Systemic lupus erythematosus in Europe at the change of the millennium: lessons from the "Euro-Lupus Project".

The "Euro-Lupus Cohort" is composed by 1000 patients with systemic lupus erythematosus (SLE) that have been followed prospectively since 1991. These patients have been gathered by a European consortium--the "Euro-Lupus Project Group". This consortium was originated as part of the network promoted by the "European Working Party on SLE", a working group created in 1990 in order to promote research in Europe on the different problems related to this disease. The "Euro-Lupus Cohort" provides an updated information on the SLE morbidity and mortality characteristics in the present decade as well as defines several clinical and immunological prognostic factors.

Harvesting peripheral blood progenitor cells from healthy donors with a short course of recombinant human granulocyte-colony-stimulating factor.

A short-course administration of non-glycosylated granulocyte-colony-stimulating factor (G-CSF) was investigated in 68 healthy donors (HDs) in order to collect > or = 4 x 10(6) CD34+ cells per kilogram of recipient's body weight. G-CSF was given at 10 microg/kg per day administered in two divided doses for 3 days. Leukapheresis was scheduled on day 4, 12 h after the last dose of G-CSF. A median of 35.6 circulating CD34+ cells microL(-1) (range, 3.1-185) was found on the day of leukapheresis. This allowed a median collection of CD34+ cells of 4.2 x 10(6) per kilogram of recipient's weight (range, 1.0-17.4). One single procedure was sufficient to reach the target level of CD34+ cells in 36 (53%) of 68 donors; significant correlations were found between the number of CD34+ cells collected on day 4 and the patient's sex, body-weight and volume of blood processed. A retrospective analysis was made with a historical group of HDs collected on day 5. The day 5 schedule allowed a more consistent achievement of the target cell dose with one leukapheresis (P = 0.005) and resulted in the initial collection of a significantly larger number of CD34+ cells (P = 0.006).

Infectious complications in breast cancer patients undergoing peripheral blood stem cell transplantation: a single center retrospective analysis towards outpatient strategy.

Infectious complications were retrospectively analyzed in 129 transplants, performed in 90 patients, to identify characteristics that qualify breast cancer patients for outpatient-based PBSCT. Thirty-one cases (24%) did not develop fever. Of the remaining 98 cases, 84.7% developed fever during severe neutropenia. On univariate analysis, disease stages II-III, first PBSCT, mucositis grades II-IV and the use of two alkylators were associated with a higher risk of fever development. The latter two factors also affected fever occurrence on multivariate analysis. A longer median time to fever onset was observed in patients conditioned with single as compared to double alkylating agent-containing regimens (respectively 8th vs 6th day, P < 0.00001). As compared with metastatic breast cancer (MBC), high risk breast cancer showed a 2.3-fold increased risk of developing early fever during neutropenia (CI 2.3-3.8), remaining the only variable still significant on multivariate analysis (P = 0.0039). Combination antibiotic therapy was equivalent to single agent therapy. Patients suffering from microbiologically documented fever were at higher risk of undergoing second-line antibiotic therapy. In conclusion, MBC patients treated with a conditioning regimen containing only one alkylating agent and adequate prophylaxis for mucositis may qualify for outpatient-based PBSCT on the basis of a lower risk of infection.

Severe thrombotic microangiopathy: an infrequent complication of bone marrow transplantation. Gruppo Italiano Trapianto Midollo Osseo (GITMO).

Thrombotic microangiopathy (TMA) usually occurs during the first weeks following transplantation in the setting of systemic infections or graft-versus-host reaction. However, some cases without any evidence of other complications or after autologous transplantation have been reported. Transplant-associated TMA (BMT-TMA) incidence ranges from 0% to 74%, possibly due to different diagnostic criteria. The GITMO Group provided the opportunity to retrospectively study 4334 consecutive Italian patients who received bone marrow transplants (1759 allogeneic and 2575 autologous BMT), during the 1985-1995 period. The present report focuses on patients with severe TMA requiring specific treatment. We identified nine cases of TMA as a complication of allogeneic BMT (0.51%), whereas three patients developed the syndrome after ABMT (0.13%); four of the 12 patients were not receiving CsA at the time of TMA onset. Finally, it is noteworthy that TMA occurred in seven patients as a late complication (up to 90 days after BMT). Despite intensive treatment, five of the seven patients with thrombotic thrombocytopenic purpura died. One death was observed among the five cases with hemolytic uremic syndrome.

In vitro drug-induced cytotoxicity predicts clinical response to fludarabine in B-cell chronic lymphocytic leukaemia.

