[The diagnosis and treatment of blunt renal trauma].

225 patient with blunt renal trauma were treated from 1955 to 1994. Coexistent injuries of other organs were noted in 79 patients (35.11%). The extent of renal trauma was not demonstrated by hematuria. B-ultrasonography seemed to be best for the diagnosis of renal trauma. Abdominal incision was helpful to explore injuries of other organs and manage renal trauma. Early treatment of renal trauma was necessary for the treatment of shock and prevention of serious complication.

The retinoblastoma protein binds to RIZ, a zinc-finger protein that shares an epitope with the adenovirus E1A protein.

The retinoblastoma protein (Rb) is a target of viral oncoproteins. To explore the hypothesis that viral proteins may be structural mimics of cellular proteins, we have searched cDNA libraries for Rb-binding proteins. We report here the cloning of a cDNA for the protein RIZ from rat and human cells. RIZ is a 250-kDa nuclear protein containing eight zinc-finger motifs. It contains an Rb-binding motif that shares an antigenic epitope with the C terminus of E1A. A domain is conserved between RIZ and the PRDI-BF1/Blimp-1 differentiation factor. Other motifs of RIZ include putative GTPase and SH3 (src homology domain 3) domains. RIZ is preferentially expressed in both adult and embryonic rat neuroendocrine tissues. It is also expressed in human retinoblastoma cells and at low levels in all other human cell lines examined. While the function of RIZ is not yet clear, its structure and pattern of expression suggest a role for RIZ in transcriptional regulation during neuronal differentiation and pathogenesis of retinoblastoma.

Quantitative analysis of (l)-ephedrine and (d)-pseudoephedrine in plasma by high-performance liquid chromatography with fluorescence detection.

Quantitative analysis of (l)-ephedrine (l-Ep) and (d)-pseudoephedrine (d-Ps) in plasma by high-performance liquid chromatography (HPLC) is described. The newly developed method is based on a precolumn derivatization with 5-dimethyl-aminonaphthalene-1-sulfonyl chloride (DNSC1) in acetonitrile. The diastereomers formed were separated on a reversed phase column by HPLC with fluorescence detection employing 0.6% phosphate buffer (pH 6.5)-methanol (3:8, v/v) as mobile phase. The detection limit of each Ephedra alkaloid stereoisomer was 0.5 ng at a signal-to-noise ratio of 3:1, the linear response to each stereoisomer being 1-800 ng.ml-1. The plasma level profile of l-Ep and d-Ps in guinea pig was investigated by this method. The determination of l-Ep and d-Ps in plasma of a volunteer after oral administration of Xiao Qinglong Heji was also performed.

[Evaluation of head-chest lead ECG in the diagnosis of acute right ventricular infarction].

The clinical features, hemodynamic parameters, Wilson's lead and head-chest (H-C) lead ECG of 23 cases with acute right ventricular infarction (ARVI) were analyzed. Results showed that 13 cases revealed clinical right heart failure, but 10 of them had clear lung fields. 11 cases showed hypotension. 20 cases had mean right atrial pressure (MRAP) > or = 10 mmHg. The remaining 3 cases had MRAP between 8 mmHg and 10 mmHg, but it was > or = 10 mmHg after volume loading. The ratio of MRAP to pulmonary wedge pressure was > 0.65 and the right ventricular stroke work index < 5.0 g.m/m2 in all the 23 cases. ST elevation > or = 1 mm in V3R to V6R was found in 12 cases (52%), while ST elevation > or = 1 mm in HV3R to HV8R in 20 cases (87%). The results showed that the diagnostic accuracy for ARVI with H-C lead (right chest) ECG is higher than that with Wilson's lead. H-C ECG is better correlated with hemodynamics in patients with ARVI.
