Complex formation between junctin, triadin, calsequestrin, and the ryanodine receptor. Proteins of the cardiac junctional sarcoplasmic reticulum membrane.

Several key proteins have been localized to junctional sarcoplasmic reticulum which are important for Ca2+ release. These include the ryanodine receptor, triadin, and calsequestrin, which may associate into a stable complex at the junctional membrane. We recently purified and cloned a fourth component of this complex, junctin, which exhibits homology with triadin and is the major 125I-calsequestrin-binding protein detected in cardiac sarcoplasmic reticulum vesicles (Jones, L. R., Zhang, L., Sanborn, K., Jorgensen, A. O., and Kelley, J. (1995) J. Biol. Chem. 270, 30787-30796). In the present study, we have examined the binding interactions between the cardiac forms of these four proteins with emphasis placed on the role of junctin. By a combination of approaches including calsequestrin-affinity chromatography, filter overlay, immunoprecipitation assays, and fusion protein binding analyses, we find that junctin binds directly to calsequestrin, triadin, and the ryanodine receptor. This binding interaction is localized to the lumenal domain of junctin, which is highly enriched in charged amino acids organized into "KEKE" motifs. KEKE repeats are also found in the common lumenal domain of triadin, which likewise is capable of binding to calsequestrin and the ryanodine receptor (Guo, W., and Campbell, K. P. (1995) J. Biol. Chem. 270, 9027-9030). It appears that junctin and triadin interact directly in the junctional sarcoplasmic reticulum membrane and stabilize a complex that anchors calsequestrin to the ryanodine receptor. Taken together, these results suggest that junctin, calsequestrin, triadin, and the ryanodine receptor form a quaternary complex that may be required for normal operation of Ca2+ release.

[Percutaneous transvascular embolization as a palliative measure in bleeding gynecological malignancies of the pelvis].

Between 1981 and 1983 48 patients with an average age of 54 years have had an arterial embolization of internal iliac artery. In 7 patients this was done preoperatively within a clinical study, in 41 ones because of profuse vaginal bleedings. In 6 patients there was a vital indication. In all cases survival time was 152 days. In two cases a re-embolization was necessary. In these two patients survival time was on average 338 days. The manoeuvre was done with particles of gelatin of different size suspended in contrast medium. One complication could be observed, but was not caused directly by embolization. In cases of profuse or vital bleedings which conservatively cannot be stopped embolization is method of first choice with no contraindication.

[Efficacy of an adjuvant norethisterone acetate therapy (NEA) of endometrial carcinoma treated with primary surgery or irradiation].

UNLABELLED: The efficiency of NEA (norethisteronacetate) as an adjuvant hormone therapy in 196 pat. suffering from endometrial cancer has been evaluated in a prospective randomized trial. Primarily 93 patients had been operated on and 103 ones irradiated. From these 45 patients following surgery and 51 ones following radiation therapy had been treated with NEA. Schedule of adjuvant NEA therapy was for a time 6 ms 50 mg/d followed by 6 ms pause and so on till 5 years post op. 5 years survival in the operation group was 91.7 per cent without and 81.6 per cent with NEA, in the irradiated group 43.1 per cent and 44.8 per cent respectively. Side effect (47.9%) were caused by NEA at least twice higher than in control groups. IN CONCLUSION: the adjuvant hormone treatment without hormone receptor analysis is not recommendable.

Early clinical evaluation of a robot arm/worktable system for spinal-cord-injured persons.

In Part I, "Developmental Case Study", this evaluation traces anecdotally the increasing application of microcomputer technology in a research program examining the uses of a robot arm in the rehabilitation of the high-spinal-cord-injured person. This program, supported by the Veterans Administration since 1974, built upon still earlier VA-supported work at Johns Hopkins on powered upper-limb prostheses. The Johns Hopkins University powered shoulder prosthesis served as the basis for the Robotic Arm/Worktable System, whose evolution is described in applications ranging from handling simple reading materials to self-feeding and the operation of a personal computer, all under the user's control through the same chin control interface employed for the control of the user's power wheelchair. Part II of this report focuses on the results of clinical evaluation in the development of the system. In the early stages of development, evaluation was provided by four subjects who used the system regularly for periods ranging from 4 months to a year. A later stage of evaluation featured a total of 16 subjects located in two VA Medical Centers, using the system for periods of from a few days to 4 months in duration. The VA's new Rehab R&D Evaluation Unit has begun an intensive evaluation process based upon 25 of the Robotic Arm/Worktable Systems currently on order.

[Cathepsin D activity in human ovarian carcinoma].

