Echocardiographic and pulsed Doppler features in glycogen storage disease type II of the heart (Pompe's disease).

Two cases of cardiac glycogen storage disease type II are described: the first one, male aged 3 months, presented with generalized muscular hypotonia and decreased deep tendon reflexes; a 2/6 systolic murmur was audible at the left sternal border; chest X-ray and ECG were consistent with left ventricular hypertrophy; an echocardiogram disclosed an impressive and diffuse cardiac hypertrophy; the pump function appeared preserved and the estimated ejection fraction was about 70%. Pulsed wave Doppler demonstrated a normal envelope of mitral flow with E/A ratio = 1.27 in averaged 20 beats. The patient died suddenly at 6 months of age. The second patient was a female 4 months old with generalized muscular hypotonia. ECG and chest X-ray were consistent with left ventricular hypertrophy; 2D echocardiogram showed diffuse hypertrophy with estimated ejection fraction of 68% and an almost normal aspect of the mitral flow curve, with E/A ratio of 1.18. This child died at 13 months of age of cardiopulmonary insufficiency. In both cases the diagnosis was made by muscular biopsy and biochemical tests (alpha 1-4 glucosidase deficiency). We stress the fact that, despite the severe and diffuse hypertrophy, the pump function and the ventricular filling did not seem compromised.

Partial trisomy 4q: two cases with a familial translocation t(4;18)(q27;q23).

Clinical and cytogenetic data of two related patients, both trisomic for the segment 4q27 to qter, are reported. Familial studies determined that the mothers of the two probands were carriers of the same balanced translocation between chromosomes 4 and 18. Altogether, two partial trisomies 4q, five balanced karyotypes, and one 45,X0 karyotype were found in the family. The 18 cases reported to date are reviewed with respect to the karyotype-phenotype correlation.

[Corrected transposition of the great vessels. A clinical review of 21 cases (author's transl)].

Clinical and instrumental features of 21 cases of congenitally corrected transposition of the great vessels are reviewed. We confirm the rarity of the isolated form of the disease and its frequent association with ventricular septal defect and pulmonary stenosis. Electrocardiographically, the inversion of the first vectors is represented more frequently by the absence of the q wave on the left precordial leads than by the presence of the q wave on the right precordial leads; a very frequent electrocardiographic feature is a positive T wave on the right precordial leads. The angiographic image of the two ventricles in the lateral projection where they appear as crossing each other in their upper part were described. We confirm the poor results of the surgical treatment of the disease and the non-benign prognosis, because of the other associated cardiac anomalies.

Sialic acid content in four cell strains transformed by adenoviruses and/or SV40.

The sialic acid content in hamster cells primarily transformed by adenovirus was found lower than in cells transformed by papovavirus SV40. Intermediate concentrations were shown in a cell strain transformed by an adenovirus-SV40 hybrid. An adenovirus 19 - transformed cell strain derived from the NIL-2 hamster cell line, adapted to unlimited growth in vitro prior to the exposure to the adenovirus, revealed a high content of sialic acid. The concentration of sialic acid in cells transformed by DNA-viruses may depend both on the transforming virus and on the preceding history of the cell line. The tumors revealed more sialic acid than the in vitro cultures derived from the same cell strains. This finding is discussed.

[Juvenile hyaline fibromatosis. Description of a case and review of the literature].

The authors report a typical case of JHF. The complete review of the literature shows that no more than 20 cases have been so far described. The clinical, pathological and electron microscopic features are stressed and the main clinical and pathological differential diagnoses are considered. The disease, formerly thought to be due to an abnormal composition of collagen fibers, seems to be nowadays, thanks to Japanese authors, related to an error in glycosaminoglycans metabolism.

Age Factors↗

[Gastroesophageal reflux update].

Authors describe importance of anatomical and functional factors in ethiopatogenesis of G.E.R., diagnostic iter and methods. Importance of 24-hour computerized pH-metry in pediatric age is particularly stressed out. Medical and surgical therapeutic procedures are emphasized.

[Psychomotor recovery in an operated case of subdural hygroma of unusual dimensions].

A case of unusually large subdural chronic effusion is described in a child 3 years old with severe developmental retardation, without obvious symptoms of increased intra-cranial pressure. Diagnosis was made by angiography. The child underwent to a Pudenz-shunt. An intensive physical and educational care program is performed. Behavior improvement and partial integration in social structure are obtained.
