[New technics for producing inhalation aerosols--initial experiences and results with the piezoelectric inhalation device].

In the treatment of chronic obstructive airway diseases, the inhalative application of medicaments with a bronchodilatory effect is given preference throughout the world. In addition to inhalation solutions and powder capsules, so-called metered-dose aerosols have formed an established part of the treatment plan for approximately 30 years now. With the piezoelectric inhalation device, a fully portable ultrasonic atomiser that is small enough to fit within the pocket, has been developed for the first time. In this device, the piezoelectric effect is utilised to produced, pressure-free, microfine aerosols. Within it, an accurately metered volume containing the therapeutic dose is applied to an atomising element oscillating in the ultrasonic range. The particle spectrum generated with the piezo-system corresponds, in terms of size, to that of metered aerosol devices, and the mean particle diameter is approximately 2.1 microns. The atomisation process takes approximately one second.

[Pilot study of the action of Doqualast on dietary hyperuricemia in volunteers].

For eight days 8 healthy volunteers were treated with a formula diet and additional 1680 purine base per day. Beginning with day 5, doqualast was administered orally in a dosage of 200 mg t.i.d. Serum uric acid level was lowered by doqualast from 7.84 +/- 0.9 mg/dl to 5.09 +/- 0.8 mg/dl (p less than 0.01). Uric acid clearance increased from 7.6 +/- 2.5 ml/min to 11.5 +/- 2.4 ml/min (p less than 0.05). Excretion of uric acid in urine increased from 622 +/- 231 mg/d to 890 +/- 233 mg/d at the end of therapy. Total cholesterol in serum was lowered from 222 +/- 70 mg/dl to 159 +/- 48 mg/dl (p less than 0.05). The new uric acid lowering drug doqualast was well tolerated clinically. No adverse effects on biochemistry parameters were observed.

[Arterial supply to the anterior segment of the eye. Radioanatomical study of a series of 25 human eyes].

We studied the arterial vascularization of the anterior segment of 25 cadaver eyes. Arteries had been opacified by an injection of an aqueous suspension of barium. Our study has focused on the anterior ciliary arteries (ACA), emerging from the four rectus muscles. Among the numerous individual variations, we have noticed the following predominant aspects: for the external rectus muscle, the ACA is usually unique and thin, located in the superior or inferior third of the muscle. For the others rectus muscles, the ACA are rather big and frequently observed in pairs. They tend to be found on each lateral side of the muscle. By and large, the vertical ACA look bigger and more numerous than the horizontal ACA. The number, the size and the position of the long posterior ciliary arteries are quite uniform. The major arterial circle of the iris seems to be discontinuous, and is more and less made of arterial anastomoses. This study enabled us to reconsider the usual anatomical description of arterial supply of the anterior segment. The vascularization by the ACA, especially at the level of the inferior and superior rectus muscle, has to be known in order to avoid an anterior segment ischemia following strabismus surgery.

[Morphologic aspects of intravascular disseminated coagulation. Apropos of 100 cases].

The authors report a morphological study of one hundred cases of disseminated intravascular coagulation (D.I.C.) of various origins. In each case laboratory findings demonstrated an authentic consumption coagulopathy. In all cases organ biopsies were performed either in vitro (rarely), or mainly immediately after death or at autopsy. Optic microscopy was performed for all of them (3 stains were chosen: hemalum eosin saffron, Mallory's phosphotungstic acid hematoxylin stain, P.A.S.). For a smaller number of cases ultrastructural microscopy and immunofluorescence were performed. As far as microscopic findings are concerned, the authors insist particularly upon intravascular microthrombi (fibrinous type and hyalin type). Haemorrhages and particularly necrosis, because of multiple origin, are not detailed. The repartition of these microthrombi is particularly detailed (in connection with the different organs, etiologies, clinical and/or biological findings, treatments). A review of literature is reported.

