Hemorheological and oxygen free radical associated alterations during and after percutaneous transluminal coronary angioplasty.

Percutaneous transluminal coronary angioplasty (PTCA) is a frequently used method in the treatment of coronary artery disease. Coronary stenosis, endothelial injury, and ischemia-reperfusion caused by the balloon inflation and deflation during this procedure can cause several changes in blood flow. In our study 19 patients (mean age: 58 +/- 9 years) undergoing PTCA were examined. For the laboratory measurements several blood samples were taken from the femoral vein and the coronary sinus before and 30 minutes after PTCA, and from the cubital vein 1, 2, 5 days and 1, 6 months after PTCA. Among hemorheologic parameters hematocrit, plasma fibrinogen level, plasma and whole blood viscosities were measured and corrected blood viscosity value was calculated. To characterize the oxidative stress, samples were analyzed for thiobarbituric acid reactive substances (TBARS) of blood as a marker of lipidperoxidation and changes in the antioxidant system were investigated by measuring the activity of superoxide dismutase, catalase and the concentration of glutathione; superoxide generating capacity of isolated leukocytes and platelet aggregation were examined as markers of cellular activation. Plasma fibrinogen concentration increased markedly during the first and second day after PTCA (p < 0.001), which was accompanied by the elevation of plasma viscosity (p < 0.05). Plasma fibrinogen returned to the baseline at the one-month check-up visit, but there was a significant increase in its concentration by the end of the sixth month follow-up. Apparent whole blood viscosity at 90 s (-1) showed gradually increasing values up to the one- and six-month check-up visits (p < 0.01), which can partially be explained by the elevation of hematocrit. Corrected blood viscosity was significantly elevated on the fifth day already (p < 0.01), and one month later also. Superoxide production of leukocytes showed an increasing tendency (p = 0.05), and blood TBARS was elevated after one day (p < 0.05) and remained higher during the following days. Catalase activity showed significantly increasing values (p < 0.01) during the hospital phase, then at the end of the first month. SOD activity and spontaneous platelet aggregation were higher in the samples from the coronary sinus than in those from the peripheral vein before the procedure; 30 minutes after PTCA increased levels in the peripheral sample were found (p < 0.01). Our findings indicate that PTCA may cause significant changes in the hemorheologic and free radical associated parameters, which can affect the final outcome of this intervention.

[Hemorheologic parameters in coronary artery disease].

Hemorheological parameters were proved to be primary risk factors in ischemic heart disease (IHD) by several studies. In the present study the relation of these parameters to the severity of coronary artery disease (CAD) was examined. The data of 109 patients (mean age: 55 +/- 9 yrs) undergoing coronary angiography and 59 healthy controls (mean age: 35 +/- 10 yrs) were analyzed. Hemorheological parameters [hematocrit, fibrinogen level, plasma viscosity (PV) and whole blood viscosity (WBV)] were measured and circulatory index (CRI) was calculated. Patients were classified into three groups according to their coronary vessel state based on the coronary angiogram: Group 1 (n = 19, mean age: 53 +/- 8 yrs) without significant CAD, Group 2 (n = 19, mean age: 51 +/- 11 yrs) with single vessel disease, Group 3 (n = 71, mean age: 57 +/- 8 yrs) with multivessel disease. All the measured hemorheological parameters of IHD patients were significantly higher than those of controls. Fibrinogen and PV was significantly elevated in Group 2 and 3 comparing to Group 1 (p < 0.05 and 0.01). Hematocrit and WBV was significantly increased in Group 3 comparing to Group 1 and 2 (p < 0.05). CRI was significantly decreased in IHD patients, and it was also lower in Group 3 than in Group 2 (p < 0.05). These results indicate that hemorheological parameters may play a role in the pathogenesis and development of CAD.

[Possibilities of the use of 3-dimensional ultrasonography in gynecology and obstetrics].

The authors report their experiences with 3 dimensional ultrasound technique applied in cases of 286 obstetrical and 35 gynecological examinations. Combison 530, 3 D ultrasound equipment was used. Pathological signs were found in 14 cases. The introduction of this technique has been increasing the diagnostic accuracy of prenatal and gynecological ultrasound examinations, and some malformations, that could have never been seen by "traditional" 2 D methods, can be visualized as well.

