[A non-invasive method of evaluation of Helicobacter pylori urease activity: its perfection and introduction to clinical practice].

The authors have developed a highly sensitive method of non-invasive diagnostics of Helicobacter pylori, the most frequent human infection. Detection of urease activity is based upon measurement of the degree of the elevation of stable 13C isotope content in exhaled air after administration of C-urea as a test reagent. The method has been scarcely applied in Russia because the test preparation, 13C urea had not been produced domestically until 2002. The method can be easily applied by any healthcare institution; however, it requires special equipment and trained personnel to perform measurement of 13C content in exhaled air samples using mass spectrometers. The article presents the first experience in clinical application of the method.

[Comparative effectiveness of different methods of diagnosis and correction of impaired portal circulation].

The diagnostic effectiveness of X-ray radionuclide and ultrasound determination of portal blood flow and the impact of different treatments on the blood flow were evaluated in 76 patients with hepatic cirrhosis. The treatment regimens were used propranolol (Group 1), isosorbide-5-mononitrate (monosan) (Group 2), and a combined therapy with propranolol in combination with diuretics and a course of intravenous laser irradiation (Group 3). In terms of both clinical and laboratory parameters and the status of portal circulation, the highest effect was observed in the patients receiving the combined therapy.

[The breath test is a necessary step in a physician's practice].

The authors advocate introduction of the respiratory test into clinical practice, show its advantages over other methods of Hp diagnosis and the ability to diagnose non-invasively other gastrointestinal diseases especially in advanced infection primarily in viral hepatitides, HIV infections. The method is safe and comfortable.

[Effectiveness of lamivudine in the treatment of chronic HBeAg-negative viral hepatitis B: results of continuous therapy for 18 months].

The efficacy of monotherapy with one of the analogues of synthetic nucleosides (lamivudine) was studied in 22 patients with chronic viral hepatitis B (CVHB). The specific feature of the selected group was that there was no serum HBeAg, which is usually regarded as a sign of viral mutation. The time course of changes in the activity of blood and hepatic inflammatory processes and viral replication were studied at equal intervals. There was a direct correlation between the suppression of viral replication and the regression of activity indices, such as ALT, serological markers, DNA of viral hepatitis B, and the morphological constituents of the Knodell index) during lamivudine therapy. However, there were no substantial changes in the parameters of fibrosis throughout the treatment. Therapy resistance was revealed in 3 of the 22 patients with CVHV. Its basis was the mutation of hepatitis B virus in the YMDD-motive, which was detected in 2 of the 3 patients.

[Mental disorders in patients with chronic hepatitis].

51 patients with chronic hepatitis were examined clinicopsychopathologically for depression, anxiety, self-estimation, cognitive defects. Non-psychotic mental disorders were found in all the examinees: 15 had asthenia, 36 had asthenia and affective disturbances (astheno-depressive, anxious-depressive, anxious-hypochondriac, hystero-depressive). A correlation exists between psychic disorders, severity of chronic hepatitis, premorbid personality traits and efficacy of therapy. Psychiatrist's observation is needed for early detection and treatment of the above disorders.

[Correlations of the indicators of biochemical composition and physico-chemical properties of the bile in chronic cholecystitis].

The study of the factors relevant to the bile colloid stability in aggravation of calculous or ++non-calculous cholecystitis demonstrated changes in aggregative-kinetic stability of bile presenting with low boundary of the phase transition, electrokinetic potential, mass and growing size of macromolecular compounds. This is much coupled with high bile levels of protein, calcium and iron which exhibited mutually potentiating effect. At the same time, hydrolysis of protein-containing bile components +determined by the increment of amine and sulfhydryl groups evidences for colloid destabilizing active role of calcium and iron. In view of this finding elevated concentration of iron and calcium in exacerbations of cholecystitis may have both diagnostic significance and be indicator of the role of inflammation in lithogenesis.

[Proliferative activity of the surface foveolar gastric epithelium in acetysalicylic acid injury].

The proliferative activity of gastric mucous cells (GMC) was studied in white mice following the administration of acetylsalicylic acid (200 mg/kg). One hour before sacrifice all the animals received intraperitoneal injection of 3H-thymidine. The mitotic index and index of labeling nuclei were calculated by means of radioautographers 3, 10 and 20 days after administering the drug. Following 3-day exposure to acetylsalicylic acid the proliferative activity of GMC remained unchanged as compared to controls. Long-term administration (10 and 20 days) of the drug produced no increase in the number of erosions, but there was a statistically significant rise in the proliferative activity of GMC. This rise was accounted for by increased number of proliferating cells in the foveated and cervical divisions of the glands as well as due to the extension of the zone of distribution of precursor cells in the gastric glands as far as the basal divisions. These changes may be considered as manifestation of the protective-adaptive reaction of the gastric mucosa in response to the damaging effect of acetylsalicylic acid.
