Feedforward and feedback processes in motor control.

In this study, we utilized functional magnetic resonance imaging (fMRI) to examine which brain regions contribute to feedback and feedforward motor control processes. Several studies have investigated the contributions of cortical and subcortical brain regions to motor performance by independently varying factors such as movement rate, force, and speed, and observing the neural responses. Such studies have contributed greatly to our understanding of neural coding of movement variables. Under natural movement conditions, however, these factors interact in a complex manner to produce differing performance levels. In the current investigation, we induced performance changes in a less constrained way, by having subjects move a joystick to hit targets of differing sizes on an LCD screen. These parametric changes in target size resulted in the well-known speed-accuracy tradeoff effect, allowing us to examine the brain regions responsive to global shifts in motor performance levels. That is, movements made to larger targets relied more on feedforward control whereas movements made to smaller targets relied more on feedback control. Using functional MRI, we identified two sets of brain regions in which activation was modulated with task difficulty. Areas exhibiting activation that was positively correlated with increasing target size included primary motor cortex, premotor cortex, and the basal ganglia, regions that are typically classified as playing a role in force control and movement planning. Brain regions whose activation was negatively correlated with increasing target size included the ipsilateral sensorimotor cortex, multiple cerebellar regions, and the thalamus. These areas contributed to motor performance under higher levels of task difficulty. The results elucidate cortical and subcortical brain regions that are responsive to global shifts in motor performance, reflecting changes along the continuum of feedforward and feedback motor control.

[Institute for Hygiene and Epidemiology: tasks and assignments 1996. Role in present-day federal Belgian future perspectives].

In most European countries there are research institutes with responsibilities similar to those of the I.H.E. in Belgium. In English they are generally called "National Institute of Public Health". However, these Institutes are quite different from one country to another, because they are the result of general political factors as well as of the specific epidemiological context of each country. The evolution of the I.H.E., which for decades had research activities which are now the responsibility of three different political levels (Federal/Regions/Communities), has been strongly influenced by the profound institutional reforms which took place in Belgium since 1970. The accompanying difficulties and changes are described. More details are given concerning the specific tasks and responsibilities of the institute, its structure, perspectives for its future and the planned merging with the Pasteur Institute.

Sero epidemiological characteristics of hepatitis C encountered in general practice in Belgium.

A study carried out between 1982 and 1984 established by exclusion diagnosis that 35% of viral hepatitis cases registered in Belgium were due to non A, non B (NANB) viruses. Recently, a new anti-hepatitis C virus (HCV) detection test was used to analyse the sera of patients in whom NANB hepatitis was diagnosed in that study. Using this new serological test for HCV, 29% of the NANB group was found to be positive for anti-HCV. In the 1982-84 study on viral hepatitis diagnosed by general practitioners, the number of clinically recognized infections was estimated at 14,700 (+/- 2,170; confidence interval at 95%) per year. By combining these data and the results of the present study, the following estimates could be calculated: HAV (7,129 +/- 1,054/year), HBV (2,426 +/- 358/year), HCV (1,470 +/- 216/year) and non-identified hepatitis viruses (3,675 +/- 543/year).

Investigation of the geographical distribution of female cancer patterns in Belgium using pattern recognition techniques.

Different multivariate methods have been applied to obtain an overview of the Belgian female cancer mortality distribution. The resulting maps and figures show the patterns of female cancer mortality to be strongly geographically determined. Two major trends can be derived namely along the north-south axis and along the east-west axis of the country. The cancer sites responsible for these patterns are identified.

[Various sero-epidemiological characteristics of viral hepatitis in general medicine: results of a pilot survey performed in Belgium].

A study was made of 110 cases of viral hepatitis diagnosed by general practitioners between 31 May 1982 and 30 June 1984. Hepatitis A was diagnosed in 53 patients, hepatitis B in 18 patients, hepatitis of both types A and B in one patient, and--by exclusion--hepatitis non-A non-B in 38 patients. Hepatitis A appears to be generally acquired by contact with infected persons or by travelling in endemic regions. In more than 50% of the hepatitis B cases, a hospital stay and/or ambulatory treatment were mentioned. All patients with hepatitis A became negative for anti-HA/IgM within 9 months after the diagnosis. 30% of the patients with hepatitis B remained positive for HBs Ag 9 months after diagnosis.

Yersinia enterocolitica infections and pork: the missing link.

