Primary intratesticular sarcoma of the testis: a rare manifestation of testicular cancer.

A 57-year-old white male presented with a 5-month history of a painless right testicular mass. Physical examination revealed a right testicular tumor. The patient underwent a right inguinal orchiectomy. Histopathologic evaluation showed a primary spindle-cell sarcoma of the testis. A retroperitoneal lymph node dissection was performed and revealed no lymphatic metastases. There was no recurrence of the disease over a 2-year follow-up period.

Dose escalation study with rhenium-188 hydroxyethylidene diphosphonate in prostate cancer patients with osseous metastases.

The aim of this study was to determine the maximum tolerated dose of rhenium-188 hydroxyethylidene diphosphonate (HEDP) in prostate cancer patients with osseous metastases who are suffering from bone pain. Twenty-two patients received a single injection of escalating doses of carrier-added 188Re-HEDP [1.3 GBq (35 mCi), 2.6 GBq (70 mCi), 3.3 GBq (90 mCi) and 4.4 GBq (120 mCi)]. Blood counts and biochemical parameters were measured weekly over a period of 8 weeks. Haematological toxicity (WHO grading) of grade 3 or 4 was considered unacceptable. Clinical follow-up studies including methods of pain documentation (medication, pain diary) were performed for 6 months after treatment. In the 1.3-GBq group, no haematological toxicity was observed. First haematotoxic results were noted in those patients with a dose of 2.6 GBq 188Re-HEDP. In the 3.3-GBq group, one patient showed a reversible thrombopenia of grade 1, one a reversible thrombopenia of grade 2 and three a reversible leukopenia of grade 1. In the 4.4-GBq group, thrombopenia of grades 3 and 4 was observed in one and two patients (baseline thrombocyte count <200x10(9)/l), respectively, and leukopenia of grade 3 was documented in one patient. The overall nadir of thrombopenia was at week 4. The individual, maximum percentage decrease in thrombocytes in the 1.3-, 2.6-, 3.3- and 4.4-GBq groups was 17%, 40%, 60% and 86%, respectively. In two patients, a transient increase in serum creatinine was observed (max. 1.6 mg/dl). Pain palliation was reported by 64% of patients, with a mean duration of 7.5 weeks. The response rate seemed to increase with higher doses, reaching 75% in the 4.4-GBq group. It is concluded that in prostate cancer patients, the maximum tolerated dose of 188Re-HEDP is 3.3 GBq if the baseline thrombocyte count is below 200x10(9)/l. In patients with thrombocyte counts significantly above 200x10(9)/l, a dose of 4.4 GBq might be tolerable. Thrombo- and leukopenia are the most important side-effects. Pain palliation can be achieved in 60%-75% of patients receiving a dose of 2.6 GBq or more of 188Re-HEDP. Studies in a larger patient population are warranted to evaluate further the palliative effect of 188Re-HEDP.

Positron emission tomography in the clinical staging of patients with Stage I and II testicular germ cell tumors.

OBJECTIVES: To evaluate the accuracy of fluorodeoxyglucose positron emission tomography (PET) compared with computed tomography (CT) staging in patients with Stage I and II testicular germ cell tumors (GCTs). METHODS: From January 1995 to July 1997, in 37 patients with clinical Stage (CS) I (n = 25) and CS II (n = 12) GCT (24 nonseminomas, 13 seminomas), PET and CT were compared in the initial staging. After PET, the patients with nonseminomatous GCT were staged surgically by retroperitoneal lymph node dissection and the patients with seminomatous GCT were followed up clinically. RESULTS: Correct staging by PET was achieved in 34 of 37 patients compared with correct CT staging in 29 of 37 patients. Of 10 metastatic lesions, 7 and 4 were detected by PET and CT, respectively. PET did not show false-positive signals. PET was unable to detect vital cancer with a maximal diameter less than 0.5 cm or teratoma at any size. CONCLUSIONS: PET was useful for detecting viable tumor in lesions that are visible on CT scan and, thus, it may omit false-positive CS II lesions. However, PET was not able to identify mature teratoma. In this study, PET did not improve the staging in patients with CS I tumor.

Clinical course and histopathologic risk factor assessment in patients with bilateral testicular germ cell tumors.

OBJECTIVES: To determine whether the clinical course or the histopathologic risk factor assessment of the secondary tumor justifies early detection of the premalignant testis, since only 5% of contralateral testicular biopsies in the case of a testicular tumor reveal a testicular intraepithelial neoplasia (TIN). METHODS: From 1975 to 1997, 30 patients with bilateral germ cell tumors were treated; histologic and clinical data were available in all patients. In 12 patients, histopathologic re-evaluation and immunohistochemical staining of both tumors was performed for risk factor analysis. RESULTS: At the time the primary tumor was diagnosed, the mean patient age was 28 years. The metachronous secondary tumors were detected after a mean of 5.6 years. Seminoma was the predominant histologic finding in primary (53%) and secondary (56%) tumors. Eighty-three percent of patients had clinical Stage I (Lugano classification) disease at the diagnosis of the secondary tumor and 17% Stage II. Only 3 of 12 secondary tumors showed vascular invasion. The proliferation analysis (MIB-I score) showed a mean proliferation rate of only 34% within the primary and 26% within the secondary tumors. CONCLUSIONS: The results of our study show that the low clinical stage and good outcome associated with a histologic low-risk score of the secondary tumor does not mandate contralateral biopsy to detect TIN at the time of diagnosis of the first tumor.

