High-performance liquid affinity chromatography of beta-lactamase.

New silica and organic polymer-based, wide-pore, affinity chromatographic phases have been developed for beta-lactamase purification. The resins have been evaluated in terms of binding capacity, ligand coverage, protein recovery and long-term stability of activated resins prior to ligand attachment. The columns have been applied to the isolation of beta-lactamase from fermentation media. Protein purity following high-performance liquid affinity chromatography was assessed by polyacrylamide gel electrophoresis.

Universal bone cutting device for precision knee replacement arthroplasty and osteotomy.

Since malplacement contributes most to loosening of total knee replacements (TKR), a jig was devised, aligned to and mounted on the tibia, with a 3 degrees of freedom sliding saw. A central distractor, attached to the jig, positions and aligns the knee at 0 degrees or 90 degrees. The femur is then rigidly linked to the jig for bone cutting. Resurfacing designs (Cloutier and Townley) have been regularly implanted, aligned +/- 1 degree. This precision should minimize loosening and improve function.

Anatomy and function of the ligaments of the lower cervical spine in the dog.

Ligaments of the lower cervical spine (C3 to C7) were studied in 27 canine cadavers. The gross anatomy and attachment sites were determined by dissection, and representative samples of the flaval ligament, facet capsules, and dorsal longitudinal ligament were examined by light microscopy and were stained for elastin content. Selected functional spinal units were placed in a specially constructed humidification chamber and were fixed in position, using methyl methacrylate bone cement. The specimens were stressed in flexion and extension, and vertebral displacement was measured before and after severance of the spinal ligaments. The flaval ligament had a marked elastin content and contained randomly oriented collagen bundles. The dorsal longitudinal ligament possessed a negligible elastic fiber content and demonstrated longitudinally oriented collagen bundles. The facet capsules possessed an inner synovial lining and outer fibrous layer typical of synovial joint capsules. In general, the severance of spinal ligaments resulted in small increments of displacement until failure occurred, which was often sudden and complete. The flaval ligament, facet capsules, dorsal longitudinal ligament, and intervertebral disk were the major ligamentous stabilizers of the lower cervical spine.

Beyond retention: National Health Service Corps participation and subsequent practice locations of a cohort of rural family physicians.

BACKGROUND: This report addresses the long-term career paths and retrospective impressions of a cohort of family physicians who served in rural National Health Service Corps (NHSC) sites in return for having received medical school scholarships during the early 1980s. METHODS: We surveyed all physicians who graduated from medical school between 1980 and 1983, received NHSC scholarships, completed family medicine residencies, and served in rural areas. Two hundred fifty-eight physicians responded to our survey with complete information, 76 percent of the members of the cohort who could be located and met the study criteria. RESULTS: In 1994 one quarter of the respondents were still practicing in the county to which they had been assigned by the NHSC, an average of 6.1 years after the end of their obligation. Another 27 percent were still in rural practice. Of the entire group, less than 40 percent were in traditional urban private or managed care settings. CONCLUSIONS: Although only one quarter of NHSC assignees remain long term in their original assignment counties, they provide a large (and growing) amount of nonobligated service to those areas. Of those who leave, many remain in rural practice or work in community-oriented urban practices.
