[Migraine: dynamics of psychophysiologic pattern].

85 patients with migraine without aura and 20 healthy individuals were examined between the attacks, before the attack, during and after it (1-2 days). The examination included clinical-neurological analysis; psychological Spilberger's and Beck's tests, scale of alexithymia and complex algesic questionnaire; recording of the conditional negative wave; determination of nociceptive flexor reflex; recording of trigeminal evoked potentials; spectral analysis of the heart's rhythms; polysomnographic study. Psychophysiologic pattern characteristic for all the patients was established. Psychological and neurophysiological parameters changed synchronously in the periods before the attack, during and after it. Before the migraineous attack a specific pathologic integration increased and presented maximally (the activity of antinociceptive system weakens anxiety increased, tension of catecholaminergic systems grew, hyperactivity in trigeninal sphere elevated, tolerance to the pain decreased). A clear tendency to the reversibility of these changes was observed after an attack. A dynamics revealed in the psychophysiological pattern could serve as a predictor of a migraineous attack and open some perspectives for prophylaxis of the algesic attack.

[Treatment of fibromyalgia].

The paper reports the results of therapy of 23 patients with fibromyalgia (FM). Tetracyclic antidepressant lerivon, was administered to group 1, nonsteroid antiinflammatory (NSAI) preparation nurofen to group 2 and phototherapy (exposure to bright white light) was used in group 3. Clinical effect in the form of a decrease of both the intensivity of algesic syndrome and autonomic manifestations as well as improvement of night sleep were clearly seen in group 1. Manifestations of both anxious and depressive disorders were less pronounced. Treatment by Nurofen resulted in slight decrease of intensivity of pains but didn't lead to pronounced alterations of emotional sphere. Administration of either Lerivon or Nurofen promoted the increase of pain thresholds (according to the data of nociceptive flexory reflex). The data obtained testified the necessity of complex therapy of FM patients including administration of antidepressants and analgetic drugs of NSAI group. Dynamic polysomnographic examination of patients from group 3 revealed the increase of total sleep duration, decrease of the time of falling asleep, the latent period of the phase of the fast sleep, activated movement index, intensivity of movements and the time of being awake in the sleep. The conclusion was made that it was worth while to use phototherapy as alternative, nonmedicine method of phothotherapy.

[Status of the mechanisms of autonomic regulation in Raynaud's disease].

Patients suffering from Raynaud's disease underwent clinicoelectrophysiological studies for the status of the segmental and autonomous nervous system. Based on the distribution of the patients' group with Raynaud's disease according to the type of the disease course and intensity of Raynaud's phenomenon, the author shows a relationship between the clinical manifestations of the leading phenomenon and disorders of sympathetic conduction in the limbs. That made it possible to suggest that peripheral vegetative neuropathy together with the well-defined psychovegetative syndrome may be implicated in the pathogenesis of Raynaud's disease.

[Cardiovascular tests in various forms of pathology].

Quantitative estimation of the status of the autonomic nervous system is a research priority in neurology. One of the approaches to its solving lies in the use of cardiovascular tests. The authors brief the mechanisms of the tests, provide their standards. Show the characteristic patterns of the cardiovascular tests in different diseases of both organic and psychogenic nature. Taking into consideration the simplicity, accessibility, and noninvasive nature of the tests, it is recommended that the cardiovascular tests may be employed for estimating the status of the autonomic nervous system in a wide range of diseases of both neurological and somatic origin.

[Electrophysiological analysis of the functions of the sweat gland fibers in various diseases].

The authors relate the data on vegetative sweating fibers studied by the method of evoked skin sympathetic potentials in patients with diabetes mellitus, Charcot-Marie atrophy, spinal amyotrophy, Guillain-Barre syndrome, essential hyperhidrosis, and in those with Raynaud's phenomenon and thyrotoxicosis. A remarkable decrease of conduction by sweating fibers was revealed in patients with diabetes mellitus, Charcot-Marie atrophy and Raynaud's phenomenon; a moderate decrease was shown by the patients with Guillain-Barre syndrome and by those with spinal amyotrophy. The conclusion is made about the high information content of the method.

[Evaluation of the autonomic innervation of the pupil].

The purpose of the study was to examine vegetative innervation of the pupil in different diseases associated and not associated with vegetative disorders. In addition to the ++clinico-neurological method, the main method of examination was to study the pupillary cycle (determination of the time of the pupillary cycle (TPC) with the aid of a slit lamp). Fifty healthy test subjects and 262 patients suffering from different somatic and neurologic diseases were examined. The patients were divided in 9 clinical groups: spinal amyotrophy, neural Charcot-Marie amyotrophy, Guillain-Barre syndrome, diabetes mellitus, thyrotoxicosis, hemicrania, Raynaud's disease, idiopathic hyperhidrosis, vegetative crises. The data obtained demonstrate varying grades of the rise of the TPC in the indicated diseases, which reflects varying degrees of vegetative innervation of the pupil insufficiency. However, no well-defined correlation was obtained between the intensity of the psychovegetative syndrome and the TPC. Therefore, it has been shown that the method of determining the TPC is unsophisticated and noninvasive and can be successfully used in different forms of pathology to identify vegetative insufficiency at different stages of the disease.

[The epidemiology of autonomic disorders--the autonomic dystonia syndrome--and its characteristics in cerebrovascular pathology].

To study the prevalence of vegetative dystonia (VD) and the role it plays in the pathogenesis of cerebrovascular diseases (CVD), 8,500 population of different regions of this country and patients with the main forms of CVD underwent an epidemiological study with the aid of a specially devised questionnaire. It is shown that (1) VD is highly prevalent among the healthy population; (2) the intensity of VD depends on the region of living, sex, age, occupational factors; (3) VD in patients with CVD is characterized by high intensity, especially in initial manifestations of cerebral circulatory disorders, and by a particularly severe course in menopausal women.

[Raynaud's phenomenon (a clinico-physiologic analysis)].

On the basis of examination of three groups of patients with Raynaud's phenomenon (RP) (Group I was composed of patients with an idiopathic forms of RP; Group II, of those with neurovascular syndromes of cervical osteochondrosis; Group III, of those with systemic scleroderma) which involved a series of vegetative tests (measurement of the heart rate, the cold test, Aschner's test, determination of autonomic provision of intellectual and other types of activity), authors draw a conclusion about the presence of a close correlation between RP and the psychovegetative syndrome and dysfunction of nonspecific systems of the brain. On the other hand there are differences in the psychophysiological organization of the idiopathic form of RP vs. Raynaud's syndrome, which should be taken into account in the treatment of angiotrophopathic disturbances.

[Efficacy of treating patients with vegetative and vascular crises of a neurotic nature (comparative electrophysiologic study)].

On the basis of clinical, electrophysiological, psychologic and vegetative parameters, the authors treated 66 patients with vegetovascular crises of neurotic nature using acupuncture, beta-adrenoblockers, psychotropic means and a combination of beta-adrenoblockers with psychotropic drugs. Acupuncture led to changes in all the aforegoing parameters. Psychotropic therapy reduced activating influences, predominantly in the "ascending" direction, anaprilin led to the maximum inhibition of the vegetative component of the crisis. With all methods of treatment emotional-personality alterations showed the greatest degree of normalization. This confirms the correctness of the term "psycho-vegetative" syndrome which emphasizes the leading role of mental disorders in the genesis of vegetovascular dystonia in neurotic patients.