Hydronephrosis after aorto bifemoral graft surgery: a marker for late graft complications.

Ureteral obstructions are serious late complications after aortoiliac reconstructive vascular surgery, which lead to loss of kidney function if they remain untreated. One case report serves to describe the incidence, aetiology, clinical presentation and treatment options of an obstructive uropathy following graft surgery. Hydronephrosis due to a ureteral obstruction is considered as a "marker" of graft complication. Therefore, ultrasound examination and close follow-up beyond 1 year are recommended in all patients who undergo aortoiliac surgery.

Squamous cell carcinoma of the penis.

Squamous cell carcinoma of the penis is a rare disease in Western countries. In geographical locations where infantile circumcision is not routinely practised and genital hygiene is poor, penile cancer may comprise 10-20% of all malignancies. Superficial tumours (Ta-Tl) should be treated with organ-preserving therapy. Partial of total penectomy is recommended for invasive penile carcinoma (stage T2 or higher). Currently, management of the ilioinguinal lymph nodes is controversial. The value of radiation therapy and chemotherapy is still uncertain; these treatments are only palliative therapy modalities.

Extracorporeal shock wave lithotripsy and glycosaminoglycans in urine.

In 50 calcium oxalate stone-forming patients, the total excretion of glycosaminoglycans (GAGs) and of four of its subgroups [chondroitin-4-sulfate (CS-A), chondroitin-6-sulfate (CS-C), dermatan sulfate (DS), and hyaluronic acid (HY)] were investigated before ESWL and on the following 5 days. The standard value was determined by reference to a group of healthy test subjects. The excretion of GAGs was significantly higher in healthy test persons than in stone-forming patients. Twenty-four hours after ESWL, GAG excretion increased significantly but returned to normal values in the course of three days. ESWL had no influence on the proportional composition of GAG subgroups CS-A, CS-C, DS and HY. The increase in GAG excretion after ESWL indicates a transient injury of renal tissue and of the mucous layer lining the urothelium, respectively. This lesion, however, can be regarded as temporary with restitutio ad integrum later.

Percutaneous chemolysis--an important tool in the treatment of urolithiasis.

Persistent residual calculi after therapy, i.e. extracorporeal shock wave lithotripsy (ESWL), percutaneous nephrolitholapaxy (PNL), as well as pyelo- and nephrolithotomy are big problems in the treatment of urolithiasis. Furthermore, the therapy of stones is problematic in patients with inadequate drainage, impaired kidney function, or with high risks against anaesthetics. Between 1991 and 1997 percutaneous antegrade chemolysis was carried out in eleven patients. In nine of them complete dissolution of stones was achieved. In two further cases, in which calcium oxalate was the main component of the stones, chemolysis was unsuccessful. Through our own cases and under consideration of the literature, we will show that percutaneous chemolysis in these cases is useful and effective in the treatment of urolithiasis.

[Indications for nuclear medicine therapy in advanced prostate carcinoma].

For patients with advanced prostate cancer efficient therapy of painful bony lesions is the primary goal of interdisciplinary treatment strategies. Preservation of quality of life appears to be the main aim rather than prolongation of life. Apart from oral pain relief and local irradiation systemic treatment with radionuclides offers low-risk radiotherapeutic strategies for the palliation of painful, multifocal osteoplastic bone metastases. Depending on the radiopharmaceutical substance chosen response and reduction of pain are described in 65-80%. The duration of pain relief lasts between 6-12 weeks. During this time the morphine based medication can be reduced and in some cases withdrawn which positively effects quality of life. After improvement of myelosuppression treatment with radionuclides can be repeated. Patients have to be hospitalized for two days because of protection from radiation procedures.

Tumour induction as a consequence of immunosuppression after renal transplantation.

Immunosuppressed recipients of organ transplants have a higher incidence of carcinoma than the general population. A retrospective analysis was made at the Department of Urology of Bonn University, investigating 236 renal allograft recipients as to the incidence of neoplasms before and after transplantation. Eleven patients developed malignant tumours after transplantation. In 4 out of these 11 patients, case history showed pre-existing malignancies. Two of the 4 patients developed a second tumour, while the other two had tumour progression (latency period 21-77 months). Three of the 4 patients died of their tumours 21, 42 and 77 months after transplantation, whereas one female patient is still alive and free of neoplasms 32 months after transplantation. In 7 out of these 11 patients de novo tumours were diagnosed (latency period 3-88 months). All of them are still alive (NED between 15 and 85 months), six of them with good transplant function. There was no difference to be seen in the incidence of malignancies between kidneys supplied by Eurotransplant (n = 40) and ABO compatible kidneys from our own donors (n = 196). The higher incidence rate of neoplasms in transplant recipients requires high standards in preventive measures. Any suspicious change that may occur in the course of a thorough follow-up of transplant recipients must be removed and examined histologically. Patients with previous malignant diseases must be payed special attention, since they frequently tend to develop another malignant tumour and progression of existing tumours, respectively. As far as immunosuppression is concerned, therapeutic guidelines for the treatment of transplant recipients do not differ from those set up for patients on haemodialysis. Since immunosuppression with increased rates of tumour incidence can also be observed in dialysis patients, the mere fact of increased incidence of neoplasms cannot be taken as an argument against transplantation. With a more or less equal risk of tumour incidence the crucial factor should be the higher quality of life for transplant recipients.