Influence of oral contraceptives of differing dosages on alpha-1-antitrypsin, gamma-glutamyltransferase and alkaline phosphatase.

We have studied the effects of oral contraceptives on three glycoproteins, alpha-1-antitrypsin (A1AT), gamma-glutamyltransferase (GGT) and alkaline phosphatase (AP), in terms of age (20--40 years), duration of administration, and levels of estrogens. We have confirmed that oral contraceptives increase the concentration of A1AT and the activity of GGT and decrease the activity of AP, but that the modifications are less pronounced with pills containing low levels of estrogens for GGT and AP. We observed a decrease in A1AT and GGT after 2 and 5 years of treatment, respectively, but an increase in AP. Patient age seemed to have little influence on the change in AP activity and A1AT concentration but GGT activity was higher for the 25--30 year group.

[Arterial vascularization of the musculi rectus verticalis and obliquus oculi (author's transl)].

In a recent article, we described arterial vascularization of the musculi rectus medialis and lateralis, using a radiological technique. We have now studied, in the cadaver, arterial vascularization of the musculi obliquus and rectus verticalis, from 30 new orbital contents. The posterior two thirds of the ocular face of each muscle and its edges are particularly rich in vascular inlets. The musculus rectus superior often has one pedicle only, in the shape of a tuning fork, stretched over nearly the whole length of the muscle. The musculus rectus inferior often has only a single central pedicle clearly visible in the anterior third of the muscle only. The musculus obliquus superior has two vascular sources which form one or two central axes. The arrangement of the arterioles in the musculus obliquus inferior is always the same, the main point of entry being located in the middle of the posterior face of the muscle. The anterior eiliary arteries in arising the two musculi rectus verticalis, are particularly large.

[Arterial vascularization of the rectus medialis and lateralis oculi muscles (author's transl)].

The arterial vascularization of the rectus medialis and lateralis oculi muscles was studied in cadaver following the injection of a stable suspension of radiopaque barium sulfate in the internal carotid artery. This straightforward and reliable original technic allows a good visualization of the arterial branches assigned to the muscles at the time of the orbital dissection. The arterial pedicles intake points inside the muscles are optionally placed on the level with the posterior two-third and generally they are more posterior for the lateral rectus than the medial rectus. We have then removed 30 rectus medialis muscles and 30 rectus lateralis muscles for the radioanatomic study of their intrinsic vascularization. Histological tests proved that an impregnation of 10 microns was up to the capillaries and that the vascular structures were respected. We have been able to characterize main and secondary intrinsic arterial pedicles which constitute a very rich anastomotic network. The main pedicles have a central or lateral topography and their distribution is independent of the way the ophthalmic artery crosses the optic nerve.

[Primary malignant tumour of the liver associated with the ingestion of oral contraceptives (author's transl)].

A 42-year-old woman, who had been taking oral contraceptives for 10 years, developed a primary carcinoma of the liver. The diagnosis was made by biopsy under laparoscopic control. Twenty one cases have already been reported in the literature. The aetiological role of contraceptives in the development of generally benign (approximately 200 published cases) and sometime malignant hepatic lesions, has not been definitely proven. The regression of certain benign tumours when contraceptives are stopped is the essential argument. In some patients, the association of benign lesions and of hepatic carcinoma suggests a single pathogenesis. The early recognition of such lesions justifies their being sought routinely in patients taking oral contraceptives (palpation of the liver, questioning concerning pain in the hepatic region). A national register will be required in order to determine their exact prevalence.

Pseudotumoral aspect of posttraumatic digital synovitis.

A case of posttraumatic subacute synovitis with pseudotumor of the digit with bone lesions on plain film is reported. Arteriography showed a pattern of hypervascularity that did not appear malignant and evoked a benign tumor of the synovial. Biopsy helped establish diagnosis of common subacute synovitis. Swelling disappeared spontaneously during subsequent months. Diagnosis of villonodular synovitis is difficllt when proliferation of histiocytes is present.
