Internal mammary vessels: a reliable recipient system for free flaps in breast reconstruction.

In breast reconstruction with a free flap following mastectomy, the recipient vessels most widely used are in the axillary system, which limits flap movement and flexibility in breast shaping. In addition, scarring and fibrosis can make dissection of the vessels difficult. We have performed 22 breast reconstructions using a free transverse rectus abdominis myocutaneous (TRAM) flap anastomosed to the internal mammary (thoracic) vessels. There has been no flap failure. The surgical techniques and the advantages and limitations of the internal mammary system are presented and the internal mammary vessels compared with the axillary vessels as a recipient vascular system.

[Plastic surgery treatment of chronic venous ulcera cruris].

Due to basic differences between the conservative and surgical approaches to the treatment of chronic venous leg ulcer, there are still a large variety of different therapeutic strategies applied today. Satisfactory results in the treatment of this kind of recurrent ulcers can only be achieved by a combination of plastic and vascular surgical techniques. Our surgical procedure is illustrated by 30 cases treated between 1990 and 1993.

[Biochemical analysis of the endolymph and perilymph of the trout (Salmo gairdneri) (author's transl)].

Cerebrospinal fluid, perilymph and endolymph of the saccule, the utricle and the semicircular duct of the trout were prepared from frozen inner ears. Blood was collected after the decapitation of the animal. In all samples of each animal the concentrations of sodium, potassium, protein and glucose were determined. The electrolyte values and the total protein content were used to estimate the purity of a sample. The concentration of sodium is high in perilymph and low in endolymph whereas potassium is high in endolymph and low in perilymph. The differences between perilymph and the cerebrospinal fluid as well as the blood serum are significant. A relative independent production of the inner ear fluids can be assumed.

Histomorphologic and volumetric analysis of implanted autologous preadipocyte cultures suspended in fibrin glue: a potential new source for tissue augmentation.

Previous efforts to use adipocyte transplants for tissue augmentation have been limited by high and unpredictable resorption rates. Preadipocytes are precursor cells that are capable of replication and differentiation into mature adipocytes. Furthermore, they are more resilient to ischemia, making them a desirable transplant media. Utilizing fibrin glue as a transport vehicle and a prefabricated intramuscular capsule pouch as the recipient site, we have demonstrated the successful transplantation of cultured preadipocytes without the previously presented resorption sequelae. Histological analysis at 2 weeks has demonstrated establishment of vascular supply and the complete resorption of fibrin glue. Most importantly, using planimetric analysis, volume retention has been demonstrated in implanted areas up to 1 year following implantation. Finally, BrdU labeling has been utilized to demonstrate the lack of increased and uncontrolled replication rate, an index of potentially tumorigenic tissue. In conclusion, we have demonstrated a potentially new and safe source of tissue augmentation in the rat model.

An analysis of breast sensation following inferior pedicle mammaplasty and the effect of the volume of resected tissue.

The aim of our study was to evaluate if the amount of resected breast tissue has an influence on breast sensitivity after inferior pedicle mammaplasty. In the prospective study, 15 patients (30 breasts) were divided into two groups: group I (less than 400 g resection) and group II (more than 400 g resection). Preoperatively and six months postoperatively a touch test (Semmes-Weinstein monofilaments) and temperature (warm and cold) tests were performed. The study showed that all patients had increased touch sensitivity six months after inferior pedicle mammaplasty regardless of the amount of resection. In group I an average of 65% and in group II an average of 83% retained sensitivity for temperature postoperatively. These results suggests that this mammaplasty technique preserves intercostal nerves within the inferior pedicle and reliefs chronic nerve traction injury and improves thereby breast sensibility.

A triangular suture to simplify reduction mammaplasty.

A technique to facilitate correct preoperative marking for an en bloc resection in superiorly based mammaplasty is presented. A temporary triangular suture is placed to control the estimated resection lines for the removal of the medial and lateral tissue block. The described technique helps to avoid the formation of medial and lateral dog ears and the unpleasant and visible scars that result.
