[Condition and satisfaction of patients after gynecologic surgery].

The effects of gynaecological operations on the well-being and satisfaction of patients can be viewed as important supplements to quality assurance which is mainly based on clinical parameters and medical ratings. In the present study 215 patients undergoing gynaecological operations assessed different components of their subjective health status at times of admission to the hospital, at discharge and six weeks after discharge. Additionally, the patients gave satisfaction ratings at discharge. The study resulted in demonstrating considerable improvements of complaints and well-being that continued into the postdischarge period. The improvements were most pronounced in the cases of patients with vaginal hysterectomy. Satisfaction with the hospital department was high, especially with regard to the care given by the physicians and the nurses. After mastectomy, women expressed problems with their self-image and with their partners. Considerations to take the results of the study into account for quality assurance are discussed.

[Primary therapy of acute angle-block glaucoma].

Primary therapeutic measures in 94 eyes suffering from acute angle-closure glaucoma were assessed in a retrospective study. Sixty-eight eyes were treated with acetazolamide, timolol, and pilocarpine. The remaining 26 eyes were treated with acetazolamide and timolol only. The therapy without pilocarpine proved to be significantly more effective.

[Cryocoagulation of a prolapsed iris in hypertensive uveitis].

An ICCE was complicated by iris prolapse. Three years later the eye appeared quiet and intraocular pressure was normal when the patient was admitted for secondary implantation. An uneventful IOL implantation (CILCO MT 4U) was followed by hypertensive uveitis. Local and systemic corticosteroids and antihypertensive treatment were insufficient. Cryocoagulation of the bleb was performed. The inflammation rapidly subsided and intraocular pressure normalized without further therapy.

Aged

[A new iridocorneal endothelial syndrome].

Three iridocorneal endothelial syndromes are distinguished: essential progressive iris atrophy, iris nevus (Cogan-Reese) syndrome, and Chandler's syndrome. Obviously, there are patients in whom findings do not correspond fully to any one of the three syndromes; signs and symptoms which justify being considered as a new iridocorneal endothelial syndrome are described.

[An unusual cause of amblyopia ex anisometropia].

The case history is reported of an infant with a capillary haemangioma in the superior temporal quadrant of the anterior right orbit. Radiotherapy was not performed in view of the well-known tendency of these tumours to involute. Constant pressure on the globe led to marked oblique astigmatism of the eye with consequent amblyopia, in spite of complete regression of the lesion. The tendency of such tumours to regress must be weighed against possible irreversible functional impairment when a decision is made as regards treatment of neonatal haemangiomas in the region of the orbit.

Autologous hair implantation for drainage in rabbit eyes.

Autologous hair material was used as a wick for drainage from the anterior chamber into the subconjunctival space in 8 rabbits. Though pressure reduction and methylene blue leakage was evident in the operated eyes, the histological observations seemed rather discouraging.

[Argon laser trabeculoplasty in glaucoma with a narrow chamber angle].

In 49 eyes (33 patients) with open-angle glaucoma and narrow angle, which did not respond to conservative therapy, argon laser trabeculoplasty (ALT) was performed after preparative gonioplasty. The success rate was 65.3%. A postoperative rise in IOP was observed in 44.3%, and aqueous humor flare in 63.3%. Goniodysgenesis proved to be prognostically unfavorable for ALT.

Follow-Up Studies

[Gonioscopic changes following cyclocryocoagulation].

Gonioscopic changes after cyclocryocoagulation are reported in 4 patients with angle-closure glaucoma resistant to all forms of therapy. During cryoapplication frozen aqueous humor was observed in the chamber angle near the treated area. Widening of the angle followed in the same place. Possible causes of this change are reported.

[A simplified ultrasonographic measuring technic in combination with a computer program for calculating the power of intraocular lenses].

In the last few years intraocular lens implantations have gained in popularity. In biometry new electronic methods have been introduced. For measuring the axial length by ultrasound we use the Ocuscan DBR 400 ST unit in combination with the digital storage system and an immersion technique. For lens power calculation we have developed a new computer program for a Commodore CBM 4032 using R.D. Binkhorst's formulae. In a series of 50 Kelman S-Flex anterior chamber lenses we found a mean error of +0.250 D and a standard deviation of +/- 0.662 D. Sixty-four percent of predictions were within the +/- 0.5 D range, and in 88% of these patients we predicted within the +/- 1.0 D range. The maximum errors of prediction were -1.0 D and +/- 1.75 D. Using an immersion technique an accurate axial length measurement can be obtained.

Computers

Pigmentary dispersion syndrome. A biometric study.

A-Scan biometry was performed on both eyes of 5 patients suffering from unilateral pigmentary dispersion syndrome, partly with glaucoma. The anterior chamber was significantly deeper and the lens significantly flatter in the eyes with pigmentary dispersion syndrome.
