Heart transplantation from an ethical perspective.

Ethical issues in health care have increased during the past two decades, primarily in response to rapidly evolving advanced technologies. This increase in ethical dilemmas has been most felt by nurses because they are the primary health care providers to whom patients and family members turn for support, counseling, and empathy. Heart transplantation is an area of health care that evokes numerous ethical issues. Ethical beliefs must be examined from the aspects of everyone involved: the patient, family members, and health care providers. Some aspects of the transplantation process also warrant special consideration such as transplantation, lack of a viable support system for patients, and cultural issues. Future projections include a growth in the number and types of ethical dilemmas as technology continues to evolve, populations become more diverse and older, and the health care delivery systems continue to employ a more diverse group of health care providers. This article has attempted to give nurses some insight and guidance about the ethical issues currently being encountered. The article has also included the need for future education of nurses and society.

Advanced nursing practice: legal concerns.

Empowerment of nurses in advanced nursing practice begins with an understanding of the current legal status of the advanced nurse practitioner. True empowerment will not occur until the reform regulations needed to expand the role of the advanced nurse practitioner is understood by nurses and measures are taken to begin such reform. In this article, the author reviews the current legal status of advanced nurse practitioners, while projecting the needed reforms that must occur across the country. The aim is to help advanced nurse practitioners undertake measures to ensure that legal empowerment for themselves becomes a reality.

Be an expert witness for critical care nursing.

This presentation concerning the role of a nurse expert witness in cases involving critical care nursing standards of care may be used in determining if the individual nurse would want to serve as an expert witness and in alerting the potential expert witness to role expectations. All phases of the process are described, including discovery, deposition, and trial level testimony.

Personality factors of critical care nurses.

Two hundred members of the American Association of Critical-Care Nurses responded to a mail-out survey done to determine the psychologic profile of critical care nurses in terms of self-esteem, gender identity, and selected personality characteristics. The instruments used were Cattell's 16 PR, the Personal Attributes Questionnaire (PAQ), and the Texas Social Behavior Inventory (TSBI). Their personality factors tended to be aggressive, assertive, competitive, persevering, moralistic, resourceful, and mechanical. The nurses who enjoyed the field most were of the androgynous or masculine type and had high levels of self-esteem. On the basis of these findings, the nurse recruiter or faculty member doing career counseling could assess the personality characteristics, gender identity, and self-esteem levels of interested nurses. The goal would be to identify nurses who would both enjoy the field and remain active in critical care nursing after orientation. The goal could also be to help nurses dissatisfied with critical care nursing to seek means of improving their self-esteem.