In the tissue of 54 ovarian carcinomas and 38 normal human ovaries the activity of cathepsin D was determined by means of the Anson-method. In ovarian carcinomas the mean activity of cathepsin D was 4515 pmol T/s/g tissue, in normal ovarian tissue only 2119 pmol T/s/g. The difference is significant (p less than 0,001). The dependency on age of the activity of cathepsin D suitable for normal ovary, got lost in cancer tissue. There is no relationship between the amount of activity of cathepsin D and the stage of the tumour. Extreme high activities of cathepsin D were found in metastatic ovarian carcinomas. The activity of cathepsin D is higher in endometrioid and papillary than in mucigenous and hypernephroid ovarian carcinomas. The different activities of cathepsin D were discussed with regard to the invasive growth of cancer and to deranged protein metabolism.

[Therapy of puerperal mastitis].

Mastitis puerperalis is a severe complication of puerperium. Results of treatment of 70 patients with mastitis by low-dose conventional X-ray irradiation and secondary inhibition of lactation by conservative physical measures are reported. Breast abscesses were observed only in 6 of 70 patients. Therefore the antiinflammatory irradiation with low-dose X-rays is a suitable method of treatment.

[Extended medical care for patients with ovarian carcinoma (author's transl)].

Reported in this paper is a scheme of extended medical after-care for patients with ovarian carcinoma at the authors' outpatient department. An account is given of special consultations with routine care as well as individual postoperative treatment, using cytostatics and hormones. A strong demand is made for centralised treatment of patient with ovarian carcinoma which should cover the entire population within reach of the hospital concerned. Such centralisation also should be backup with sufficient personnel, equipment, and laboratory capacity.

[Post-pill amenorrhea following hormonal contraceptive therapy in adolescence (author's transl)].

Disorders of the ovarian function after going off hormonal contraceptives are a serious complication when appearing as postpill amenorrhea in young nullipara. We could diagnose a postpill amenorrhea in 31 cases out of 52 patients suffering from functional ovarian disorders after stopping the application of hormonal contraceptives. 19 woman younger than 28 years and wanting to get pregnant had begun as adolescents to take contraceptive drugs. Concerning the etiology of post-pill amenorrhea the type of contraceptive is of importance as well as predisposing factors such as late menarche, unstable menstruation and other cycle anomalies. Our patients majority had taken gestagen-accentuated combination drugs. During adolescent age there should be prescribed only sequential drugs if necessary.

[The utilisation of the leukocyte migration inhibition test (L.M.T) in carcinoma of cervix uteri (author's transl)].

The leukocyte migration inhibition test (L.M.T.) was utilized for in vitro studies of cell mediated immunity in gynecologic malignomas. Autogenous peripheral blood leukocytes from 50 patients with a carcinoma of the cervix uteri were studied in about 100 tests. There was found an inhibition of the migration of the autogenous leukocytes in 25 of 47 tests of patients with a CCI without evidence of metastasis or widespread disease. However, the migration of autogenous leukocytes from patients with metastasis or widespread disease (CCII) were only inhibited in 14 of 46 cases. At normal tissue were inhibited in 15 of 89 cases the migration of the autogenous leukocytes.

[Primary carcinomas and sarcomas of the vagina - therapeutic results (author's transl)].

Treatment was given to 41 cases of primary carcinoma and nine cases of sarcoma of the vagina, over ten years at the Gynaecological Hospital of the Medical Academy of Erfurt. Combined therapeutic radiation has remained to be the optional method. Surgery has continued to be justified, depending on indication. Treatment of vaginal sarcoma has remained to be problematic. All therapeutic facilities available at appropriately equipped carcinoma centres should be used on such cases.

[Effectiveness of adjuvant chemotherapy in ovarian carcinoma].

A report is given about the efficiency of the so called adjuvant chemotherapy in 26 cases (26 patients suffering from ovarian cancer in the stage T1M0N0) in a time of 10 years (1968-1977).--After the primary operation-therapy, a part of the patients (9) got the cytostatic drugs Trenimon or Cyclophosphamid. The patients of the control group got nothing. The survival times of both groups showed no significant distinctions. -- The results show that the adjuvant chemotherapy in cases of ovarian cancers in stage T1M0N0 can not be recommended because there are too many side effects of the these cytostatic drugs.

An assistive equipment controller for quadriplegics.

A joystick is traditionally used to manipulate electrically powered wheelchairs and radio-controlled model aeroplanes. Extensive modifications of the joystick have resulted in the development of an inconspicuous chin-operated control; the latter enables quadriplegics with high-level spinal cord injuries to operate not only electric wheelchairs but also a complete range of other assistive devices, including robotic arm and environmental control systems currently under development. Through the use of the chin-operated control such quadriplegics can manipulate any of these devices or systems without the help of an attendant and without interfering with the use of a mouthstick.

[Effectiveness of supplemental bleomycin therapy in vulvar cancer].

It is reported about the application of bleomycin within a randomized prospective investigation. Bleomycin was used for additional chemo-therapy at the vulva-carcinoma. There were treated with Bleomycin 27 of 55 women from two gynaecological hospitals. -- After a period of two years and a half there can not be shown by the "life-table-method" advantage neither in comparison to the only surgical therapy not to the cryo-surgical treatment.
