[New diagnostic and therapeutic prospects in peripheral nodules of the lung: surgical video thoracoscopy].

The new findings and results of laparoscopic surgery lead us to apply this technique in thoracic surgery. Thoracoscopy is an old diagnostic procedure, but advanced imaging techniques, improved optics and new classes of instrument have paved the way for many diagnostic and therapeutic approaches, before not feasible. Now it is possible to treat spontaneous pneumothorax, removal of mediastinal and peripheral tumors, with minimally invasive surgery. It is our belief that thoracoscopic surgery will be a valuable tool in the near future for a variety of chest disorders.

[The Roux loop syndrome: a postgastrectomy syndrome. Description of 2 clinical cases].

The Roux-En-Y operation has been used to control enterogastric reflux occurring after previous distal gastric resection and it has been used extensively in the treatment of alkaline reflux. Recently a postgastrectomy syndrome characterized by chronic abdominal pain, persistent nausea and vomiting that is exacerbated by eating and that develops after the Roux-En-Y operation has been described. It has therefore been postulated that the Roux-En-Y limb acts as a functional obstruction and causes the symptoms.

[Diagnostic value and therapeutic limits of endoscopic polypectomy of the stomach].

Digestive endoscopy represents the current therapy of the gastric polyps. These lesions are endoscopically found in an unexpected number of patients, because of the frequent failure of radiology by itself. Furthermore endoscopic radical polypectomy is mostly possible for the inflammatory and adenomatous polyps. Otherwise endoscopy favours the early detection of malignant changes and coexisting per-cancerous gastropathy. A wide review of the literature is given.

Intramedullary osteosynthesis in fractures of the upper limb.

The authors present forty-four fractures of the upper limb treated by osteosynthesis using Rush intramedullary nails. These included metaphyseal, spiral, and multi-fragmentary fractures. This extension of the classic indications for this method is suggested as an alternative to rigid internal fixation. It is preferred because of its simplicity, rapidity of performance, and because there were no cases either of retarded consolidation or infection. In these respects it is superior to the traditional method using screws and plates which at some sites is very difficult to carry out.

[Presumed natural history of peritonitis caused by encapsulating membranes. Clinical aspects and therapy].

Incapsulating peritonitis is rare, casually detected by surgery or autopsy. Its pathogenesis and evolution are uncertain. Only 200 cases are reported in the world literature, but its frequency is diminishing probably in relation to the reducing spread of tuberculosis. Capsulating membranes seem to grow from a poorly cellular connective deposition in many layers on the intestinal peritonium, and are preceeded by inflammatory ascites. When mechanical occlusion occurs, surgery is the choice therapy, to remove the obstacle and, if possible, the membranes too. A wide review of the literature is given.
