[Effect of carsil and legalon in treating chronic liver diseases].

The clinical effect from the treatment of 48 patients with carsil and 24 patients with legalon is studied. The patients were subdivided into three groups in the base of the clinical, laboratory-chemical and instrumental examination: light hepatic lesions--hepatic steatosis, chronic persisting hepatitis (ChPH), post-hepatitis states, chronic active hepatitis (ChAN) and cirrhosis of the liver (CL). Both preparations were administered 3 three times, 2 tablets daily for 3 months. The results obtained revealed that the bioflavonoid preparation carsil did not much differ in its clinical effect from the preparation legalon. The preparations carsil and legalon had a good effect, but not with statistical significance, on the subjective symptoms--pain, sense of heaviness and upper dyspeptic syndrome in the patients studied. Both preparations had a good effect on the biochemical indices: thymol test, SGOT, gamma-globulins, immunoglobulin G, blood bilirubin. The three month administration of carsil and legalon did not essentially change the histological findings in liver. The preparations carsil and legalon are indicated in light and moderate hepatic affections--hepatic steatosis, ChPH, post-hepatitis states. No contraindications have been reported for the administration of those preparations even in advanced hepatic disorders where they could be included as "basis" therapy.

[Single nonspecific ulcer of the rectum].

The authors report 4 cases with single, non-specific ulcer of rectum. The diagnosis, in all the cases, was made after endoscopic and histological studies. Single ulcer of rectum is for the first time reported in a pregnant woman, that necessitated the interruption of pregnancy. The ulcer could spontaneously be healed and be combined with an ulcer of duodenum. The patients with single ulcer of rectum should be under follow-up care and yearly controlled in the course of three years.

[Serum immunoglobulin levels and histomorphological changes in the liver in chronic liver diseases].

The level of serum immunoglobulins were juxtaposed, in the paper, to the histomorphological liver changes of 121 patients with chronic liver diseases. Immunoglobulins were also studied in 33 patients with alcoholic cirrhosis as well as in 66 healthy control subjects. Myelogram was performed to 40 patients. The authors' own actualized histomorphological classification of the chronic liver diseases was used. The patients with chronic hepatitis and cirrhosis have an elevated serum level of immunoglobulins G, A and M, proportional to the degree of the histomorphological liver changes. Most manifested was the increase of IgA in alcoholic cirrhosis, and steatosis, alone or in combination with inflammatory process, induces no substantial changes in serum immunoglobins. Increased number plasmatic cells was established in the bone marrow of the patients with chronic liver diseases, with cirrhosis in particular, suggesting an extrahepatal participation in immunoglobulin biosynthesis.

[Changes in the histamine content and morphology of the gastric mucosa in duodenal ulcer after pentagastrin stimulation].

A parallel study on histamine concentration and morphology of biopsy from gastric mucosa, was carried out with 60 patients with duodenal ulcer, prior to and post stimulation with pentagastrin--6 micrograms/kg. Histamine was spectrofluorimetrically determined. The functional-morphological changes that developed were followed up on 15, 30 and 60 min after the stimulation. They were a functional character and were due to disturbed microcirculation of the gastric mucosa: edema, venous stasis and leukopedesis. The latter were progressively increased within the course of 1 hour after the stimulation. The histamine-mobilizing effect of pentagastrin was manifested with a short-term decrease, followed by a considerable increase of histamine content in gastric mucosa of the patients with duodenal ulcer.

[Reflux esophagitis (clinical and endoscopic studies)].

The results from the esophagoscopies of patients with esophagitis are reported. The diagnosis esophagitis was made in 238 patients (17.5%) out of the 1389 esophagoscopies performed. Esophagitis incidence is presented according to age groups. With the involvement of the distal third of esophagus proved to be 189 patients (71.0%). The incidence of reflux-esophagitis was studied in some diseases of stomach and duodenum. Reflux-esophagitis is more frequently found in patients with resected stomach, duodenal and gastric ulcers, than in patients with atrophic and superficial gastritis. The clinical symptoms, the characteristic endoscopic and histologic changes in esophageal mucosa were studied in a group of 56 patients with reflux-esophagitis. Esophagoscopy with intentional biopsy is the most valuable method for the diagnosis, precision of the stage and dynamics of reflux-esophagitis.

[Fiber colonoscopic and histomorphological characteristics of true polyps of the large intestine].

By fibrocolonoscopy, fibrocolonoscopic biopsy, polypectomy, operation resp., one hundred and eight true polypi in the colon of 75 patients were studied, grouped according to localization: in caecum -- 2.8 per cent, in ascending colon -- II. I per cent, transverse colon -- 13.9 per cent, descending colon -- 19.4 per cent, in sigmoid colon -- 40.8 per cent and in rectum -- 12 per cent. Endoscopically they were grouped as follows: adenomatous -- 91.7 per cent and villous -- 8.3 per cent and after the pathohistological examination as: adenomatous 73.3 per cent, adenovillous -- 17.2 per cent and villous -- 9.5 per cent. In 40 (52.8%) of the cases, well differentiated polypi were concerned (I grade); in 30 (38%) -- moderately differentiated polypi (II grade) and in 5 (6.5%) -- poorly differentiated polypi (III grade). The studies revealed that the pathohistological examination was a method, specifying the morphological structure of polypus, determining the therapeutic behaviour of the clinicists. Fibrocolonoscopic polyrectomy solves the problem of diagnostics and treatment of the true polypi, contributing to the prophylaxis of colon carcinoma.

[Treatment of duodenal ulcer with Tagamet (cimetidine)].

The results from the treatment of 25 patients with duodenal ulcer with 1 g tagamet daily (3x200 mg after meal and 400 mg in the evening before going to bed) in the course of 20 days, are reported. The subjective complaints of the patients disappeared in 87--100 per cent (in 2/3 of the patients even in the first three days after the initiation of the treatment) and the ulcer in 44 per cent. The immediate results obtained are statistically more significant as compared with the control group (only on diet), but the differences are not significant as regards the groups treated only with oxyferoscorbon and only with Caved-S. Tagamet treatment leads to a decrease of the basic indices of gastric secretion and does not change the content of N-acetylneuroamine acid in the gastric juice. The venous administration of tagamet inhibits the gastric acid production stimulated by pentagastrin. Tagamet treatment does not change the biochemical laboratory indices, the inflammatory and structural changes in the gastric-duodenal mucosa and induces no adverse effects and complications.
