fatal pause" between the first symptoms and the correct diagnosis is too long. Once the disease has progressed to an advanced stage, carcinoma of the rectum has a very unfavourable prognosis even in children. Simple digito-rectal examination can decisively improve the survival rate.

[Diagnosis and therapy of fibular ligament injuries in the ankle joint in children].

Turn fibular ligaments of the ankle go often undetected when not accompanied by osseous lesions. The significance of injuries to the major supporting ligaments of the ankle is underestimated in children. Next to the clinical findings stress radiographs of the ankle joint in two projections are mandatory. Measurements of anterior and lateral displacement under stress and displacement index need to be compared with the healthy ankle. Diagnosis of a fibular ligament rupture is based on clinical findings and on differences between the measurement results (injured compared to healthy) by way of three different techniques. Rupture of a fibular ligament is an absolute indication for surgery--even in children.
