[Risk of laparoscopic surgery. 100 records of complications. A qualitative study].

A total of 100 accidents occurring during laparoscopic surgery between 1969 and 1993 were studied. Fifty-five of the cases concerned gynaecology surgery over a 24 year period. The other 45 concerned digestive surgery over a 5 year period (1989-1993). The surgical risk was not statistically greater compared with open surgery. Overall morbidity was 1%. Overall mortality was 0.5/1000. A more precise statistical analysis was recently reported. The examination of the 100 cases was qualitative rather than quantitative. The study demonstrated an imbalance between age (mean age 38 years) in patients with major pathology initially and the gravity of the induced accidents: 26 deaths for 100 observations. These deaths were related to technology trocar, capnoperitoneum, electrocoagulation. Technical skill, cautiousness, vigilance and humility are qualities the surgeon must have to perform successful operations. The irreversible acceleration of the new surgical technique should not cloud the basic precepts of sound surgical technique.

[Acute left colonic obstruction of neoplastic origin. Study of a series of 128 cases].

A retrospective study of 112 patients with acute obstructive carcinoma of the colon (added to 16 prospective recent cases), whose mean age was 74 years, with metastasis or peritoneal carcinosis in 23% of the cases, shows a high operative mortality (38.4%), an average survival of 804 days and a five years survival actuarial rate of 17.5%. 80% of further deaths were due to cancer. These results and the data of literature encourage the authors to suggest, whenever a curative resection is to be considered, a decompressive proximal colostomy combined to a short investigating laparotomy, followed by a two-stage resection 2 or 3 weeks later.

[Intra-operative precautions and a proposition for a postoperative monitoring protocol in hepatic hydatidosis].

In the expectation of new biological technics, immunoelectrophoresis is currently the most valuable one owing to its very high specificity. Precipitation line 5 which develops usually disappears after surgery, provided there is no recurrence. However, it may persist in many cases despite repeated negative check-ups. In view of such observations, there is some concern about the peroperative sampling methods commanded for the test, as well as regarding the postoperative follow-up procedure and schedule considering costs that are involved.

[Plurifragmentary ruptures of the kidney. Wrapping by absorbable mesh].

In two patients presenting with rupture of the kidney, the fragments of that organ were kept together by wrapping in a resorbable mesh after haemostasis and trimming. The post-operative period was uneventful. This method facilitates conservative renal surgery, even in cases with severe damage.
