[Maisonneuve fracture of the fibula: clinical and experimental study].

OBJECTIVE: To study the traumatic mechanism and clinical treatment of Maisonneuve fracture. METHOD: 19 of 25 patients (6 - 60 months, average 16.8 months) were followed up. 11 patients were operated on and 8 were treated conservatively. Of the 25 patients, 8 had MRI. Osteotomy and serial section of the interosseous membrane (IOM) on fresh cadaveric specimens were doc carried out to observe the diastasis on stress mortise view. RESULT: Functional rehabilitation of the ankle joint: in 11 operative cases, 8 showed good results and 3 fair. In 8 conservative cases, 3 showed good results, 2 fair and 2 poor. MRI: IOM was ruptured in a range 3 - 8 cm proximal to the tibial plafond in the 8 cases. No IOM rupture was found at the high fibular fracture level. Fresh cadaveric experiment: fixation of the medial side alone can not control the syndesmotic diastasis on the artificial Maisonneuve fracture model made on cadaveric specimen. CONCLUSION: The injury level of the IOM during Maisonneuve fracture may not be consistent with the high fibular fracture. The limited external rotation of the fibula during injury by the superior tibio-fibular syndesmosis comprise to the high level fibular fracture. ORIF with syndesmotic fixation is recommended for the treatment.

[Cloning, sequencing and expression of hepatitis E virus structural gene in E. coli and application of the recombinant products for diagnosis].

The prokaryotic expression vector PEGX-3X was used to express hepatitis E virus (HEV) open reading frame 2(ORF2 402-660). The recombinant protein was soluble in PBS buffer and was purified by the glutathione Sepharose 4B affinity column. When the purified recombinant protein was applied to detect HEV antibodies in sera of clinical patients, the results were quite consistent with the HEV diagnostic kit from Diagnostic Biotechnology Ltd(DBL), Singapore. The recombinant protein was more reactive with HEV antibodies than DBL reagent.

[Detection of hepatitis G virus RNA in sera in non A-E hepatitis patients using polymerase chain reaction with multiple set primers].

A double polymerase chain reaction, involoving nested primers deduced from NS3-5 regions of the hepatitis G virus (GBV-c/HGV) genome, was developed for a sensitive and specific detection of GBV-C (HGV) RNA. With this method, 10 anti-HGV positive patients with non A-E hepatitis and 10 healthy subjects were tested. Among 10 patients, GBV-C RNA was detected in 9, 8, 4, 9 patients by the NS-3 (1) primers, the NS3(2) primers, the NS4 primers, the NS5(1) primers and the NS5(2) primers respectively. Control sera from 10 healthy subjects did not reveal GBV-C (HGV) RNA by any set of the primers. These data suggest that the detection rate of GBV-C (HGV) RNA is different in various set of primers, the NS3(1) and the NS5(2) primers are superior to other primers and are more suitable for RT-PCR of GBV-C (HGV) RNA.

[Telescopic adhesive anastomosis of small blood vessel applied in formation of arteriovenous fistula for hemodialysis].

The formation of an arteriovenous fistual for dialysis by routine interrupted sutures anastomosing the vein and artery is difficult to perform and time-consuming. A new method, telescopic adhesive anastomosis was studied and applied in 10 hemodialysis patients, who were in need of an arteriovenous fistula. The external diameter of the vessels anastomosed was 2.40 +/- 0.20 mm (radial artery) or 2.40 +/- 0.35 mm (cephalic vein). After thorough debridement of the vascular ends, the arterial end was put in the venous lumen. In order to fix the telescopic vessels, two stitches were applied 180 degrees apart from each other and tied. Each stitch was inserted from vein (penetrating the whole wall) to artery (just through the adventitia and partial thickness of the media vasorum). The distance from the stitch to the edge of the vein was 0.5 mm, and that of the artery was approximated to the external diameter of the vessle. The medical adhesive was then applied for sealing the anastomotic adventitia. Ten seconds were given for the solidification of the adhesive. The patients were followed up for 8 months. The patency rate was 100%, and the rate of blood flow was more than 300 ml/min (measured by ultrasonography). It was shown that this method could be managed easily and quickly, and the so-formed fistula would fulfill the need of hemodialysis.

