[Adenomatoid tumor of the epididymis].

Primary tumors of the epididymis are very rare. The neoplasms most frequently affecting the epididymis are adenomatoid tumors. In this report, the authors describe a case observed personally. Various histogenetic hypotheses and the histological characteristics of the tumor are discussed.

[Primary retroperitoneal tumors (clinical findings in yolk sac tumor, vascular leiomyosarcoma and extraosseous chondrosarcoma)].

Primary retroperitoneal tumors (PRT) constitute about 0.02 per cent of all tumors examined in the Department of Pathology of our hospital. We report 3 cases of malignant PRT - yolk-sac tumour, vascular leiomyosarcoma and extraskeletal chondrosarcoma - selected for their histological rarity and clinical features. Recent techniques available for the diagnosis of these neoplasms and surgical treatment are discussed. The role of radio-chemotherapy is also dealt with.

[Adenoma nephrogenic. Morphologic aspects].

Nephrogenic adenoma is a rare lesion of urothelial mucous membrane. At present, the cases described in literature are 85. In this work the Authors study the morphologic aspects and etiopathogenetic theories.

[Spontaneous rupture of the transplanted kidney].

The spontaneous breakage of a kidney is a rare complication which may occur in the first weeks after a transplantation. The cause which can determine it is not yet cleared up, although the Authors think the greatest responsibility for this can be ascribed to rejection. In this work the authors report the experience and casuistry of the Transplantation Centre of Verona.

[Lymphocele after renal transplant: considerations based on a clinical case].

The Authors study again the problems of lymphocele in kidney transplantation by showing an interesting clinical case: a lymphocele with smallest size, developing an intensively compressive action on iliac vein. In the light of the experience of 354 transplantations, they discuss the etiopathology and clinics thereof. They particularly emphasize the diagnostical role of echography. Whereas the asymptomatic lymphoceles require no treatments for their trend to be reabsorbed, the symptomatic ones need a quick solution of the compressive effect. Then they consider the therapeutical solutions suggested as an alternative to the interperitoneal marsupialization, which remains the most widely used method.

[Gastric leiomyoma (presentation of 3 clinical cases)].

Gastric leiomyoma represents a rather rare report in abdominal surgery, above all because it very often shows no symptoms. Among the non-epithelial gastric neoplasms, it occupies the second place after lymphoma. After describing its anatomo-pathological aspects and clinical features, the authors report their casuistry, including three observations of such neoplasm, and point out that both the endoscopic inspection and the bioptic abstraction, contextually performed, were not so useful in formulating a sure preoperative diagnosis.

4'-Methylangelicins: new potential agents for the photochemotherapy of psoriasis.

Three derivatives of angelicin (1) [4'-methyl-, 4,4'-dimethyl-, and 4',5-dimethylangelicin (2a-c)] have been prepared with the aim of obtaining new agents for the photochemotherapy of psoriasis. These compounds form a complex in the dark with DNA that shows an affinity for the macromolecule higher than that of the parent angelicin (1). A correlation between their octanol/water partition coefficients and the association constants of the complexes has been observed. Compounds 2a-c photobind to DNA to a much higher extent than 1 and also more effectively than 8-methoxypsoralen (8-MOP), taken as reference compound. When activated with UV-A, the three compounds strongly inactivate T2 phage and inhibit epidermal DNA synthesis in mice. Moreover, they show a mutagenic activity markedly lower than that of 8-methoxypsoralen on Escherichia coli wild-type strain. Due to its lack of skin phototoxicity, its low mutagenic activity, and its antiproliferative activity, 2c was chosen for clinical evaluation. It proved to be effective in clearing psoriasis in two patients.

[Urologic complications in kidney transplants: experience in 274 transplants].

The urological complications after a renal transplantation still represent an event anything but rare, ranging, according to the different statistics, from 0.9% up to 23%, with a mortality between 0 and 25%. In this work done by the Verona Centre the experience on 274 transplantations is reported. The urological complications weighed with 6.2% and absence of mortality among the patients. The importance of the prevention and precocious diagnosis is confirmed.

[Indications and technic of percutaneous external biliary drainage].

After focusing the high surgical morbidity and mortality of icteric patients, the Authors describe their technique of insertion of percutaneous bile drainage after percutaneous transhepatic cholangiography. They insert drainage via the epigastric region, usually in the left hepatic duct. One case is described where the Authors associated percutaneous transhepatic drainage to regional chemotherapy through the catheter used for bile drainage. After for cycles of chemotherapy with 5-FU both intravenously and in the catheter, the Authors obtained the patency of the main bile duct which was previously obstructed by a methastases of stomach cancer.

[Current data on transhepatic percutaneous cholangiography in the differential diagnosis of jaundice].

The authors stress the actuality of percutaneous transhepatic cholangiography (P.T.C.) in differential diagnosis of jaundice, for absence of complications, low cost, simplicity of execution, and high percent of success. They derive their technique by Okuda et al., with some proper variations. Particular stress is placed on the decompression of biliary tree before and after injection of contrast medium (M.D.C.). Results obtained coincide with those of other authors.

Biliary Tract Neoplasms

Uselessness of thermography for diagnosis and follow-up of cutaneous malignant melanoma.

Thermography is useless for diagnosis of cutaneous malignant melanomas (CMM). In fact, false negative thermograms (29.9%) are much more numerous than clinically suspicious cases. Thermography is useless for follow-up of CMM. False-negative and false-positive thermograms of metastatic lymph nodes, subcutaneous nodules and removal site of CMM are much too numerous.

Follow-Up Studies
