Parity measurements of nuclear levels using a free-electron-laser generated gamma-ray beam.

The quality and intensity of gamma rays at the High Intensity gamma-ray Source are shown to make nuclear resonance fluorescence studies possible at a new level of precision and efficiency. First experiments have been carried out using an intense (10(7) gamma/s) beam of 100% linearly polarized, nearly monoenergetic, gamma rays on the semimagic nucleus (138)Ba. Negative parity quantum numbers have been assigned to 18 dipole excitations of (138)Ba between 5.5 MeV and 6.5 MeV from azimuthal gamma-intensity asymmetries.

Antibodies to tissue transglutaminase: comparison of ELISA and immunoprecipitation assay in the presence and in the absence of calcium ions.

Endomysial antibodies are characteristic of coeliac disease and tissue transglutaminase (tTG) has been identified as a major component of the endomysial antigen. tTG autoantibodies were measured in sera from patients with coeliac disease using ELISA based on guinea pig tTG and immunoprecipitation assay (IPA) based on 35S-labelled human tTG produced in an in vitro transcription/translation system. In addition, the effect of calcium ions on the interaction between tTG autoantibodies in the two assays was studied. Under standard (i.e. Ca2+-free) conditions, 36/39 (92%) coeliac sera were positive for IgA tTG antibodies by ELISA and 34/39 (87%) sera were positive by IPA. Comparison of ELISA and IPA results showed three sera positive by ELISA but negative by IPA and one serum which was positive by IPA but negative by ELISA. Bland and Altman analysis of the correlation between the ELISA and IPA showed that the results for 37 out of 39 samples were in the agreement. The results by ELISA carried out without and with Ca2+ were in good agreement (r=0.99; n=39). IPA using Ca2+ containing buffer detected fewer samples compared to IPA using standard assay buffer however the results of the two assays also showed a good agreement (r=0.93; n=39). Our studies confirm that antibodies to tTG are good markers of coeliac disease and indicate that the autoantibody binding sites on tTG are formed in a way which is essentially independent of Ca2+.

Surgical wound infection surveillance: the importance of infections that develop after hospital discharge.

BACKGROUND: The aim of this study was to evaluate two methods of post-discharge surgical wound surveillance and to compare the incidence and outcomes of wound infections that develop prior to patients' discharge with those that develop after hospital discharge. METHODS: One thousand, three hundred and sixty inpatients who underwent major elective surgery in an 800-bed teaching hospital in western Sydney between February 1996 and July 1997 were followed prospectively. Pre-discharge wound surveillance was performed by clinical assessment by an independent researcher on the fifth (or later) postoperative day. Post-discharge wound surveillance was performed by a mail out of questionnaires completed independently by patients and surgeons. RESULTS: Overall, 138 wound infections were diagnosed (incidence 10.1%), of which fewer than one-third (n = 44) were diagnosed before discharge (average 10.4 days postoperatively) and the remainder (n = 94) after discharge (average 20.6 days postoperatively). Seven hundred and eighty-two (57.5%) post-discharge survey forms were returned by patients and 680 (50.0%) by surgeons. When forms were returned by both surgeons and patients for the same wound (641 cases), there was substantial agreement in diagnosing infection or no infection (kappa = 0.73). CONCLUSIONS: The majority of nosocomial surgical wound infections develop after the patients' discharge from hospital. A post-discharge surveillance programme including self-reporting of infections by patients and return of questionnaires by patients and surgeons is feasible in an Australian hospital setting. However, such a programme is labour and resource intensive and strategies to increase return of questionnaires are required.

Histamine involvement in UVB- and cis-urocanic acid-induced systemic suppression of contact hypersensitivity responses.

