Distinct distribution of periarticular erosions of the bones of the hand in chronic renal failure.

The frequency of radiological changes of the bones of the hand in 142 patients on hemodialysis, 19 on CAPD, and 15 patients with advanced renal insufficiency not requiring dialysis was analyzed. Forty-seven percent of all patients had subperiostal cortical bone resorptions, 31% periarthritis calcarea, 22% periarticular erosions, 3% chondrocalcinosis and 1% aseptic necrosis. Sixty-six percent of the periarticular erosions were observed in the distal and proximal interphalangeal joints of fingers II, III, IV and V, whereas only 33% were found in the remaining joints (120 vs. 62, p less than 0.001). The distal interphalangeal joints showed about 70% more often periarticular erosions than the proximal interphalangeal joints (77 vs. 43, p less than 0.001). Since patients with chronic renal failure exhibit a decreasing incidence of periarticular erosions from the distal interphalangeal joints to the intercarpal joints, while patients with rheumatoid arthritis have their erosions preferentially localized in the proximal interphalangeal, metacarpophalangeal and wrist joints, it can be concluded that renal osteodystrophy causes a distinct distribution of periarticular erosions of the bones of the hand.

Altered body fat distribution in patients with glucocorticoid treatment and in patients on long-term dialysis.

Fat distribution was assessed by computed tomography in normal volunteers (n = 42), patients on long-term dialysis (n = 18), and patients on glucocorticoids [renal transplant patients (n = 49), other diseases (n = 17)]. Patients on glucocorticoids had higher mediastinal (deep) and identical or increased posterior cervical, buccal, and midthigh (superficial) fat areas when compared with normal subjects. The pattern of fat distribution in dialysis patients mimicked the distribution observed in patients taking glucocorticoids. Healthy females had higher ratios of superficial to deep fat than healthy male subjects. Patients on prednisone or on dialysis lost this sex-associated difference in fat distribution. Since patients on prednisone exhibit increased or normal thigh fat depots in the presence of increased mediastinal fat, the current concept that glucocorticoids induce redistribution of body fat from peripheral to central fat compartments has to be revised. Furthermore, disease states and/or glucocorticoids abrogate sex-associated differences in body fat distribution.

High oral doses of mebendazole interfere with growth of larval Echinococcus multilocularis lesions.

The natural development of the larval stage of Echinococcus multilocularis in man has been studied in 7 patients after presumed radical operation 2 8/12-11 2/12 years prior to detection of a relapse. The volumes of the recurring lesions were assessed by CT-scanning, and assuming linear growth a median increase of 14.8 ml/year (range 3.8-220 ml/year) was calculated. In 6 patients treated for a median duration of 4 5/12 years with high oral doses of mebendazole a median growth rate of -3.0 ml/year (range-470- + 10.2 ml/year) was found, which differed significantly from the natural growth rate (P less than 0.01). Although the patients improved clinically, there was evidence of persistent infection. These data are the first controlled evidence that high oral doses of mebendazole may be parasitostatic in alveolar echinococcosis in man. Although not curative, this pharmacological effect appears to be clinically beneficial.

The lower-extremity musculature in chronic symptomatic instability of the anterior cruciate ligament.

We studied the musculature of the lower extremity in forty-one patients with chronic symptomatic instability of the anterior cruciate ligament. Computed tomographic and clinical measurements of the limb were taken at levels fifteen and twenty-five centimeters proximal to the medial joint line and ten centimeters distal to it. Biopsy specimens from the vastus lateralis of both lower extremities were analyzed by histochemistry and electron microscopy. Computed tomography revealed a decrease in the muscle square area of the affected thigh of 8 per cent as compared with the control lower extremity. There was quadriceps atrophy of 10 per cent and hamstrings atrophy of only 4 per cent. The atrophy of the vastus medialis was significantly greater than that of the entire quadriceps. Histochemistry revealed a similar decrease in fiber size for all fiber types, and hence no shift in fiber type. Electron microscopy showed an increase in intracellular fat but no change in mitochondrial volume density or capillarization.

[Computed tomography in primary mesenteric tumors].

The value of CT in the investigation of mesenteric tumours was studied in 21 patients. It is possible to differentiate mesenteric cysts from sarcomas, and CT provides information concerning the size, localisation, structure and extent of the tumour and the degree of infiltration. Density measurements provide evidence concerning the structure of the lesion, whether it is liquid, lipomatous, myxoid or osseous. Only rarely is it possible to make a histological diagnosis. Localised infiltration of the gut cannot be recognised, but diffuse intra- and retro-peritoneal tumour spread can be seen by CT. Ascites, intra-peritoneal and haematogenous metastases are rare from mesenteric sarcomas.

[Computer tomography in primary soft tissue tumors].

Computed tomography was performed in 37 patients with primary soft tissue tumours (24 malignant and 13 benign lesions). Except for lipomas it was not possible to relate attenuation values to histologic type or grade of malignancy. Major advantages with CT include the ability to demonstrate infiltration of surrounding structures and to determine tumour size. There is a risk of overestimating the size of malignant tumours because of peritumoural edema.

[Computed tomographic classification of acetabular fractures].

The contribution of computed tomography (CT) in classifying acetabular fractures was analysed retrospectively in 33 cases. CT and plain radiography classification agreed in 27 cases (82%). CT revealed more extensive fractures in 6 patients (thereof 5 patients with associated fractures). In 10 patients (thereof 9 patients with associated fractures) CT showed intraarticular fragments; radiographically intraarticular fragments were seen only in 2 patients and suspected in 4. CT is of considerable aid in defining the fracture pattern. It should be used mainly in patients with radiographically difficult interpretable associated fractures in order to assess preoperatively the weight-bearing part of the acetabulum, the degree of displacement and the presence of intraarticular fragments.

[Computed tomography of chondrogenic tumors].

The computed tomography (CT) morphology of chondrogenic tumors and the utility of CT in their diagnostic work-up is presented on the basis of 19 cases. CT provided unique informations regarding definition of tumor extent and tumor relationship to adjacent structures particularly in the axial skeleton. CT has diminished the indications for angiography in chondrogenic tumors.

Acetabulum

Measurement of liver volume by ultrasound and computed tomography.

A morphometric method to calculate liver volumes from transverse sections is evaluated (point-integrating method). In the first part of the study, 10 liver specimens were investigated by computed tomography (CT) and ultrasound (US); the calculated volumes were compared to the volumes obtained by water displacement of the organs. While CT showed an ideal agreement (r = 0.994), volumes calculated from US sections correlated less well (percentage differences from +12.5% to -9%, r = 0.915). In the second part of the study, the livers of 10 randomly selected patients were investigated by CT and US. Liver volumes were calculated using the point-integration method. Compared to the CT examination, US results show a good correlation with a correlation coefficient of r = 0.977. The point-integration method is very valuable to measure organ volumes from transverse sections. The method can be applied "offline" to photographic films, data do not have to be recorded electronically. The time required to calculate the volume of an organ is comparable to other methods.

Humans

The diagnostic impact of computed tomography in blunt abdominal trauma.

Sixty patients with blunt abdominal trauma were investigated by computed tomography, 21 within 48 h of injury, 39 within days and weeks. Organ laceration of the spleen, liver, pancreas and kidney and haematoma formation were comprehensively demonstrated and post-operative complications readily identified. Computed tomography is currently the radiological method of choice to evaluate injuries to abdominal organs.