We investigated the possible value of an in vitro drug-induced cytotoxicity assay in predicting clinical response to fludarabine (FAMP) in chronic lymphocytic leukaemia (CLL). The median FAMP-LD50 values for cases designated as complete response (CR), partial response (PR), no response (NR) and progressive disease (PD) were 1.43, 2.05, 24.8 and 77.9 microg/ml, respectively (P=0.013). A significant lower mean FAMP-LD50 value was observed in the in vivo responding cases as compared to non-responding ones (3.0 +/- 0.82 SEM v 42.3 +/- 12.6 SEM, P=0.001). Only FAMP-LD50 < or = 2.5 microg/ml and more than four FAMP courses significantly influenced the response rate in univariate and multivariate analyses. The MTT assay may be of value in predicting clinical response to FAMP in CLL patients.

Outcome of patients with acute myeloid leukemia who failed to respond to a single course of first-line induction therapy: a GIMEMA study of 218 unselected consecutive patients. Gruppo Italiano Malattie Ematologiche Maligne dell'Adulto.

The outcome of a cohort of 218 consecutive patients who failed to respond to a single course of standard daunorubicin plus ARAC (three + seven) induction regimen has been retrospectively evaluated to assess the characteristics of this group of AML patients and the effectiveness of second-line induction programs. Seventy-four of the 218 patients (33.9%) attained complete remission with salvage chemotherapies. The multivariate analysis of pretherapy characteristics of the patients showed that peroxidase positivity and age were the most important factors in determining whether or not the patient would have a favorable response to second-line induction regimen. In addition, comparison of marrow characteristics at diagnosis with those of marrow after the first-line therapy (marrow leukemic index, MLI) provided the greatest differences between second-line CR and resistant patients. Finally, peroxidase positivity and MLI predicted for remission duration and overall survival. Allogeneic BMT, however, appeared the most important factor for survival and event-free survival of remitting patients. These results are of importance when considering that better defined prognostic factors provide an objective rationale for selecting appropriate strategies for the treatment of patients who do not respond to a single course of induction regimen.

[Arthroplasty of the knee using total condylar prosthesis. Long term results (10 to 17 years) and survival analysis].

INTRODUCTION: This study refers to the first 76 Total Condylar knee prostheses that were implanted in 66 patients, with a minimum follow-up of 10 years. MATERIALS AND METHODS: There were 54 women and 12 men with an average age of 65 years (range 45-81). The diagnosis was osteoarthrosis in 63 knees and rheumatoid arthritis in 13. We were able to review 54 prostheses with an average follow-up of 13 years (10-17) using the rating system of the Hospital for Special Surgery and the Survivorship analysis. RESULTS: The results were excellent and good in 72.5 per cent knees and fair and poor in 22 per cent, 3 knees (5.5 per cent) had been revised for deep infection in 2 cases and for aseptic loosening in 1. Pain at rest was absent or mild in all but one patient; walking pain was absent in 32 knees (63 per cent), mild in 12 (23 per cent), moderate in 5 (10 per cent) and severe in 2 (4 per cent). Average range of motion was 89 degrees, ranging from 40 degrees to 120 degrees. A flexion contracture greater 5 degrees was seen in 8 knees (16 per cent). Instability in the frontal plane tested in full extension was more than 5 degrees in 10 patients (19.5 per cent). A significant deterioration of the results occurred in the last 4-5 years, since in 1988 excellent and good results were still 82 per cent. A loss of postoperative alignment of 5 degrees or more was observed in 11 knees: aseptic loosening was identified in 2 of these cases and deformation of the tibial component was seen in 1 knee. In the remaining 8 knees we observed increased lateral instability, compared to the postoperative evaluation. In this group of 8 knees we measured in the AP view the thickness of the tibial component on the lateral and medial side. We found polyethylene wear on the medial side in 4 cases. Ten of the 11 knees with loss of alignment had an early postoperative femoro-tibial axis less than 5 degrees. We observed radiolucent lines in 18 patellar and 27 tibial components. A width greater than 2 mm, correlated to an extension to 5 or more zones, was seen in 3 tibial components with aseptic loosening. Survivorship analysis using aseptic loosening, mechanical failure of the polyethylene and deep infections as end point gave a cumulative success rate at 10 years of 92 per cent% with 95 per cent confidence interval ranging from 85.1 to 98. DISCUSSION: Our study reports a lower percentage of excellent and good results if compared to other series of Total Condylar prosthesis with a comparable follow-up. A significant deterioration of the results occurred in the last years, but this was often not related to the knee itself but to poor general conditions in some patients. In this series there is a relevant number of knees with a post-operative alignment that today we don't consider correct and all the aseptic loosenings occurred in knees with a tibial component positioned in varus. CONCLUSION: Survivorship analysis gave a cumulative success rate comparable with those reported in other studies and confirmed the durability and longevity of this model of prosthesis.

