[Reopening of infarct-occluded vessel by transluminal recanalisation and intracoronary streptokinase application (author's transl)].

Coronary angiography was performed 2 hours after onset of complaints in a 57-year-old patient with acute anteroseptal infarction. Using a wire spiral in the coronary catheter the acutely occluded anterior interventricular branch was reopened. With intracoronary streptokinase (120 000 units in 50 minutes) the recanalisation canal was enlarged. The clinical picture and electrocardiographic findings improved under the influence of treatment. An aorto-coronary bypass operation was performed on the following day. The post-operative angiogram showed marked improvement of left ventricular function. The results indicate that this intervention prevented widespread necrosis.

[Sclerosing lipogranuloma of male genitalia (author's transl)].

The term "sclerosing lipogranuloma" was coined in 1950 to name a peculiar granulomatous fatty tissue reaction that according to Smetana and Bernhard represents a "local reactive process which sometimes follows injury to subcutaneous fat tissue". Later other investigators have pointed out that injections of exogenous lipids probably cause this lesion. A case of sclerosing lipogranuloma in the scrotum is presented, in which paraffin hydrocarbon could be identified. As prior to the appearance of the lipogranuloma a paraffin containing ointment had been applied locally for three weeks a percutaneous resorption of paraffin as a possible etiology is discussed together with a review of the literature.

[Aspiration biopsy cytology in malignant tumors of the testis? (author's transl)].

Fine-needle aspirates and tissue prints of 23 malignant tumors of the testis were obtained after surgery. In all cases malignancy was recognized by cytological examination. Only in two cases there was a discrepancy between cytological and histological classification. Malignant teratoma undifferentiated (MTU) was cytologically diagnosed in two cases verified as malignant teratoma intermediate (MTI) by histology. The value of fine-needle biopsy in diagnosis of malignant tumors of the testis is discussed.

[Atresia of the larynx, a rare cause of postnatal respiratory distress (author's transl)].

An infraglottic atresia of the larynx with normally developed vocal cords necessitated postnatally an emergency tracheotomy after intubation attempts had failed. Laryngeal atresia is a very rare malformation. As in our example, it is in most cases accompanied by a tracheo-hypopharyngeal fistula which has to be interpreted ontogenetically. The survival chances of the newborn with this malformation are generally very poor.

Autopsy

[Familial hemophagocytic reticulosis (Farquhar) (author's transl)].

An account is given of six cases of familial hemophagocytic reticulosis (familial lymphohistiocytosis). After a period of illness varying from 2 weeks to 7.5 months the infants studied died with pancytopenia, fever and hepatosplenomegaly. Histologic examination revealed three characteristic features: Lymphocyte infiltration of the organs, reduction of the lymphatic and hematopoetic system, histiocytosis with erythrophagocytosis. The etiology of this disease remains obscure. Congenital allogeneity may be a possibility, but further immunologic investigations would be necessary to support this hypothesis.

[Giant cell tumor of tendon sheath in association with synovialchondroma. An unusual tumor-combination at the temporomandibular joint (author's transl)].

A tumor at the temporomandibular joint is reported, which so far has not yet been described. Histologically it turned out to be a combination of synovialchondroma and giant cell tumor of tendon sheath. By reason of the microscopic picture and the course of 2 1/2 years without relapse it is a benign tumor. In postoperative x-rays of the mandible a thickening and sclerosis as well as cystic areas of increased translucence at the place of removal of the tumor were found, which had already existed preoperatively in a lower degree.

Giant Cell Tumors

[Cardiac tamponade after perforation of ventricle by central venous catheters and transvenous pacemakers (author's transl].

Two cases of cardiac tamponade after perforation of the right ventricle by central venous catheter and transvenous pacemaker are reported. The apex of the right ventricle represents a "locus minoris resistentiae" for a perforation because of its thin wall and frequent infiltration of fat with relaxation of the small muscle bundle. An analysis of 41 heart perforations by central venous catheters and 47 by transvenous pacemakers from the literature demonstrates the particularly high mortality of the perforation by a central venous catheter referring to the possible production of hydropericardium. We think that extreme carefulness in the use of catheters, knowledge of the danger and vigorous treatment could reduce the mortality.

Aged

Intramural esophageal diverticulosis.

An investigation of the formal patho-genesis of the so-called intramural esophagus diverticulosis is carried out on a case confirmed by autopsy. The roentgenological diagnosis corresponded morphologically with a proliferation of sub-mucous esophagus glands and a dilatation of their outlets, a portion of which displayed platelet epithelial metaplasia. Diverticle formation is feigned by widening, predominantly of the outlets, which could be traced into the muscular wall layers. In the present case esophagitis with attendant inflammation along the outlets lead to fistula formation into the anterior mediastinum with attendant mediastinitis. Aspirations were due to constrictions of the esophagus lumen. The patient came to exitus as a result of purulent abscessing aspiration pneumonia complicated by pulmonary artery emboli.

Diverticulum, Esophageal

[Endocrine and morphological investigation in undescended testis (author's transl)].

Determination of the basal and LH-RH-stimulated luteinizing hormone (LH) levels as well as testicular morphology in patients with surgically treated (n equals 112) and non-treated (n equal 96) undescended testes gave the following results: Unilateral of bilateral undescended testes are probably a form of primary, secondary or tertiary hypogonadism. In a high percentage abnormal basal and stimulated serum-LH values can be demonstrated. The testosterone values are within normal limits. The gonadal tissue damage in the abnormally situated testes of prepubertal boys seem to be congenital. With increasing duration of the malposition, particularly after the onset of puberty, a secondary tissue degeneration in the dystopic testis occurs. The damaging influence on the testicular tissue caused by the abnormal position seems to take place mostly at the end of prepuberty. A decrease in the number of spermatogonia in the tubuli of dystopic testes during the first 2 years of life is physiological. A surgical repositioning of the dystopic testis into the scrotum within the first 2 years of life is not indicated. Damage of gonadal tissue in the scrotal testis caused by the dystopic testis in unilateral maldescent could not be demonstrated. The optimal space for orchidopexy is between the third and fifth year of life.

Adolescent

[The left ventricle in chronic cor plumonale (author's transl)].

The volume of both ventricles was estimated in 25 hearts with chronic cor pulmonale. Also, the weight of the left ventricle, including the whole septum and the free part of the right ventricle, was determined to be 100 g. Distinct hypertrophy of the right ventricular part of the septum could be demonstrated in all cases. A weight increase of the left ventricle, including the whole septum, is frequently caused by the hypertrophic ventricular part of the septum. Th relative weight of the right ventricular portion )calculated according to Muller's method) was subtracted from the weight of the left ventricle plus the whole septum. After this weight correction, a hypertropyh of the left ventricle could only be demonstrated in cases with hypertension or renal arterio= and arteriolosclerosis. In one case of left ventricular hypertrophy, hypertension could not be ruled out. In most of the cases a distinct dilation of the left ventricle could be found. The highest degrees of dilation were found in patients with brohchiectases of the lungs. Our results indicate that the structural dialation of the left ventricle may be due to an increased shunt-volume by bronchopulmonary anastomoses.