Studies in experimental models have implicated histamine and prostanoids in ultra-violet B (UVB)- and cis-urocanic acid (UCA)-induced systemic immunosuppression. This study examined the hypothesis that UVB irradiation and cis-UCA suppressed contact hypersensitivity responses to hapten by induction of histamine, which in turn evoked a prostanoid-dependent component of immunosuppression. BALB/c mice were administered with a cis-UCA monoclonal antibody, a combination of histamine types 1 and 2 receptor antagonists, or indomethacin. Mice were sensitized to 2,4,6-trinitrochlorobenzene (TNCB) on their ventral surface 5 days after UVB irradiation, or cis-UCA or histamine administration. Ears were challenged with TNCB 5 days later. Cis-UCA antibody inhibited the suppressive effects of UVB by approximately 60% and confirmed that suppression of contact hypersensitivity responses by UVB was due, at least in part, to mechanisms involving cis-UCA. Histamine suppressed contact hypersensitivity responses and the effects of cis-UCA and histamine were not cumulative, suggesting that cis-UCA and histamine signal largely through the same pathway. The immunosuppressive effects of histamine were not affected by the cis-UCA antibody, consistent with the model that histamine acts downstream of cis-UCA. Administration of histamine receptor antagonists and indomethacin each approximately halved the UVB- and cis-UCA-induced systemic suppression of contact hypersensitivity responses. The effects of the reagents that inhibited the action of histamine and prevented prostanoid production were not cumulative, and suggested involvement in the same pathway. These results support the involvement of cis-UCA, histamine and prostanoids, in a sequence, in UVB-induced systemic suppression of contact hypersensitivity responses.

Connections: a contextual model for holistic nursing practice.

Nursing is a holistic practice in which the science and technology of curing is cradled by the art of human caring within a healing context. Caring is the human capacity to connect with another from a place of compassion and unconditional support in the process of growth and actualizing one's potential in the face of human challenge. Healing reflects the process of one's search for wholeness in the ultimate expression of one's unique being. This wisdom is multidisciplinary and interconnecting, integrating the multidimensional aspects of the human process engaged in the art and science of healing.

Biodegradability of polymers in the environment: complexities and significance of definitions and measurements.

There is a world-wide research effort to develop biodegradable polymers as a waste-management option for polymers in the environment. This effort may prove to be fruitless unless we can agree on a definition and test protocols--what do we expect biodegradable polymers to do in the environment and how do we demonstrate that they do what we expect? Establishing a definition and test protocols is not trivial; the task is made complex by the wide range of disciplines involved directly in or interested in the subject, including polymer scientists, biochemists, environmentalists, legislators, and laypeople, all with their own perspectives and expectations. In this paper, I present arguments in favor of biodegradable polymers and indicate what remains to be done to satisfy detractors that they represent a viable option for polymer waste-management.

A scale for estimating the health status of opioid users.

The health status of injecting drug users has both clinical and research relevance. In order to evaluate the impact of treatments for opiate dependence on drug users' health status we need valid and reliable instruments which are short and easily administered by non-medical personnel. This paper describes the construction of a brief interviewer administered scale for assessing the current health status of injecting drug users. Data evaluating its reliability and validity are presented which indicate that the scale has excellent psychometric properties.

Salivary and serum antibodies to gliadin in the diagnosis of celiac disease.

Salivary IgA and serum IgA and IgG antibodies to crude gliadin and alpha-gliadin were measured in adult patients with untreated celiac disease, ulcerative colitis, Crohn's disease and normal controls. Antibody levels in saliva and serum in untreated celiac disease were significantly higher than in other groups. Alpha-gliadin antibodies appeared to be a better discriminator for serum but not for saliva. The assay of salivary IgA antibodies to gliadin offers a non-invasive test which would be particularly useful in the investigation of celiac disease and for monitoring compliance with a gluten-free diet.

DNA sequence of a plasmid-encoded dihydrofolate reductase.

The sequence of the methotrexate-resistant dihydrofolate reductase (DHFR) gene borne by the plasmid R-388 was determined. The gene was subcloned and mapped by an in vitro mutagenesis method involving insertion of synthetic oligonucleotide decamers encoding the BamHI recognition site. Sites of insertion that destroyed the methotrexate resistance fell in two regions separated by 300 bp within a 1.2 kb fragment. One of these regions encodes a 78 amino acid polypeptide homologous to another drug-resistant DHFR. The second region essential for DHFR expression appears to be the promoter of the DHFR gene.

The history of vocational training for general practice.

We feel that it is worth recording the story of how vocational training for general practice was first proposed, how the earliest experiments were started, and how this prolonged exercise in teamwork developed throughout the UK. It is appropriate to do this now that Parliament has recently voted that this training should be an obligation for all doctors who wish to work as general practitioner principals in the NHS. Moreover, the first person to exert a crucial influence, Henry Cohen (Lord Cohen of Birkenhead) has recently died.We write this account in historical sequence, not comprehensively, but selecting what in retrospect we believe to have been of chief importance. We describe initiatives at first unrelated to each other, but all contributing to one end. As the story unfolds, so does coordination in the common effort. The account ends at 1970.
