Monthly injectable steroid contraceptives and cervical carcinoma.

The World Health Organization Collaborative Study of Neoplasia and Steroid Contraceptives is a large multinational hospital-based case-control study of steroid contraceptives and gynecologic, hepatobiliary, and mammary neoplasms. Monthly injectable steroid contraceptives which contained the long-acting progestogen dihydroxyprogesterone acetofenide plus a shorter-acting estrogen (usually estradiol enanthate) were used by women in two of the countries (Chile and Mexico) from which data were collected. In preliminary analyses of data from Chile (1979-1983), a strong association was observed between use of these products and invasive cervical cancer. Therefore, three additional data sets from these two countries were analyzed in further detail for this report. Analyses of additional data from Chile on invasive cervical cancer (1983-1985) and cervical carcinoma in situ (1979-1986) and of data on invasive cervical cancer from Mexico (1979-1986) failed to confirm the initially observed association. The original finding was probably due to chance, but a causal interpretation cannot be confidently ruled out, and additional studies are warranted.

Incidence and morbidity of cancer of the cervix in Geneva, Switzerland.

The incidence and mortality rates of invasive carcinoma of the cervix in the state of Geneva, Switzerland, is reported for the time periods 1970-1973 and 1980-1983. A reduction in mortality (7 to 4.3/100,000) occurred as well as a considerable decrease in the overall incidence (20.3 to 11.4/100,000). This decrease is particularly marked in the age group of 35-64 years. It is less marked between the ages of 60 and 69 and did not appears before 35 years or after 70 years of age. In the time period 1980-1983, more than one half of the cases were found after age 60 years, two thirds of them in stage 2 or worse. It is therefore essential that screening programs recruit a high enough proportion of older women.

Risk factors for invasive cervical cancer and reduction of risk by 'Pap' smears in Thai women.

A case-control study of invasive cervical cancer was conducted in Siriraj Hospital, Thailand, as part of a WHO-sponsored collaborative study of neoplasia and steroid contraceptives. Data from 189 histologically confirmed cases and 1023 randomly selected hospitalized controls who were recruited from October 1979 through March 1983 were analysed to identify risk factors for cervical cancer in Thai women, and to assess the effectiveness of Papanicolaou smears (Pap smears) in preventing invasive cervical cancer in Thailand. Variables that distinguish women at significantly increased risk of invasive cervical cancer, to whom preventive programmes should be directed, include a history of treatment for abnormal vaginal discharge (an indicator of vaginal or cervical infection), a history of venereal disease, and little or no education (an indicator of low socioeconomic status). Risk of invasive cervical cancer decreased significantly as the frequency of Pap smears increased, and was reduced by 75% in women who had at least one Pap smear per year.

Endometrial cytohistology by the pistol-aspiration technique: clinical applicability.

The purpose of this study was to assess the value of the endometrial pistol-aspiration technique as an office procedure for diagnosing corpus uteri cancer in symptomatic and asymptomatic women. Review of 3000 pistol aspirations in symptomatic outpatient women from 1974-1981 confirms that this method, with its high specificity and sensitivity, can be used for such women as a preliminary diagnostic procedure, eliminating risks from anesthesia and curettage as well as the cost of hospitalization. Analysis of resident incident cases shows that 82% were localized to the uterus, and more than 85% of these women were already symptomatic. Older women represent particularly difficult problems, both technical (cervical stenosis) and in recruitment. Because the natural history of the precursor lesions of corpus uteri cancer are not yet well established, use of this method for systematic population screening of asymptomatic women is questionable.

[Evaluation of screening for uterine cervix cancer: from theory to practice (author's transl)].

In many regions, screening for cervical cancer activities by means of cytology have developed "on faith", without fulfilling sufficient quality criteria (identification--smear taking--staining--reading--nomenclature of ciagnosis--communication with the practitioner). Progress made in cytological diagnosis and better knowledge epidemiological data make, at present, such a situation inacceptable from the point of view of public health expenses, as well as for the population of women concerned. Thorough evaluation of this problem shows that inadequate programmes could be more harmful than useful. It is then indispensable to evaluate the needs, then to create technical structures of good quality, before starting any mass screening campaign.

[Creation of an intercantonal tumor register].

A population-based Cancer Registry is a permanent system which records all the malignant tumors occurring in a particular geographical area. Its main objective is to set up the basis for the establishment of statistics, either for practical use (cancer control) or for research. More than a hundred such registries exist nowadays throughout the world, and in Switzerland a number have been recently created or are in process of organization (cantons of Geneva, Neuchâtel, Vaud, Zurich, Basel). A coordination group has accordingly been created to harmonize the data and the coding system, in order to allow the organization of an Intercantonal Registry.

Endometrial cytohistology by aspiration technic and by Gravlee Jet Washer: a comparative study.

An office procedure of intrauterine aspiration yielded a 99% diagnostic accuracy in 100 patients admitted for either curettage or hysterectomy. After correlation with surgical diagnosis, no false-positive or false-negative results were found. The method provides tissue for cell blocks adequate for histologic diagnosis of endometrial adenocarcinoma and its precursors. The major advantage of this technic in comparison with the Gravlee Jet Washer is its low cost. Moreover, in this study, it proved to be less painful, superior for cell block preparation, and yielded more satisfactory specimens. As a screening procedure for asymptomatic patients at risk, this intrauterine aspiration technic holds promise and must be evaluated by further studies.

A modified intrauterine aspiration technic for detection of endometrial adenocarcinoma and its precursors has been described. This method which is simple, inexpensive and requires neither anaesthesia nor analgesia, allows cyto-histologic diagnoses of endometrial lesions. A correct cytologic or cyto-histologic diagnosis was made in 93.9% of the 277 cases examined. Of the premalignant and malignant endometrial lesions, the diagnostic accuracy was 100%.

Cytochemical properties of human endometrial cells in vitro.

The cytochemical properties of the human endometrial cell nuclei have been studied during the menstrual cycle in eight women by means of two different microfluorometric methods, both considered to be specific for DNA: the BAO-Feulgen method and a method based upon an interaction between DNA and mithramycin. An in vitro system was developed where dissociated endometrial cells were studied at 4 hours and 18 hours of incubation at 37 degrees C. The cytochemical properties of the dissociated endometrial cells did not change significantly in the in vitro system used. An increase in the BAO-fluorescence intensity per cell nucleus was observed in all the endometrial samples when compared with the diploid standard DNA value of a non-growing cell population. A similar increase in the mithramycin fluorescence intensity per cell nucleus appeared only during the secretory phase. It is suggested that the nucleoproteins play an essential role in changing the cytochemical properties of the human endometrial cell nuclei during the menstrual cycle.
