House dust mite-specific T cells in the skin of subjects with atopic dermatitis: frequency and lymphokine profile in the allergen patch test.

The mechanism underlying positive patch tests with house dust mite-allergen, Dermatophagoides pteronyssinus (Der p), in patients with atopic dermatitis was investigated by isolating T cells from the test sites of two patients. Eighty-five T cell clones (TCC) were established from the epidermis and dermis of lesional skin by the limiting-dilution method with Der p and interleukin (IL)-2. With restimulation assays, 29 of 60 TCCs tested demonstrated specific proliferation; 85% were of the CD3+, CD2+, and CD4+ phenotype. Der p-specific T cells constituted 0.4% to 2.7% of lesional T cells, and they were more frequent in the skin than in the blood of the patients by one order of magnitude. The mitogen-stimulated lymphokine profile of 55 TCCs was assessed; 42% (11/26) of the allergen-specific TCCs secreted IL-4 but almost no interferon-gamma, as described for the Th2 subset of the mouse. Also, six selected TCCs supported IgE secretion by autologous lymphocytes. Only three of 26 allergen-specific, skin-derived TCCs demonstrated a Th1-like lymphokine profile. These results support the specific nature of Der p-induced patch test lesions in patients with atopic dermatitis, and the results demonstrate also that a considerable proportion of lesional T cells are allergen-specific, IL-4-producing T cells that are capable of enhancing IgE production.

[Thalamic infarcts--clinical aspects, neuropsychological findings, prognosis].

Results of clinical investigations and CCT of 23 patients with thalamic infarctions were evaluated retrospectively. Nineteen patients underwent a clinical and 15 a neuropsychological examination one to 5 years after their stroke. Regarding the affected vascular territory, the patients were divided into the following subgroups: paramedian infarctions (posterior thalamo-subthalamic arteries, 13 patients, among them 5 bilateral infarcts); anterolateral infarcts (tuberothalamic arteries, 5 patients); posterolateral infarcts (thalamo-geniculate arteries, 5 patients). The leading symptoms of paramedian thalamic infarcts were disturbance of consciousness, amnesia and vertical gaze palsy. The patients with anterolateral thalamic infarctions became acutely confused and disorientated, whereas those with posterolateral infarcts suffered from focal neurological deficits in the first place. Five patients with leftsided thalamic infarctions of varying localization were aphasic. Two patients died within the observation period. A slight hemiparesis was detected in two patients and a vertical gaze palsy in 4, respectively, by the neurological reexamination one to 5 years after the stroke, whereas the clinical findings in 11 patients were completely normal. However 5 patients had a chronic psychosyndrome. The neuropsychological examination showed in some cases disorders of visual retention and verbal function as well as deficits in concentration. Neither the character nor the degree of the neuropsychological deficits did depend on the affected vascular territory but on the existence of further cerebral infarcts on CT-scan.

Biosynthesis of gallotannins: beta-glucogallin-dependent formation of 1,2,3,4,6-pentagalloylglucose by enzymatic galloylation of 1,2,3,6-tetragalloylglucose.

An acyltransferase was detected in young leaves of pedunculate oak (Quercus robur) that catalyzed the formation of 1,2,3,4,6-penta-O-galloyl-beta-D-glucose, the common precursor of gallotannins and the related ellagitannins. This enzyme depended on beta-glucogallin (1-O-galloyl-beta-D-glucose) as acyl donor; 1,2,3,6-tetra-O-galloyl-beta-D-glucose was specifically required as acceptor molecule, whereas no reaction occurred with the 1,2,4,6-isomer of this substrate. The partially purified enzyme (Mr 260,000) was stable between pH 5.0 and 6.5; highest activities were observed at pH 6.3 and 40 degrees C. Km values of 2.3 and 1.0 mM, respectively, were determined for the substrates beta-glucogallin and tetragalloylglucose. In accordance with stoichiometric studies, the systematic name "beta-glucogallin: 1,2,3,6-tetra-O-galloylglucose 4-O-galloyltransferase" is proposed for this new enzyme.

[Comparison of the action of Cordemcura on selected cardiovascular parameters following intraduodenal and intravenous application in beagles].

The novel cardiotonic Cordemcura was tested for its cardiovascular effects after intraduodenal application to anaesthetized dogs. For this purpose, beagles received Cordemcura intraduodenally as a Tylose suspension, with 4 bolus injections (10(-5), 3 X 10(-5), 6 X 10(-5), and 10(-4) mol/kg) given at 30-min intervals. The test parameters used to analyze the onset of action and its course include: heart frequency (HFR), maximum rate of pressure rise (dp/dtmax) and decline (dp/dtmin) in the left ventricle, cardiac output (CO), stroke volume (SV), total peripheral resistance (TPR) and peripheral arterial blood pressure, systolic (Ps) and diastolic (Pd), and dose-effect curves determined for the individual test parameters. There was a dose-dependent increase of dp/dtmax up to a maximum of 130%, which 3 h afterwards was still about 60% above the initial value. The heart frequency, too, rose dose-dependently by a maximum of 80%, though not being significantly different from the initial value any longer 3 h after the last bolus injection. The Ps was slightly, but significantly decreased with the higher dose levels used. The influence on dp/dtmin and Pd was insignificant. A significant rise was found in the cardiac output, whereas the peripheral resistance showed a slight decline. After 3 h, a significant rise was still seen in the contractility (dp/dtmax), whereas the other parameters were no longer significantly different from the initial values. Moreover, selected dose levels of Cordemcura were tested after i. v. application, and a comparison was made of their effect on heart frequency, peripheral blood pressure, and inotropism. The importance of the findings was discussed.

