[Toxic epidermal necrolysis (Lyell's syndrome) after rubella].

Rubella was confirmed clinically and serologically in a 14-year-old boy who had been admitted with mild general symptoms and an exanthematous rash presenting with medium size maculae. Within only three days the exanthema had faded. Five days later the boy again became ill with high fever (40 degrees C) and renewed erythema with at first irregular then rosette-like and widely confluent spots. On admission several nonspecific inflammatory signs were markedly elevated (erythrocyte sedimentation rate, C-reactive protein, alpha-globulin). There were mild abnormalities of left-ventricular repolarization at the electrocardiography. The skin changes were progressive and at first interpreted as erythema exudativum multiforme. Fever persisted, despite antibiotic treatment, and signs of epidermolysis appeared. After the diagnosis of non-staphylogenic Lyell's disease was made high-dosage prednisolone administration was begun (initially 3 mg/kg in 6 hours, then 2.5 mg/kg daily for 8 days). The patient's condition and the electrocardiographic findings returned to normal within 2 days. The epidermolysis was arrested within a few hours and the blisters dried out within 24 hours. There were no complications. Since no drugs were given before the second day of epidermolysis being noted, it is highly likely that rubella precipitated Lyell's disease.

Monoclonal antibodies against antigens expressed by human sperm.

The production and characterization of 21 mouse monoclonal antibodies (TüS1-TüS21) with specificity predominantly for human spermatozoa antigens is described. Reactivity of cells from human ejaculates, peripheral blood and several organs was determined using the alkaline phosphatase-anti-alkaline phosphatase (APAAP)-technique as well as the indirect immunofluorescence test. 15 of the monoclonal antibodies reacted with various regions of human sperm and often also with their precursor cells in the testis. Cross-reactivity with animal spermatozoa was frequently observed.

[Value of detecting pathogens in fertility disorders].

The present study was carried out on 1092 asymptomatic male patients suffering from unexplained infertility without any history or clinical signs of inflammatory diseases of the genital tract. Semen samples were cultured for mycoplasma. In 214 of the patients, we additionally performed bacteriological investigations on seminal fluid, smears from the glans penis or prepuce, and urine specimens. Semen analysis included the common ejaculate parameters. We found significantly elevated counts of mycoplasma in 13% of the semen samples examined; pathogenic isolation rates of bacteria were observed in 36%. Increased counts of various bacterial species associated with high numbers of mycoplasma seem to be one of the causative factors in male infertility. There was not observed any close relationship between the recovery of microorganism and poor semen quality, except for fructose and citrate. When follow-up investigations were carried out in untreated patients, the concentrations of mycoplasma and bacteria in the semen fluid revealed a spontaneous variability of considerable extent, indicating frequent contamination of the semen. Therefore, we recommend to perform a culture control test of the semen prior to specific antibiotic treatment.

Absence of HLA class I and class II antigens as well as beta 2-microglobulin from normal and pathological human spermatozoa.

Human spermatozoa from 87 donors with normal or pathologic semen specimen were examined for the expression of HLA-class I and class II antigens as well as beta 2-microglobulin (beta 2m) using a panel of monoclonal antibodies in an indirect immunofluorescence test. The results make it very unlikely that HLA-class I and class II molecules as well as beta 2m are expressed on human sperm cells.

[Education of the dermatologist in medical mycology].

Principal considerations on graduate medical education in mycology revealed that it should be more effective. For this purpose, an intensive training program has been performed at our department. Here we report on our experiences with this conception as well as our conclusions drawn from it.

Immotile cilia syndrome: reduced chemotaxis and reduced number of intramembranous particles in granulocytes.

The neutrophil granulocytes of four patients with immotile cilia syndrome were investigated by means of freeze-fracture technique. Whereas most granulocytic functions (adherence, phagocytosis, killing of micro-organisms, reduction of NBT, and chemoluminescence) were in the normal range, chemotaxis of the neutrophils was clearly reduced; their plasma membrane revealed a profound reduction in the density of intramembrane particles. An interrelationship between reduced particle density and defective chemotaxis in neutrophils is assumed, but not yet proven.

[Alkali neutralization and alkali resistance in persons with healthy skin and in eczema patients].

Alkali resistance (AR) and alkali neutralisation (AN) time, according to Tronnier and Bussius, were measured at three month intervals on 77 people with healthy skin and compared with test results from 205 patients with occupational dermatitis for whom expert opinion had been given once or several times. It was established that AN and AR react independently of one another. No significant difference was noted in either of the groups with reference to sex, age, occupation and skin type, nor in the ill patients with reference to the duration of the dermatitis, reduction in earning capacity, sensitivity or state of illness at the time opinion was given. Both AN and AR changed usually in the course of occupational dermatitis, however not parallel to one another, nor in relation to the respective skin condition. An and AR results from the dermatitis patients are significantly worse than those from persons with healthy skin tested as comparison. 35% of the healthy persons had pathological AR. In spite of considerable variations in AN in individuals, a satisfactory reproduction of the test results was obtained both for the AN and AR in repeated investigations.

[Herpes virus hominis diseases (author's transl)].

In a 14-year-old girl treated with cytostatics because of leukaemia a severe and chronic atypical herpes virus hominis infection occurred. The same disease also occurred in a 30-year-old man under immunosuppression after a renal transplant. The skin reactions posed diagnostic and therapeutic problems. In both cases withdrawal of the cytostatic agent or drastic reduction of the immunosuppressive drug were successful therapeutically.

[Immunology of malignant melanomas].

The intraperitoneal diffusion-chamber-technique is a closed cell culture system in vivo. Hamster melanoma cells were capable in iso- and heterologous host animals for exponential proliferation. When the host animals had been preimmunized with melanoma cells or hamster lymphocytes a rapid cell lysis was observed in the chambers within 4 hrs after implantation. The cell numbers were reduced about 1-10% of the inoculum. After 6-10 days the cell numbers in chambers from immunized hosts slowly increased in spite of a high level of cytotoxicity in the serum of the animals.

Dose-response hyperplasia and neoplasia from feeding N-2-fluorenylacetamide (2-FAA) to BALB/c mice for varying time intervals.

Hyperplasia induced by N-2-fluorenylacetamide requires the continuous presence of the chemical for its maintenance. Feeding the chemical at levels of 8 to 86 ppm results in a dose-response related urinary bladder hyperplasia and, after 9 to 12 months, in bladder neoplasia at the higher dose levels. Neoplasia of the lungs, liver, parotid gland, lacrimal gland, harderian gland, and submaxillary gland was also observed. Measurement of oncogenic viruses revealed no synergism with 2-FAA and no relationship between virus counts and neoplasia. Calcareous pericarditis was observed and most general toxicological effects occurred in the liver. A pronounced sex difference in response was observed with the males giving greater responses than the females in all instances.