To determine the risk factors for Yersinia enterocolitica (YE) infection in Belgium, which is the country with the highest incidence of this infection, 40 persons with YE infections due to serotype O:3 (n = 36) or O:9 (n = 4) were compared with matched controls. Most patients had acute gastroenteritis; 1 had a liver abscess. 20% were admitted to hospital; the mean duration of hospital stay was 9 days. YE infection was strongly associated with eating raw pork in the 2 weeks before onset (odds ratio = 12, p = 0.00002), a factor that accounted for 58% of the YE infections studied. 14 (18%) of 79 families surveyed at a well-baby clinic fed their children raw pork from a median age of 18 months. Specific control measures to reduce contamination and consumption of raw pork may reduce the incidence of YE infections.

[Eye and skin manifestations of endogenous oxalosis of the protracted type (follow-up)].

The present paper reports the authors' observations in a rare case of primary hereditary oxalosis (adult type). Four years after renal function loss, calcium oxylate deposits were found in the skin and in both retinae. In spite of hemodialysis and a special diet, the deposits increased considerably during an observation period of 19 months. The skin was indurated and the genucubital articulations were immobilized. In spite of incipient circumscribed edema the retinal changes did not cause any detectable restriction of function, as the fovea was not involved and the retinal border was only slightly affected.

[Therapy of psoriatic osteoarthropathy with EHDP].

After describing the problems involved in the therapy of psoriatic osteoarthropathy, we discuss the specific accumulation of EHDP at the site of the disease. Because of its pharmacological properties, EHDP may have a regulatory effect on the pathological changes in psoriatic osteoarthropathy. In an open experiment using 20 patients with psoriatic osteoarthropathy, we evaluated the influence of EHDP on the extent and seriousness of bone and joint disease. We carefully studied the case history, laboratory tests, and rheumatological status; in some cases, bone scintigrams were also carried out. The patients received an oral dosage of about 20 mg/kg body weight the first 2 months and about 5 mg/kg from month 3-6. Two-thirds of the patients (n = 12) indicated a decrease in joint and bone pain, and there was also detectable improvement in the bone scintigrams. Only one patient noticed a side effect from the therapy. He complained of severe itching.

[Significance of gallium scintigraphy in tumors and granulomatosis of the skin].

Scintigraphy with gallium has proved useful as a screening method for the diagnosis of certain malignant tumors and systemic diseases. One single scintigraphic investigation is able to detect almost all organs of a man. We checked this radiopharmacon with regard to its usefulness in dermatology. Gallium scintigraphy shows high sensitivity concerning verification and localization of metastases of malignant melanome as well as B- and T-cell lymphomas. Moreover, gallium is effective in controlling the therapie of sarcoidosis.

[Continuous surveillance of morbidity by a network of watchful general practitioners].

This paper describes a system of morbidity surveillance implemented in Belgium in 1978, which routinely collects data provided by a sample of general practitioners. Advantages and drawbacks of this system are discussed. Some results are presented, regarding the incidence and prevalence of selected health problems, as for example urinary tract infections and abortion requests.

Validity of the recording of meningococcal disease according to various sources of information.

A study was made in Belgium in order to assess the completeness and specificity of the recording of meningococcal disease by routine sources of information. Ninety-three cases identified in a hospital survey were linked with those recorded in mortality statistics, in the notification of communicable diseases, and by the National Reference Laboratory for meningococci. Statistics based on mortality data appeared to be of low validity. The overall completeness of recording was 44% for the notification of communicable disease, and 40% for the reference laboratory. When these two sources were used for surveillance, the completeness of case-finding increased to 56%. When the analysis was restricted to bacteriologically-confirmed cases, the completeness of recording was 62% for the notification system, 70% for the laboratory, and 84% for both sources. The surveillance of communicable diseases should rely on various sources of information. Laboratory data should be systematically used in order to improve both the completeness of recording and the specificity of case-ascertainment.

[The psoriatic bone manifestation].

Besides the known joint involvement as seen in the whole-body scan which uses 99m technetium-phosphate-complexes, skeletal scintigraphic investigations in 17 patients with psoriasis have demonstrated circumscribed or diffuse accumulative pathological concentrations of the radionuclide within the skeleton of the skull, the thyroid cartilage, the bony thorax and osseous long bones. These pathological accumulations of radionuclides of TcP-complexes which are distant from joint regions, point to proliferative phenomena in the bone in case of psoriasis especially in alterations of the collagenous metabolism.
