[Stress factors in myocardial infarct patients before the appearance of myocardial infarct].

Apart from somatic risk factors and the smoking of cigarettes more attention in the development of the myocardial infarction must be ascribed to the psychosocial and psychoemotional stress and its false processing. For the judgment of the stress in patients with myocardial infarction a questionnaire was constructed. It contains those questions the answer of which showed significant differences in comparison to the persons with healthy coronary vessels. The inquiries were answered by altogether 110 patients with myocardial infarction. In order to obtain references to definite characteristics concerning the structure of the personality of our patients with myocardial infarction, we used the INR-questionnaire after Böttcher. In either sex moments of "self-stressing" dominate. Professional conflicts were mentioned more by men, familial problems more by women. Among others, definite characteristics concerning the structure of the personality (rigidity) of patients with myocardial infarction seem to be a reason for the deficient strain under stress or they increase the sensitiveness to stress. From the results of the examinations requirements to the psychic rehabilitation of patients with infarction are derived, which must be carried out parallel to the somatic rehabilitation.

Diurnal changes in sensitivity to melatonin in intact and pinealectomized Djungarian hamsters: effects on thermogenesis, cold tolerance, and gonads.

Djungarian hamsters kept in long photoperiod (16:8 L:D) were injected daily at 0800, 1200, or 1600 with 25 micrograms of melatonin. During 90 days of treatment, body weight and fur coloration were checked at weekly intervals, and at the end of the treatment the reproductive status of the hamsters and their thermoregulatory properties (could limit, maximum thermoregulatory heat production, nonshivering thermogenesis, cytochrome oxidase activity in brown adipose tissue) were measured. Hamsters injected at 1600 changed from summer to winter status with regard to all functions investigated responding simultaneously; i.e., their body weights decreased, their fur became white, their gonads regressed, and their thermoregulatory properties improved. All these changes were identical to the effects of short photoperiod (8:16 L:D) exposure. Injections of melatonin at 0800 and 1200 were ineffective for reproductive functions, but the injection of melatonin at 0800 caused slight improvements of thermogenesis. The response to melatonin injected at 1600 could be suppressed by an additional injection of melatonin at 0800 (75 micrograms). Pinealectomized or ganglionectomized hamsters kept in long photoperiod did not respond to daily injections of melatonin at 1600 for the first 60 days of treatment, but during a prolonged treatment their sensitivity to melatonin was restored. Similarly, pinealectomized or ganglionectomized hamsters failed to respond to short photoperiod for about 40 days, but during prolonged exposure their sensitivity to short photoperiod was restored.

[Effective physical method for the prevention of deep venous thrombosis of the leg].

A newly developed system for the physical prophylaxis of thromboses of deep veins of the leg is presented and its effect on various parameters is described. The invasive examinations were carried out in the blood of the femoral vein and artery on 48 patients (chronic cor pulmonale, varicosis, healthy persons) before and after a 24-hour external rhythmical (dynamic) compression of the feet and lower legs. In 9 out of 19 coagulation and blood gas analytical as well as haemodynamic parameters investigated ascertained changes with antithrombotic function could be proved. The effect depended upon the size and dynamics of the compression pressure as well as upon the size of the storage blood volume. An influence on the arterial circulatory crus was not observed. The present results confirm the up to now reported clinical success in the application of similarly effecting dynamic compression methods within the prophylaxis of thrombosis.

The influence of different light irradiances on pineal N-acetyltransferase activity and melatonin levels in the cotton rat, Sigmodon hispidus.

Wild-captured cotton rats (Sigmodon hispidus) trapped and tested in September and October exhibited a rapid reduction in pineal N-acetyltransferase (NAT) activity and melatonin levels after exposure to a light irradiance of 300 microW/cm2 during the dark period. The half-time for the depression of both NAT and melatonin was on the order of 2 min. The exposure of cotton rats during darkness to much lower irradiances of light, i.e., 5.0, 0.04, 0.03 or 0.01 W/cm2, for 32 min also greatly diminished pineal NAT activity and radioimmunoassayable melatonin levels; however, a light irradiance of 0.005 microW/cm2 failed to significantly depress either the acetylating enzyme or the melatonin content of the pineal gland. The results show that the pineal gland of the wild-captured cotton rat, as judged by NAT activity and melatonin levels, is inhibited even by very low irradiances of light.