[Intrauterine ultrasonic diagnosis of ectopia cordis associated with exencephaly].

The heart positioned outside the chest is called ectopic heart. The forms of ectopia cordis is generally a particular sign of a multiplex malformation and/or genetic aberration. According to the types of this malformation the authors described the sonographic signs, incidence and consequences of the condition on the base of a clinical case.

[Fetal blood flow in the renal artery during the third pregnancy trimester].

Blood flow velocity waveforms of the renal artery were examined in 112 cases between the 28th and 40th weeks of pregnancy. The measurements were performed with Hitachi EUB 450 pulsed Doppler ultrasound system. For evaluation of the blood flow velocity waveforms of the renal artery resistence index was calculated. If the other examined vessels (umbilical artery, medial cerebral artery), and the following CT examination pathological sign were not found, the renal artery blood flow was considered as a normal value. The results showed that in normal cases the renal artery resistence index decreased moderately during the third trimester of pregnancy. This decreasing tendency might caused by the increased blood flow of the renal circulation.

Eradication of bovine leukosis from a heavily infected herd by the use of own offspring.

In a Holstein-Friesian dairy herd (n = 1,248) kept in loose housing system and showing 62% prevalence of infection with bovine leukosis virus (BLV), the newborn calves were separated from their dams and placed into individual pens. They were given freeze-stored colostrum derived from a BLV-free farm and then a calf starter diet. Strict hygienic measures of leukosis control were observed. Serum samples were taken from the calves and tested by agar gel immunodiffusion (AGID) before and after colostrum uptake, at 2.5 and 6 months of age, and then at the heifer-rearing farm at 2-month intervals. Animals giving a positive reaction were culled. No AGID positivity occurred before the uptake of colostrum. Subsequently, six times as many calves became positive among the offspring of infected cows than among calves born to negative animals. As no separate barn was available, the infected farm was divided into a "black" and a "white" part with a foil to prevent aerogenic infection. The negative heifers being in the 7th month of pregnancy were placed into the "white" part. A separate calving house was established for them, as the joint calving house proved to be a source of infection. By this method a leukosis-free progeny herd was obtained from 90% of the heifer calves of the cow population showing a high prevalence of infection. In this way the original infected herd was gradually replaced.

Serological survey for type D retroviruses and simian immunodeficiency virus in monkeys living in Hungary.

Acquired immunodeficiency in non-human primates has been associated with two types of retroviruses: (i) Simian Retrovirus (SRV), a type D virus and (ii) a lentivirus, Simian Immunodeficiency Virus (SIV). Both types represent a serious health threat to animals living in different primate research centres, zoos, medical institutions and in private homes. The sera of 175 monkeys belonging to different species were tested for the presence of antibodies against type D retroviruses and SIV. The Mason-Pfizer monkey virus, the prototype of D retroviruses, and Simian Immunodeficiency Virus, designated SIVmac251, were used. None of the sera tested proved to be positive for the presence of type D retroviruses and only one serum of an African green monkey was positive for the presence of antibodies to SIV.

Establishment of an MT4 cell line persistently producing infective HIV-1 particles.

Human T cell Lymphotropic Virus-I (HTLV-I) carrying human T cell line MT4 is highly sensitive to Human Immunodeficiency Virus-1 (HIV-1). After HIV-1 infection cell clusters characteristic of intact MT4 rapidly disintegrate, syncytia appear and the cells die. Surviving MT4 cells were subcultured following HIV-1 infection of high multiplicity. We succeeded to establish an MT4 cell line continuously producing infective HIV (MT4/HIV-1). The original and the HIV-1 infected MT4 cells were morphologically similar. The MT4/HIV-1 cells proved to be nearly 100% positive in indirect immunofluorescence assay using the serum of an HIV-1 antibody positive individual. OKT4 surface antigen could not be demonstrated on MT4/HIV-1 cells. On electron microscopic pictures typical and atypical virus particles could be seen near the surface of the cell membrane. The persistently produced virus particles were infective for H9 and MT4 cells. The antigenic structure of the virus produced by MT4 cells was similar to that produced by H9 cells.

