[Alcoholic encephalopathies in autopsy material].

A report is given on incidence, complications, causes of death, and morphological diagnosis of chronic alcoholism in autopsy material. Chronic alcoholism was registered in 8.4% of all autopsies. The average age at death of alcoholics, particularly female, was significantly lower than that of all autopsy material. Pneumonia was the most frequent direct cause of death (21.1%). Acute ethanol intoxication was in second position (15.8%). Wernicke's encephalopathy was observed in 0.6% of all autopsies (18% of alcoholics), cerebellar atrophy occurred in 0.4% (12%), and central pontine myelinolysis was found in 0.2% (7%). The mean brain weight of alcoholics was 1310 g as compared with 1400 g of controls. Problems of clinical and postmortal diagnosis as well as medical and social consequences are discussed.

[Benign fibrous histiocytoma of bone].

In this paper we report one case of a poor rarely benign lesion of bone. We describe the clinical, radiological and histological diagnosis and differential-diagnostic procedures. Good results of operative treatment were reported after excochleation and transplantation of allogen or autogen bone-chips.

Bone Transplantation

[The nosological situation of hereditary motor and sensory neuropathies (HMSN, Charcot-Marie-Tooth disease, neural muscular atrophy)].

Hereditary motor and sensory neuropathies (HMSN) comprise a heterogeneous group of disorders. Since phenotypic manifestations are similar in most families, classification is based on differences in the mode of inheritance, onset and progression of the disease, nerve conduction velocity and nerve biopsy findings. Autosomal dominant, autosomal recessive, X-dominant and X-recessive forms, substantial intrafamilial differences, intermediate forms and the combination of neuropathies with spinocerebellar degeneration within one and the same sibship have been described. On the basis of selected own cases it is demonstrated that there is a broad spectrum of functional and structural abnormalities depending on the progression of the disease and on the site of nerve studied (proximal or distal part). Both the neuronal and hypertrophic variants begin with axonal degeneration of the dying back type followed by segmental demyelination and variable degrees of hypertrophic Schwann cell proliferation. Constantly, posterior columns of the spinal cord reveal fiber loss. Since the molecular basis of the different forms remains to be clarified it seems to be of greater interest to underline common features than to separate seemingly different nosological entities. It is suggested that the latter are partly the result of a selection of cases with a variable severity. Evidently, the syndrome of myatrophic ataxia comprises apart from "pure" HMSN with unsignificant degenerations of posterior columns and "pure" Friedreich's ataxia with mild peripheral nerve fiber loss intermediate forms.

Adult

[The hypophysis and haemoblastoses].

In necropsies of 165 patients with malignant haemoblastoses, infiltrates in the pituitary were found in 38 cases (23%). The most frequent occurrence was found in lymphoblastic leucosis (33.3%) and in non-Hodgkin lymphomas (29%). Hypophyseal manifestations were found in 21% for myoblastic leucosis and in 5.5% in case of Hodgkin lymphomas. As regards the location, the capsula showed an affection of 55.2%, the anterior and posterior lobes together one of 21.1%, the neurohypophysis one of 15.8% and the anterior lobe one of 7.9%. There are different opinions with regard to the development: exclusively metastatic processes or local formation of the infiltrates. The latter opinion is favoured by the frequent detection of cells of exemplary haemopoiesis between the Dorsum sellae and the outer layer of the hypophyseal capsule (Stratum periostale) in patients without haemoblastosis as well as by the fact that in half of the patients the clinical picture of meningosis leucotica only developed in the stage of haematological full remission, that is, when there were no longer any blastomatous cells in the blood.

Hodgkin Disease

[Metastases in the hypophysis].

Among 1521 malignant extracranial tumours, 64 metastases (4.2%) were found in the pituitary. The most frequent occurrence was found for carcinomas of the breast (21.4%) and for bronchial malignomas (5.9%). A statistically significant difference with regard to the location in the anterior and the posterior lobe was not found in our own patients. A remarkable fact is the high degree of affection of the pituitary gland in case of malignant melanomas.

[The problem of cardiac metastases].

Report on metastases of the myocardium of a bronchial carcinoma in a 66-year-old woman. On the basis of an own case as well as of literature the author adopts a definite attitude to frequency, localisation and diagnostics of the secondary tumours of the myocardium.

Aged

[Aneurysms of the coronary arteries].

Report on a mycotic and arteriosclerotic aneurysm of the right coronary artery in a 13-year-old girl and a 43-year-old man. With the help of the casuistics and the existing literature the author adopts a definite attitude to genesis, frequency, localisation and complication of the aneurysma.

Adolescent

[Distinct hypokalemia in leukotic infiltrates of the kidney].

Report on a 74-year-old female patient falling ill from a chronic myelosis. Due to leukotic infiltrates of the kidneys renal loss of potassium and extreme hypopotassaemia appeared. On the basis of the present literature clinical symptomatology and morphological finding are discussed.