[Absorbable SR-PGA implant in orthopaedics: preliminary results of treatment of fractures].

This article reports preliminary results of treatment in 33 patients with 36 cancellous bone fractures using absorbable rods and screws of self-reinforced polyglycolide (SR-PGA). Plaster cast was used postoperatively for 3 weeks. All fractures but one lateral ankle fracture were fixed until healed without displacement. All of them healed at six weeks after trauma. No wound infection and fluid accumulation occurred. This new device is an ideal implant for fixation of cancellous bone fracture.

[Experimental study of Colles' fracture combined with dorsal radio-carpal subluxation].

By cutting different ligaments and the distal end of radius of 20 fresh specimens with radiophotographic records, the author explored the mechanism of formation and traumatic anatomy of Colles' fracture with dorsal radio-carpal subluxation (DRSA). The results of experimental studies indicate that under normal condition, the change of radiolunate angle has some influence on the displacement of central point of lunate (CPL) but does not lead to DRS. The first is the rupture of the dorsal and palmar ligaments of radiocarpal joint and the second is the fracture of distal end of radius with a reversed palmar tilting angle of articular surface of distal radius. To treat the patients of Colles' fracture with DRS the previous palmar tilting angle should be restored to the full, as well as the anatomic reduction. The fixatiion in neutral or light dorsal extension position should be recommended.

[Effect of WS-frequence spectrum physiatrics apparatus on survival of the random flap in rats].

In this experimental study we investigated the effect of WS-frequence spectrum physiatrics apparatus on the survival of the random flap in the back of rats. The flap was radiated vertically with the WS-apparatus for 30 minutes, twice daily for 8 days. It was found that on the ninth postoperative day, the survival areas of the radiated flaps was increased significantly (P < 0.001). The isotope (99mTc) clearance, the diaphanousflap sample and SOD content were examined. The results showed that WS-apparatus can reduce the production of oxygen free radicle in the flap, increase the tolerance for ischemia and blood supply of the flap with the proliferation and vasodilation of capillary vessels.

[Role of macrophage in presenting antigens of Schistosoma japonicum].

Indirect ELISA was employed to monitor the serum anti-UEA (urea soluble egg antigen of Schistosoma japonicum) antibody level of mice immunized by: a. UEA pulsed macrophage (Mphi+); b. Cultural supernatant of Mphi+; c. paraformaldehyde fixed M phi (P-Mphi) pulsed with UEA; d. Ammonium chloride treated M phi (NH4Cl-Mphi) pulsed with UEA; e. P-Mphi pulsed with trypsin digested UEA (T-UEA); f. NH4Cl-Mphi pulsed with T-UEA. The normal Mphi, its supernatant and the culture media RPMI 1640 acted as the negative control. The results showed: 1. Serum anti-UEA antibody levels of mice immunized by a and b raised markedly, indicating that the immunogenicity of UEA might be kept up after Mphi processing and the antigenic message could be transferred either by the Mphi+ or by its supernatant; 2. Mice immunized by c and d gave similar results, but the anti-UEA antibody level of the former was higher than that of the latter, suggesting that polyformaldehyde could not alter the UEA binding site on the surface of Mphi; 3. In the case of mice immunized by e and f, the antibody levels were much lower than that of mice immunized by c and d, suggesting that UEA binding sites on Mphi surface as well as UEA immunogenicity could be changed by trypsin.

[Preliminary study on the role of macrophage in processing and presenting antigens of Schistosoma japonicum].

The horseradish peroxidase labeled affinity purified mice anti-UEA (Urea soluble egg antigen of Schistosoma japonicum) antibody was used in enzyme-linked immunoelectrotransfer blot (EITB) to monitor the changes after UEA being processed by M phi. The immune responsive peptides were detected in the culture supernatant and homogenate of M phi pulsed with UEA in vitro (M phi+). After processing by M phi the high molecular weight UEA was cleaved into low molecular weight peptides, as shown, by the reactive bands. They markedly differed from that native UEA or trypsin-digested UEA. The bands of M phi supernatant and homogenate showed similarity with certain quantitative differences. According to the result described above, we considered: 1. UEA could be processed into smaller pieces by M phi, the style of processing is cleavage. 2. The processed peptides might be released to extracellular environment.