[Examination of the diastolic function of the left ventricle by Doppler echocardiography in coronary disease].

The authors examined the left ventricular diastolic function at rest and in pain-free period in patients with chest pain but without previous myocardial infarction. Two patient group were formed according to the results of coronarography: 15 patients had significant coronary artery stenosis, while the coronarography was negative in case of 11 patients. Four Doppler echocardiographic parameters were taken and their sensitivity, specificity, positive and negative predictive values were determined on the base of the result of the coronarography. The small number of patients allows only preliminary evaluation but the results suggest that the altered diastolic function enhances the probability of the presence of significant coronary artery disease, while a normal diastolic function decreases its probability.

[Comparative study of digitalized M-mode and Doppler echocardiography in assessing left ventricular function].

The systolic and diastolic functions were examined using digitized M-mode and Doppler echocardiography in 27 patients with previous myocardial infarction and in 20 healthy subjects. Both methods separated well the patients from the control group but their difference was significantly greater when the ventricular function was assessed with Doppler parametres, digitized M-mode and Doppler echocardiographical results were controlled for possible correlations, but only time intervals showed significant correlations. The highest correlation was found between the values for isovolumic relaxation period. The authors conclude that Doppler echocardiography is to be preferred as a method for assessing the systolic and diastolic functions of the left ventricle.

[Determination of the diastolic function of the left ventricle in post-infarction patients using a complex echocardiographic index].

The diastolic function of 25 patients with ischemic heart disease was studied by both digitized M-mode and Doppler echocardiography. The control groups were a group of healthy subjects and patients with hypertension, resp. There was a significant difference in the diastolic parametres of the healthy and the diseased groups. The altered diastolic function of the latter showed, however, little variation. A new complex diastolic index was formed from the parametres involved in Doppler and M-mode echocardiography. It contains early and late diastolic filling rates, the isovolumic relaxation period, and the diametric change of the left ventricle during the isovolumic relaxation period. This index allowed us to separate the altered diastolic functions of patients with ischemic heart disease from those with left ventricular hypertrophy.

[Reproducibility of the Doppler spectrum of the left ventricular stroke volume].

The left ventricular stroke volume was determined in 15 healthy male volunteers of the same age (21-23 yrs). The systolic function was characterized by six, the diastolic function by five parameters. The diameter and the cross-sectional areas of the outflow tract were determined. Intraobserver, interobserver and day-to-day variability of these parameters were investigated. The reproducibility was tested by 4 statistical methods. The significance of the differences between the mean values was determined, as well as the percentual difference of the mean values and of the variation coefficients. Linear regression analysis was done and the correlation coefficient was determined for each parameter. The variation coefficient could be regarded as the best statistical method for characterizing the reproducibility. Thus the simplest, directly measured Doppler parameters gave the best reproducibility. The reproducibility of the complex, calculated parameters was poorer. Comparing the intra- and interobserver reproducibility error, the former one was less.

Immunoglobulin concentration in the blood serum of foals suffering from pneumonia associated with mycoplasma infection.

Clinical, microbiological, haematological, and immunological investigations were carried out in mares and their foals of 2 studs. A considerable number of foals fell ill with pneumonia, the mortality rate was high. Mycoplasmas were mostly isolated from nasal swabs and from the lungs. The isolation rate of bacteria was lower. Serum IgG concentration was reduced in the diseased or dead foals compared to that of healthy animals, the data stress in importance of IgG in the development of respiratory diseases also in foals.

Epizootiological examination of a respiratory disease associated with mycoplasma infection in horse.

The authors examined 585 samples from 92 mares of 2 studs and 346 nasal swabs taken from their foals for the presence of mycoplasmas. The positive rates of mares and foals were 81.5% and 71.7%, respectively, with positivity of samples being variable. Clinical symptoms developed in 2-4 waves and lasted 3-7 days, with intervals of 7-12 days. The disease started in April among foals born in February, at an average age of 88 days. Later on, the average age of the affected foals decreased. There was a correlation between presence of mycoplasmas in nasal cavity and disease of foals.
