Periodontal conditions of adult regular dental care attendants. A 12-year longitudinal study.

The purpose of the present study was to examine longitudinal alterations in the periodontal conditions of regular dental care attendants. 225 randomly selected patients (age 18-65 years) at 12 community dental clinics in the county of Värmland, Sweden, were subjected to a baseline clinical and radiographic examination in 1978 and to a re-examination in 1990. During the study period, all participants received preventive and therapeutic measures according to decisions made by the dentist on duty in the clinics. The examinations involved assessments of number of remaining teeth, plaque, gingivitis, probing pocket depth, loss of probing attachment and periodontal bone height. The results showed that during the 12 years of monitoring, an average of 0.4 teeth were lost. The % of tooth sites with gingivitis was lower in 1990 (4%) than in 1978 (15%), but no major changes were found for the mean probing pocket depth. The mean probing attachment loss during the 12 years amounted to 0.5 mm. The tooth site analysis revealed that buccal sites had experienced more loss of attachment than lingual and approximal surfaces. Whereas no differences were observed between age groups with respect to longitudinal loss of attachment at lingual and approximal tooth sites, the youngest age group demonstrated more pronounced loss at buccal surfaces than older subjects. The radiographic assessments of the alveolar bone height revealed a mean longitudinal loss amounting to 0.2-0.4 mm in the various age groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Integrated caries prevention: effect of a needs-related preventive program on dental caries in children. County of Värmland, Sweden: results after 12 years.

Twenty years ago the caries prevalence in Swedish children was among the highest in the world; within Sweden, it was highest in the County of Värmland. Ongoing clinical research projects were initiated to evaluate the separate and combined effects of preventive measures, and in 1978 a preventive program, based on caries risk assessment, was introduced for 3- to 19-year-olds in the county. The effects were monitored by a computerized epidemiological system. From 1979 to 1991, caries prevalence and incidence decreased by 75-90 and 75-85%, respectively. The percentage of caries-free 3-year-old children increased from 51 to 94%, and in 12-year-old children, the caries prevalence decreased from 6.5 to 1.0 DFS, the lowest in Sweden. The program is very cost-effective, and in 1990 the mean annual treatment time per child was the lowest in Sweden. Currently, new methods of caries risk prediction and integrated caries prevention are being developed, with special reference to cost-effectiveness.

Effect of proper oral hygiene on gingivitis and dental caries in Swedish schoolchildren.

The present paper reports the effect of a 3-year study of regularly repeated professional toothcleaning on oral hygiene status, gingivitis, and dental caries. At the initiation of the study in 1971, the children were 7-8 years old (Group 1), 10-11 (Group 2) or 13-14 (Group 3). The children were divided into test and control groups. Before the study all the children were examined regarding oral hygiene, state of the gingiva and dental caries. Throughout the 3-year period the controls brushed their teeth with a 0.2% sodium fluoride solution once a month under supervision. During the first 2 years of the trial all the children of the test groups were given professional oral prophylaxis once every second week during the school terms. During the third year the interval between consecutive prophylactic sessions was prolonged to 4 weeks in Groups 1 and 2 and to 8 weeks in Group 3. The results demonstrated that regularly repeated professional cleaning of the teeth combined with fluoride applications and toothbrushing instructions, over a 3-year period in schoolchildren resulted in the establishment and maintenance of excellent oral hygiene standards. Inflammation of the marginal gingiva almost entirely disappeared, and practically no new carious lesions developed.