[The value of peripheral venous pressure measurements in the lower extremities under static and dynamic conditions].

With the help of 3 groups of persons (normal persons, carriers of varices, patients with deep phlebemphraxis) peripheral measurements of the venous pressure in a quiet standing (statical part) as well as before, during and after strong movements of the feet up and down (dynamical part) were performed. It was found that statical measurements do not allow a diagnostical evidence, and thus they are unsuitable for practice. However, measurements off the venous pressure under dynamical conditions in normal persons and carriers of varices show gradually different pressure-relieving changes in the veins of the feet, which nearly stay away when a deep phlebemphraxis of the legs is present. High frequencies of motion cause the clearest reliefs of the venous pressure in the feet, though also slow kinds of motion are still active. Sex-specific signs were not found.

[Modification of stimulus threshold increase following heart pacemaker implantation by prednisolone].

In a retrospective study concerning 39 patients with pacemakers is reported on the possibility of a medicamentous influence of so-called increases of the stimulus threshold with exit block. Inflammatory tissue reactions and disturbances of electrolytes as well as microdislocations at the tip of the electrode are given as a cause of the postoperatively appearing deficiency of the stimulation. A differentiation of these causes is not possible with simple paraclinical examination methods. Generally using prednisolone we succeeded in decreasing the stimulus threshold in about half the patients within ca. 3 weeks and thus in removing the exit block. After discussion of the pathophysiological connections recommendations for the therapeutic approach are described.

[The value of clinical and paraclinical parameters in the diagnosis of acute abdominal diseases as based on the patient material of a medical clinic].

The triad pains, tension of the abdominal wall and disturbances of the peristalsis, cited in the special literature, had no diagnostic significance for the recognition of acute abdominal diseases. In more than half of the patients examined we found leucocytosis and tachycardia as individual parameters. In about one third the simultaneous presence of several symptoms (leukocytosis, tachycardia and increase of temperature) was observed. Among the parameters got in our patients none was referring to making the diagnosis of an acute abdominal disease in the individual case. Even when the adequate symptomatology is absent in old and frail patients should be thought of an acute abdominal disease, the more so when most modern anesthetic and operative methods and an intensive postoperative care is used the prognosis of such diseases may be favourable also at older age.

[Gastrointestinal hemorrhage in the internal medicine cases of a district hospital. Retrospective study from the years 1965-1974].

In the period between 1965-1974 564 patients with gastrointestinal hemorrhages were admitted in the medical clinic of the District Hospital St. Georg Leipzig. Of them 112 patients (20%) were at once or after failure of a conservative therapy admitted in the surgical clinic for operation. 82 patients died (18%) of the conservatively treated patients). The most frequent sources of haemorrhage duodenal ulcers with 32%, ventricular ulcers with 25% and carcinomas of the stomach with 19%. In 13% of the patients haemorrhages induced by drugs could be assumed. In 13% the case were recidivation haemorrhages. Severe gastrointestinal haemorrhages were observed only in 4% of the patients and consequently were less important in our patients. A further improvement of the diagnostic exactness is possible by perfection of the early diagnosis (emergency gastroscopy and angiography), which may be important with regard to a surgical intervention.

[Objectivization of the protamine sulfate test through photometric measurements].

The demonstration of monomeric complexes of fibrin in the blood is regarded as a secure sign of an increased consumption of coagulation factors in the sense of a consumption coagulopathy. As a simple and sensitive method for the demonstration of these intermediate products of the coagulation the protamine sulphate test proved its worth in clinical practice. The subjective judgment of this test-tube test frequently becomes difficult in the borderline region between normal and pathologic. On the basis of own comparative investigations it could, however, be demonstrated that by means of photometric measurements an objective and exact evidence about the result of the protamine sulphate test can be made. It is referred to the advantages in the judgment of the course of a consumption coagulopathy during the treatment.