[Anti-ENA antibody determination in various connective tissue diseases: comparison of 2 detection techniques].

The authors compared two different techniques for the detection of anti-ENA antibodies (anti-Sm, anti-RNP, anti-SSA, anti-SSB) in several different connective tissue diseases, using double immunodiffusion (ID) and a new membrane-based enzyme immunoassay. The aim of the work was to research the assay sensibility and specificity of the results obtained with the two different methods. Notwithstanding the fact that the two techniques are equivalent in their performance, the recombinant DNA appears to have greater sensibility and less specificity compared to ID. The former requires less time of execution although it presents difficulty in interpreting low scale positive results. Although the number of sera examined was low, the authors assert that the recombinant DNA technique will require further development.

Comminuted articular fractures of the distal radius. Results of conservative treatment.

Fifty-nine comminuted articular fractures of the distal radius which had received conservative treatment were reviewed after an average of 3 1/2 years. The results--excellent in 15 patients, good in 30, fair in 12, and poor in 2--were closely linked to the quality of the articular reduction. In patients with a residual malalignment greater than 2 mm, a relatively high incidence of both degenerative arthritis (31%) and unsatisfactory results (56%) was found. The presence of a "die punch" fragment often precluded acceptable fracture reduction and was therefore responsible for many poor clinical and radiographic results. The quality of the extraarticular reduction proved to be of little importance to the final outcome.

[The results of a resection intervention in isolated fractures of the radial head].

The results of radial head excision were studied in 33 patients with isolated fracture of the radial head. The follow-up period ranged from 1 to 19 years (mean 9.5 years). Overall results were excellent or good in 25 patients (76%) and fair or poor in 8 patients (24%) with functional limitation of the elbow. Range of motion was greatly limited in those patients who had a myositis ossificans or a radio-ulnar synostosis. Periarticular ossifications were observed in 17 elbows (54%) and subluxation of the inferior radio-ulnar joint in 20 wrists (60%). Excision of the radial head is the treatment of choice for comminuted isolated fractures of the radial head, even if in some patients limited elbow function should be expected.

Prevention by hypothermic cap of antiblastic induced-alopecia.

Adriamycin, one of most effective antitumor drugs, is characterized by high toxicity. The adriamycin-induced alopecia is frequent (80-97% of patients) and rapidly shows itself (already after first treatment). Such toxic effect, so unpleasant particularly to female patients, was largely prevented in our experience (Gynecological Oncology Service) with the utilization of hypothermic caps. These caps, reducing the drug absorption above the scalp, allows limiting the hair loss. The result obtained (reduction of severe alopecia in 34% of patients) encourage the use of such a method to improve the life quality of patients.

Role of diet in endometrial cancer patients.

The hypothesis of a correlation between diet and cancer is widely supported by several epidemiological and experimental studies. Certainly the exact mechanism of this intervention is still unknown; nevertheless in the tests of a carcinogenesis the direct action of some food is recognised, whereas the indirect action of others should occur through modifications of immune and endocrine systems. Many studies conclude that a high intake of fats and animal proteins appeared to be associated with the etiology of some endocrine related neoplasms, such as the cancer of prostate breast endometrium and ovary. We studied the dietary practices and some constitutional parameters of women affected by endometrial cancer and control women. We did not find a significant difference between the two groups related to height. Conversely the daily intake of lipids and particularly of proteins, glucides and calories turned out much higher in the group of patients. Moreover we found a significant difference related to weight; obese women were more numerous in the group of patient. The above results confirm the importance of a right diet in the prevention of this neoplasm.

Skin necrosis due to antiblastics (procedures of prevention and therapy).

Among the toxic effects of antitumor drugs the injury for extravasation occurs too. The kinds of damage which result can achieve dramatic features with serious consequences on the psychophysic activity of patients who once tried other kinds of toxicity following chemotherapy. As the extravasation is caused by way of use and by drug-giving methods, we present this accident: it is necessary to observe rigorous rules of procedure during infusion and, if extravasation occurs, to make use of efficient drugs and physical methods. A recent case of extravasation occurred, at our Gynecological Oncology Service, in a patient who carried out chemotherapy without hospitalization. This case is here proposed and discussed; we think that the rarity of this accident is in our experience, due to the several precautions we observe during the infusion of drug.
