[Hormones and embryogenesis of the uterine cervix].

In the absence of the T.D.F. gene, differentiation of the genital tract follows the female development with Wolffian ducts regression, elaboration of Müllerian ducts, and homologous uro-genital sinus organization. Up to week 32, the fetal ovary secretes no organizing substance and no estrogen. Even if the maternal body is heavily impregnated by estrogens from the fetal-placental unit, the estrogenic and progestogenic activity of any steroid hormones which cross the placental barrier and enter the fetal body is reduced as a result of metabolic degradation within the fetal liver. It is only from week 32 that in normal pregnancies, phenomena of estrogenic action on the uterine cervix of the fetus are seen. This consists of hyperplasia of the endo-cervical epithelium and its emergence towards the exo-cervix. The slightest addition of non-steroid synthetic estrogens administered to the mother may interfere with this hormonal environment of the fetus and result in abnormal development of Müller's tract, particularly in the cervico-vaginal zone.

[Assessment of human papilloma virus colposcopic findings of the cervix (in the Italian colposcopic classification].

HPV-induced flat condylomatosis is not only much more frequently observed on the hexocervix than the florid type, but it also induces colposcopic changes in the epithelium without completely altering its morphology. HPV-induced changes have now been largely identified and are recognisable in colposcopy. They can therefore be classified in the various patterns that characterise colposcopically the transformation epithelium, with undeniable advantages not only from the point of view of their correct classification but also from that of their prognostic evaluation. This is important because viral modifications ANTZ have a different diagnostic and prognostic significance from those observable in the notice squamous epithelium. A classification proposal of the various findings of flat HPV condylomatosis based on italian colposcopic classification is therefore presented.

Follow-up results of a prevention program for cervical cancer.

Of 4,782 women attending a colposcopy clinic from 1964 to 1971, 4,358 were included in a prevention program for cervical cancer and were followed for at least ten years. Of them, 1,105 were untreated and 3,253 treated. One hundred two underwent surgery and 3,151, electrocautery (ECT). On the basis of the post-ECT colposcopic examination, cervices were divided into iodine dark and iodine light. One cervical intraepithelial neoplasia (CIN III) and three invasive cancers were found among the untreated women. Three neoplasias (one adenocarcinoma, one in situ carcinoma and one invasive carcinoma) were found among the treated non-CIN women, only one carcinoma among the treated CIN women and no neoplasias among the surgically treated women. The comparison showed a difference in favor of the treated group, though it was not statistically significant. A comparison between the observed and expected data on the basis of the literature was highly significant: the expected appeared high. While these data do not prove the basic hypothesis that ECT has a protective effect in cancer prevention, they do provide some evidence in favor of it.

Junctional pattern in the squamous metaplasia of the female trigone. A freeze-fracture study.

The junctional pattern of squamous metaplasia in the female trigone was studied by the freeze-fracture technique. Desmosomes were observed in all epithelial layers and varied in complexity from the basal to the superficial layers. Gap junctions were observed in the lower and upper epithelial layers. Tight junctions were detected occasionally in the superficial layer only, which appeared as maculae and did not form zonulae occludens. These data support previous suggestions that the squamous metaplastic epithelium is not waterproof and may interfere with the contraction-expansion mechanism of the bladder.

Age-related frequency of viral lesions and their association with uterine cervical intraepithelial neoplasia.

Data collection forms of 50,165 routine smears were examined, and of these 47,577 were considered for this study. Viral cytological lesions (VL) were observed in 101 cases: of these, 19 were associated with cervical intraepithelial neoplasia. The total frequency of VL was 0.21%, with the peak in the teenager group (0.49%). The frequency of CIN among VL showed an overall frequency of 18.8%: this was null under 21 years of age and highest (50%) between 51 and 60 years of age. The frequency of VL in relation to clinical provenance of patients and the season of the year did not attain significance.

The junctional pattern of the human villous trophoblast. A freeze-fracture study.

This investigation of the junctional pattern of the human trophoblastic epithelium was carried out on the fractured faces of (a) the syncytial plasma membrane bathed by the maternal blood (S-PMm); (b) by syncytial plasma membrane in contact with either the cytotrophoblast or the trophoblastic basement membrane (S-PMf), and (c) the cytotrophoblastic plasma membrane (C-PM). The most relevant findings were tight junctions on the SPMm, and the intertrophoblastic junctional pattern, not realizing a tight subcompartmentalization between syncytium and cytotrophoblast. The following aspects were discussed: the SPMf represents the last membrane barrier before the fetal capillary, and the function of the cytotrophoblast seems to be limited to the modulation of syncytial activity.

Experiences from the accident of Seveso.

Provisional data on selected sanitary events which took place at Seveso after July 10 1976 are reported. 187 cases of chloracne, mostly in children, were detected, 50 just after the accident, the others within a year. Most polluted area (zone A) provided almost all "early" and most severe cases, but the territorial distribution of chloracne prevalence rates showed some inconsistencies with the soil TCDD pollution map; interpretations for such findings are discussed. Thirty-eight birth defects were detected in 1977 (none in zones A and B), more than in previous years, but still less than expected in a well controlled "normal" population: no clustering around a given type was observed. Spontaneous abortions, evaluated both as abortion rates and as pregnancy loss rates, showed scattered and statistically non-significant variations, inconsistent with the pollution map. No differences in birth and death rates compared to surrounding areas were observed. Data on ad hoc cytogenetic, neurological and immunological surveys are commented. Limitations of the presently available data are discussed and further research lines are anticipated.
