[Echocardiographic evaluation of mitral system and complications of mitral prolapse].

AIM: The analysis of echo-CG morphology of the mitral system in patients with uncomplicated mitral prolapse (MP) in the presence and absence of mitral regurgitation and in patients with severe complications. MATERIALS AND METHODS: Echo-CG was made in 116 patients with MP: with uncomplicated (87) and complicated (29) MP, with infectious endocarditis (13), thromboembolism (6), congestive circulation failure (10). MP was diagnosed according to the standard criteria with visualization of the mitral valve from the parasternal approach. RESULTS: Mitral picture is detailed for 87 MP patients. Classification of the degree of myxomatous degeneration is proposed. 17 MP patients were exposed to loading echocardiography. Exercise may provoke transitory mitral regurgitation. Mitral status has been analysed in patients with serious MP complications. CONCLUSION: The relationship may exist between MP complications, severity of myxomatous degeneration of the mitral valve and the presence of any kind of mitral regurgitation.

[Prolapse of the tricuspid valve].

Of 113 patients who were investigated by echocardiography with regard to mitral prolapse, simultaneous tricuspidal prolapse was detected in 13.3% (15 patients), isolated tricuspidal prolapse (TP) was detected in one patient. Incomplete right bundle branch (RBB) was detected in 11 TP patients, complete RBB in one that might be associated with the involvement in the degenerative process of the proximal part of the right bundle branch in the zone adjoining the tricuspid valve. The characteristic feature of MP and TP was the presence of two systolic clicks.

[Chordal rupture syndrome of the mitral valve].

Fifty-eight patients with chordal ruptures of the mitral valve were examined. The main echocardiographic signs of chordal ruptures of the anterior and posterior mitral cusp were described. The authors provide the results of the use of two-dimensional echocardiography for diagnosing chordal ruptures of the mitral valve depending on the etiological factor of chordal pathology (mesenchymal abnormalities, rheumatic fever, infective endocarditis, coronary heart disease).

[Clinical echocardiography characteristics of cardiac myxomas].

Thirteen patients with heart myxomas (11 myxomas of the left atrium, 1 myxoma of the right atrium, 1 myxoma of the right ventricle) were placed under observation. The clinical manifestations of the myxomas were many and varied and depended on the site, size and mobility of tumor, the presence or lack of autoimmune processes (arthritis, pericarditis, myocarditis), and embolic complications. A high diagnostic value of one- and two-dimensional echocardiography is noted in the diagnosis of myxomas. In view of a possibility of radical surgical treatment early diagnosis of myxomas assumes the leading role for the disease prognosis.

[Echocardiographic diagnosis of the mitral valve cusp flotation syndrome].

The flotation of the mitral cusps is one of the forms of non-rheumatic mitral regurgitation due to disorders in the chordal fixation of the cusps. The authors have examined 26 patients with echocardiographic signs of flotation of the anterior or posterior mitral cusps. In 21 patients there was flotation of the posterior cusp and 5 had flotation of both the cusps. The comparative diagnostic value of separate echocardiographic signs of flotation of the anterior and posterior cusps of the mitral valve is discussed. It is stressed that echocardiography is a valuable method of investigation.
