[Thymoma: anatomo-clinical and therapeutic aspects. Analysis of our 26 cases].

The paper reports the authors' personal experience with regard to 26 cases of thymoma treated surgically over the past 10 years. 96% of cases were associated with myasthenia gravis. Surgery was performed in all cases and 13 cases (50%) underwent additional RT. 92% of patients were affected by invasive thymoma (11 stage II cases, and 13 stage III). From an histological point of view, 38.5% of cases were epithelial-type thymomas, 38.5% were lymphoepithelial and 23% were lymphocytic. Survival was influenced by the stage of disease and the radicality of surgery. At present, 76% of patients are still alive between 10 months and 10 years of surgery. There was a perioperative mortality of 11.5% (3 cases), whereas 2 patients died some 2 months later due to pulmonary embolism. The paper also includes a critical review of the most recent literature, and the authors compare their experience with that of other centres.

Bilateral renal cell carcinoma with caval invasion in a woman with duplicated inferior vena cava.

Workup of a woman presenting with a palpable right flank mass, detected bilateral renal solid lesions, later proved to be renal cell carcinomas, and caval invasion in the presence of duplicated inferior vena cava. This association does not appear to have been previously described. The patient underwent right radical nephrectomy, cavotomy and auriculotomy with caval thrombus removal and delayed enucleation of left renal masses and left adrenalectomy. Eighteen months after surgery the patient is alive and without local and distant recurrence.

[Safety and immunogenicity of an anti-hepatitis-B vaccine obtained using the recombinant DNA technique: results of a longitudinal study in hospital personnel].

A prospective study of the immunogenicity, safety and persistence of antibodies induced by the DNA-recombinant anti-hepatitis B vaccine Engerix B was conducted in a risk group as health care workers. 115 health care workers were randomly allocated into two groups. Immunization was carried out according to a 0-1-6 month vaccination schedule in one group and to a 0-1-2-12 month schedule in the other group. Results indicate that the DNA-recombinant anti-hepatitis B vaccine is well tolerated and highly immunogenic. Reactogenicity to the vaccine was low in incidence and mild in severity with no significant difference appearing between the study groups.

[Endo-urology in diseases of the upper urinary tract other than tumors and calculi].

Thirty-three patients underwent percutaneous treatment for upper urinary obstructive disease in our Institute. Cold-knife incision of 16 cases of uretero-pelvic junction obstruction and 4 cases of infundibular stenosis was performed. Balloon dilatation of the caliceal neck was performed in 1 patient with caliceal diverticulum. In 4 of the patients treatment failed requiring open surgical correction. Thirteen patients presented iatrogenic ureteral obstruction: balloon dilatations or cold-knife incisions were performed. Four of the 9 patients evaluable obtained significant benefit from the endourological treatment. Endopyelotomies for UPJ obstruction seem effective and reduce morbidity and operating times. Ureteral obstructions appear less responsive to the endo-urological approach. Definitive conclusions concerning the effectiveness of these procedures requires larger series of patients and longer follow-up.

Modulation of the Root effect in goldfish by ATP and GTP.

Both ATP and GTP are present in considerable amounts in red cells of the common goldfish Carassius auratus. They both influence the Root effect of the single major fish hemoglobin, but GTP is, depending on pH, 2-6 times more effective than ATP. The two triphosphates account for 3/4 of the effect of trichloroacetic acid supernatant obtained from hemolysate which contains still some compound(s) which can influence the shift of the Root effect toward higher pH.
