Antibody detection ELISAS for malaria diagnosis.

Parasite extracts of Plasmodium falciparum and P. chabaudi and three synthetic peptides from the P. falciparum MSA2 merozoite antigen were tested for suitability as antigens in an antibody detection ELISA using sera from malaria patients in Brisbane. The P. chabaudi extract was superior to P. falciparum extract for detecting P. vivax cases, while for P. falciparum cases the two parasite extracts were equivalent. Single peptide antigens were generally less sensitive than parasite extracts; however, peptides G3 and G7 were more sensitive than parasite extracts in detecting first attacks of P. vivax. Examination of isotype specific responses demonstrated that this may be explained by higher IgG responses to these peptides in first than in subsequent P. vivax attacks. Because of the differing antibody specificities in primary and secondary P. falciparum and P. vivax cases, the best sensitivity was achieved by using the combined results of assays with three antigens: P. chabaudi, peptide G3 and peptide G7. The combined sensitivity was 77.1% for P. falciparum and 88.6% for P. vivax acute cases with 91.1% specificity.

[Direct angioplasty without previous fibrinolytic treatment during the 1st hours following myocardial infarction].

Direct coronary angioplasty as an emergency procedure in the first hours of myocardial infarction without prior thrombolytic therapy requires a heavy infrastructure. The different results reported in the literature show a recanalisation rate of 90% with an average hospital mortality of 7%, an acceptable risk given the inclusion of patients of over 75 years of age in these series and cases of cardiogenic shock. The reocclusion rate in the hospital phase is between 10 and 15%. At medium term, the survival rates of single vessel disease are excellent but the mortality is higher in multivessel disease (78% survival at 2 years). Angioplasty performed in the first three hours after the onset of symptoms is associated with an improvement in global an regional left ventricular function in patients with anterior infarction and altered initial left ventricular function. The preliminary results of a randomised multicenter trial comparing direct angioplasty with intravenous thrombolytic therapy with rt-PA (PAMI) seem to show in favour of angioplasty with a lower rate of complications in the hospital phase. The results of this multicenter trial should provide information as to the exact role of direct angioplasty in the initial phase of infarction, a procedure reserved for well-equipped cardiological centres with highly trained invasive cardiologists.

Scintigraphic evaluation of renal function after extracorporeal shock-wave lithotripsy.

We evaluated renal function within 48 hours after extracorporeal shock-wave lithotripsy (ESWL) as well as 1 month and up to 6 months later employing technetium-99m dimercaptosuccinic acid and iodine-131-labelled ortho-iodohippurate. All 17 patients displayed abnormal renal function immediately after ESWL. The abnormalities identified included focal cortical lesions, diffuse reduction of renal function, increased kidney volume, and diffusely and focally increased parenchymal transit times. The follow-up scintigraphic studies indicated that the great majority of the lesions had been temporary.

[Lithotripsy of biliary calculi: evaluation of ultrasonography as a method to guide treatment by extracorporeal shock waves].

Extracorporeal shock-wave lithotripsy is a procedure recently introduced to treat gallstone disease. According to the literature, 15% to 25% of symptomatic persons will be candidates for this procedure if it proves effective. Currently, sonography is one of the best methods for monitoring the performance of lithotripsy. The authors have confirmed this. They have designed an in-vitro model which allows comparison between what is actually happening during gallstone lithotripsy and what is being seen by real-time sonography. The sonographic characteristics of the different phases of gallstone lithotripsy are presented.

Oxidation-reduction potential studies: a new method in pharmacology principles, materials, and methods.

Oxidation-reduction (redox) potential measurement is not used in pharmacology at the present time. However, it represents a physicochemical parameter that may be as important as the other currently used physicochemical parameters, such as pH and pKa. Redox potential variations include conformational modifications that may influence bioavailability of drugs. We report here an original and convenient equilibrium method to determine this potential in biological fluids, especially in blood.

Citalopram versus maprotiline: a controlled, clinical multicentre trial in depressed patients.

In a double-blind trial, comprising 96 depressed patients, citalopram was compared with maprotiline. The trial period was 6 weeks with ratings (MADRS, CGI) and side effects recordings taking place at Weeks 0, 1, 2, 4, and 6. Both drugs were administered as a single evening dose, 40 or 60 mg for citalopram, and 75 or 150 mg for maprotiline. MADRS total scores and CGI scores showed a highly significant reduction in both groups with no significant difference between them, whether the groups were considered as a whole or whether they were subdivided into endogenously/non-endogenously depressed or melancholic/non-melancholic patients. Side effects were not significantly different, but the maprotiline group showed more anticholinergic side effects, whereas the citalopram group showed more nausea, increased sweating and headache. Two patients on maprotiline were withdrawn because of side effects (hypotension and somnolence in the one case; tremor and insomnia in the other). One patient in each group was withdrawn because of increased transaminases, the citalopram-treated patient having increased values, however, already at baseline. Apart from this, no cardiovascular side effects and no pathological laboratory values related to treatment were observed. The authors conclude that citalopram is a safe antidepressant drug and as effective as maprotiline.

Objective response and disease outcome in 59 patients with stage D2 prostatic cancer treated with either Buserelin or orchiectomy. Disease aggressivity and its association with response and outcome.