Aminopyridines

[Comparison of the cardiovascular actions of Cordemcura, isoprenaline and dobutamine in cats].

A comparative study was made of the closed-chest cardiovascular effects of Cordemcura (amrinone), isoprenaline, and dobutamine in anaesthetized cats (n = 17). The haemodynamic parameters used for this purpose include: maximum rate of both left ventricular pressure rise (dp/dtmax) and decline (dp/dtmin), heart frequency (HFR), arterial blood pressure, both systolic (Ps) and diastolic (Pd). The maximum changes of the haemodynamic parameters after increasing iv. bolus injections of the test substances were used to plot the dose-effect curves of the single parameters for each substance. All the 3 pharmaceuticals caused dp/dtmax to rise, with quantitative differences being found. Moreover, quantitatively different positive chronotropic effects have been found. The influence on the remaining haemodynamic parameters was different both in the quantitative and in the qualitative sense. The results were discussed with regard to comparing the cardiovascular effects of Cordemcura with isoprenaline and dobutamine.

Resolution of etodolac and antiinflammatory and prostaglandin synthetase inhibiting properties of the enantiomers.

Etodolac, 1,8-diethyl-1,3,4,9-tetrahydropyrano[3,4-b]indole-1-acetic acid, a clinically effective analgesic and antiinflammatory agent, has been resolved via a chromatographic separation of its diastereoisomeric esters with (-)-borneol. The effects of the enantiomers were studied in vitro on prostaglandin synthetase and on adjuvant-induced arthritis in rats. The biochemical and pharmacological results show that virtually all of the effects of etodolac are due to the (+) enantiomer.

[Effectiveness of symbiotic n2-fixation in leguminous plants, as affected by inoculation with rhizobia, by substrate, n-fertilizing, and 14c-sucrose application (author's transl)].

Cultivation experiments (Mitscherlich-vessels, quartz sand, 15N-labelled soil, 15N-fertilizer) showed, that various strains of Rhizobium lupini (white and yellow lupines) and of Rhizobium leguminosarum (field beans and peas) induced a different N2-fixation of the inoculated plants, the most effective Rhizobium strains being 367a, Cz, T3, 271 (Rh. lupini), and Azotogen (Rh, leguminosarum). Yellow lupines and field bean plants were supplied with N2 from the air considerably better than white lupines and peas after inoculation with the most effective Rhizobium strains. Application of mineral N to the white lupines and peas not only substituted the inhibited N2-fixation, but increased N amounts in the plants. White lupines fixed more N2 under soil conditions than in quartz sand. An experiment with steam-sterilized and 15-labelled soil as a comparative substrate showed, that this finding was mainly caused by an additional Rhizobium infection from the soil. Contrary to field beans and yellow lupines which fix N2 up to ripeness, white lupines and peas finished N2-fixation in the time of flowering. Mineral-N applied at that time was an additional source of N for last-named plants and they utilized it for production of higher protein yields. Continual spraying of white lupine plants with 14C-labelled sucrose solution after the time of flowering caused continuance of N2-fixation up to the stage of ripeness. It is assumed that the cause of this effect was the competition of growing seeds and nodules for the photosynthates. The supply of nodules was inadequate without external sucrose application. Mineral N inhibited the sucrose-induced N2-fixation of white lupine nodules and their consumption of photosynthates. Consequently, the applied 14C was transported into seeds to a larger extent. The investigations allow the following conclusion: Effective N2-fixation requires nodules being a powerful sink for assimilates on the basis of a highly efficient photosynthetic system of the host plant.

[Experiences with the Sarmiento-cast].

The patella-tendon-bearing cast described by Sarmiento was used in the treatment of 96 tibial fractures. Full weight-bearing was permitted even if the fractures were completely unstable and had a marked tendency to shorten. Only in one case weight-bearing led to a dislocation of the fragments. Specific complications were not seen. It was evident that the PTB-cast was most advantageous to the patient in comparison with a conventional cast and sometimes even in comparison with an osteosynthesis, which does not tolerate weight-bearing. To integrate the Sarmiento-cast in the routine of a small or middle sized hospital, we recommend to observe the following rules: 1. The cast is not applied immediately after the accident, but after one to three weeks of conventional conservative treatment. 2. It is made by two specially trained doctors. 3. The controls of the out-patients are performed in the hospital and not delegated to the family doctor.

Quantitative GLC determination of conjugated estrogens in raw materials and finished dosage forms.

A GLC method is described for the quantitative analysis of conjugated estrogens. The procedure was used for the analysis of various estrogen mixtures obtained from formulations containing estrogen sodium sulfate salts. The method involves slurrying or dissolving a portion of the formulation pH 5.2 buffer, hydrolysis with sulfatase enzyme, extraction with ethylene dichloride, anf finally, GLC separation and quantitation of the corresponding trimethylsilyl ethers on a 2.5% diethylene glycol succinate column.