[Testicular loss due to hemorrhagic infarct in Tauber antegrade scrotal varicocele sclerotherapy].

A 38-year-old patient with a grade 3 testicular varicocele was treated by antegrade scrotal sclerotherapy. Although performed technically accurate, antegrade sclerotherapy led to a haemorrhagic infarction of the testis by complete occlusion of venous drainage. The testis had to be removed. This complication has not been described yet and patients should be informed about this rare event.

Management of complete ureteral replacement. Experiences and review of the literature.

The common treatment for patients with extensive damage to the ureter is complete ureteral replacement, combining Boari flap-psoas bladder hitch and downward mobilization of the involved kidney, with complete ideal replacement of the ureter, renal autotransplantation, of elective nephrectomy. Three case reports serve to describe two options of reconstructive treatment for complete ureteral replacement. The operative techniques, their limits, their postoperative results, and the treatment alternatives are discussed with due regard to recent literature. In the case of ileal replacement we have used a very short ileal segment to reduce the absorption surface of the ileal mucosa. Reflux prevention of the ileal segment was performed by creating an invaginated distal ileum nipple which was additionally fixed at the Boari flap by a third stapler row (auto suture TASS) to prevent potential nipple-gliding.

Rhenium-186 HEDP: palliative radionuclide therapy of painful bone metastases. Preliminary results.

The aim of this study was to evaluate the efficacy of rhenium-186 hydroxyethyledine diphosphonate (Re-186 HEDP) for pain relief in patients with disseminated bone metastases primarily from prostate or breast cancer. Up to now, 44 patients taking analgesics were entered in this study and received one or more injections of 1295 MBq of Re-186 HEDP. An analgesic effect of more than 20%, evaluated by using a verbal rating scale (VRS) and a visual analogous scale (VAS), was defined as significant and could be achieved in 60% of these patients. Duration of clinical response averaged 1-4 months (median 5.5 weeks). Side effects such as a moderate decrease of platelets or an increase of pain for 1-2 days (flare-up effect) were observed. Radioactive treatment with Re-186 HEDP appears to be a useful compound for the palliation of painful skeletal metastases and improvement of the remaining quality of life.

Ureterosciatic hernia.

We report on a patient with a hydronephrosis of the left kidney caused by an ureterosciatic hernia. Curlicue formation is a characteristic sign in urography. In case of symptomatic sciatic hernia the hernia should be treated by operation.

Glycosaminoglycans in urine and extracorporeal shock wave lithotripsy.

Is the transplant-preserving management of renal allograft rupture justified?

OBJECTIVE: To evaluate the transplant-preserving management of renal allograft rupture. PATIENTS AND METHODS: From April 1982 to January 1994 a total of 238 renal transplantations were performed on 227 patients. Eight cases (3.5%) of renal allograft rupture occurred. Transplant nephrectomy was necessary in one patient. Seven patients were surgically treated with collagen foam, fibrin glue and vicryl mesh. RESULTS: In all seven cases treated conservatively, renal salvage and satisfactory graft function was achieved. After a mean follow-up of 52.9 months (range 2-94) the mean creatinine level was 15.6 mg/L (range 11-21). Of these seven patients with renal allograft rupture one returned to haemodialysis 22 months after transplantation and had several episodes of rejection. Almost 4.5 years after renal allograft rupture, the creatinine value in six of the seven patients was only slightly higher than the mean creatinine values of all donor kidney recipients (14.4 +/- 5.5 mg/L). CONCLUSION: There should be an attempt to salvage the transplant after allograft rupture in all cases. Using these transplant-preserving techniques, renal function could be achieved for all patients with allograft rupture.

Laparoscopic varicocelectomy: indication, technique and surgical results.

OBJECTIVE: To assess the efficacy of laparoscopic techniques for the operative therapy of varicocele. PATIENTS AND METHODS: Fifty-one patients who underwent laparoscopic varicocelectomy due to subfertility and/or pain between June 1992 and December 1994 were evaluated. RESULTS: In all patients, except one with a remaining slight reflux on post-operative Valsalva's manoeuvre, operative therapy was effective and no major complications occurred. The procedure was minimally invasive, effective and, because the anatomy was readily seen, multiple veins and/or collaterals were easily assessed and the surgery precise. CONCLUSION: The laparoscopic technique is an efficient, minimally invasive operation with optimal results. After a brief period of training the operation can be performed in 15-30 min and is therefore no longer than embolization techniques. Treatment of both sides in one session was not a problem.