A comparative study was done in 59 recently diagnosed Stage D2 prostatic cancer patients treated with either long-term GnRH-A (Buserelin) (N = 42) or with orchiectomy (N = 17) and followed up for three years. The suppressed limits of plasma testosterone and estradiol levels after eight-week follow-up as well as the objective clinical response and disease outcome were found to be similar with either treatment. Hot flushes and loss of libido were noticed in both groups throughout the follow-up period; however, there were no other side effects. Analysis of Stage D2 patients based on their time of death enables us to identify nonhormonal variables which, in the form of an aggressiveness score, correlated well with both clinical response and disease outcome. These data confirm that (1) Buserelin is an effective and safe alternative to orchiectomy in advanced prostatic cancer, and (2) in clinical studies a multifactor aggressiveness score is useful for analyzing clinical efficacy data. Prospective application of that score may enable predictability of patient response and influence patient management.

A family study of the prevalence of antibodies to the rheumatoid arthritis associated nuclear antigen (RANA).

Sera from families with at least two members suffering from seropositive rheumatoid arthritis (RA) were examined for the prevalence of antibodies to the RA associated nuclear antigen (RANA). It was found that consanguineous relatives of patients had a significantly increased prevalence of anti-RANA antibodies compared to sera from a control group of families. Anti-RANA antibodies were also significantly more prevalent in the sera of familial RA patients who possessed HLA-DR4. No correlation of anti-RANA antibodies with disease associated haplotypes was observed in these families.

[Coronary recanalization by transluminal angioplasty in the acute phase of myocardial infarct].

Recanalization by transluminal coronary angioplasty (TCA) was attempted as the first measure in 41 patients having symptoms for less than 4h. The obstruction could rapidly be removed (between 14 and 50 min) in 38 patients (92%). Five patients (12%) died: one in shock at the end of the operation, two died on the 5th and 8th postoperative day and two from heart failure on the 8th and 15th postoperative day. Three patients were urgently operated on for severe tritruncal lesions after a successful TCA. Among the 27 patients under control between 1 and 12 months, six reocclusions were noted (5 asymptomatic, 1 with clinical recurrence), 5 had recurrent stenosis (3 of them were treated by a 2nd angioplasty) and 16 (60%) exhibited permanent good results. The global ejection fraction was not modified, neither the telediastolic and telesystolic volumes. A significant improvement in segmental kinetics was observed in 6 patients with anterior and in 5 patients with inferior infarction. In this group of patients the ejection fraction was improved (67.2 +/- 2.3 vs 52.8 +/- 2.2, p less than 0.001), the IVDT was unchanged (81.7 +/- 6.8 vs 90.7 +/- 5.1, NS). IVTS was lower (27.5 +/- 3.5 vs 41.9 +/- 2.8, p less than 0.001). Further work is, however, needed to confirm these results.

Results of the Sherbrooke-Lac Mégantic fluoride varnish study after 20 months.

The purpose of this study was to demonstrate the effectiveness of two fluoride varnishes (Duraphat and Fluor Protector) in the prevention of dental caries in children residing in a non-fluoridated community. At baseline, 787 grade one children (6-7-yr old) were examined and randomly allocated into a control and two treatment groups. At 20 months, 703 children remained in the study. The mean baseline caries experience and the mean number of permanent teeth present at 20 months were not significantly different among the treatment and control groups. The mean DMFS increments after 20 months were 1.70, 1.73 and 2.02 for Fluor-Protector, Duraphat and control groups respectively (% reductions of 15.8 and 14.4 for Fluor-Protector and Duraphat groups). The mean dfs increments for primary molars after 20 months were 1.56, 1.62 and 1.74 for Fluor-Protector, Duraphat and control groups respectively (% reductions of 10 for Fluor-Protector and 7 for Duraphat).

The results after 20 months of a study testing the efficacy of a weekly fluoride mouthrinsing program.

The purpose of this study was to determine the efficacy of weekly mouthrinsing with a 0.2 percent NaF solution in first-grade children living in a non-fluoridated community. Children in the control group were also participants in a randomized clinical trial to evaluate the efficacy of semiannual fluoride varnish treatments. Children allocated to the mouthrinse group attended school at three of the 17 area schools where the varnish study was occurring. Random allocation of children into the treatment group was considered impractical because of potential problems of teacher cooperation and compliance. The same two standardized examiners examined all participants, and were blind to group assignment for all children. After two academic years, or approximately 72 weeks of rinsing, 178 and 247 children remained in the treatment and control groups, respectively. The control group experienced a mean caries rate of 2.02 surfaces over 20 months, while the treatment group demonstrated an increment of only 1.33 surfaces, representing a savings of about 0.34 surfaces per year or a reduction in DMFS of 34.2 percent. Surface-specific incremental reductions after 20 months were 0.35, 0.19, and 0.14 for the occlusal, buccal, and lingual surfaces, respectively. The proximal increment was too small to draw any meaningful conclusions. In the primary dentition, the treatment and control dfs increments were 0.74 and 1.74, respectively. This reduction represents a savings of one-half surface increment per year, or a reduction of 57.5 percent.
