Treatment of relapsed or refractory acute myeloid leukemia with humanized anti-CD33 monoclonal antibody HuM195.

HuM195 is a humanized, unconjugated, anti-CD33 monoclonal antibody. Fifty adult patients with relapsed or refractory AML were randomized to receive HuM195 at a dose of 12 or 36 mg/m(2) by intravenous infusion over 4 h on days 1-4 and 15-18. Patients with stable or responding disease received two additional cycles on days 29-32 and 43-46. HuM195 was given as first salvage therapy in 24 patients and as second or subsequent salvage therapy in 26 patients. Pretreatment blast percentage in the marrow was between 5 and 30% in 20 patients with the others having blast counts greater than 30%. The median age of patients was 62 years (range 26-86) and CD33 was detected in 95% of patients for whom immunophenotyping was available. Of 49 evaluable patients, two complete and one partial remission were observed. All three responses were in patients treated at the 12 mg/m(2) dose level and all had baseline blast percentages less than 30%. Decreases in blast counts ranging from 30 to 74% were seen in nine additional patients. Infusion-related events of fever and chills occurred in the majority of patients and were generally mild and primarily related to the first dose of antibody. No hepatic, renal or cardiac toxicities were observed and other adverse events such as nausea, vomiting, mucositis and diarrhea were uncommon or felt to be unrelated to HuM195. In addition, anti-HuM195 responses were not detected. HuM195 as a single agent has minimal, but observable, anti-leukemic activity in patients with relapsed or refractory AML and activity is confined to patients with low burden disease. No significant differences in clinical efficacy or toxicity were seen between the two dose levels of antibody. HuM195 was well tolerated with infusion-related fevers and chills the predominant toxicities seen. Meaningful clinical efficacy of this unconjugated monoclonal antibody may be realized only in patients with minimal residual disease, or in combination with chemotherapy.

[Body build and radiation exposure in static roentgen studies (I): A contribution for determining a national reference dose value. Promoted by the Federal Office for Radiation Protection (St.Sch 4163)].

PURPOSE: To contribute data on radiation exposure in static x-ray procedures and to compare them with anthropometric parameters. METHOD: 121 chest x-rays and 100 lumbar spine examinations were carried out and the dose-area product (DAP) measured for each of the projections. Additionally, body height, body weight and the sagittal and transversal diameters of the examined regions were recorded. Dose measurements were statistically evaluated and the following data determined: Frequency distribution, median, 25%- and 75%-percentiles as well as correlations with sex, body weight and diameters. RESULTS: Median DAP was 13 (men: 16; women: 11) resp. 50 (62; 37) cGycm2 with pa resp lateral chest x-ray. Values were closely correlated with body weight (r = 0.704/0.659) and diameter of the chest (r = 0.657/0.579). Median DAP was 175 (239; 126) resp 531 (670; 361) cGycm2 with ap resp lateral lumbar spine examinations. Values were closely correlated with body weight (r = 0.678/0.666) and diameter of the abdomen (r = 0.664/0.658). CONCLUSIONS: DAP of chest x-rays and lumbar spine examinations is strongly influenced by the constitution of the patients. Men are nearly twice as largely exposed to radiation as women.

[Body structure and radiation exposure in dynamic X-ray studies. A contribution to determining national reference values].

PURPOSE: To contribute data on radiation exposure in dynamic X-ray procedures and to compare them with anthropometric parameters. METHODS: 50 venographies of the lower extremity, 51 abdominal arteriographies, and 121 double contrast examinations of the small bowel were carried out and the dose-area product (DAP) measured for each of the procedures. Additionally, fluoroscopy time, number of shots, body height, body weight, and the sagittal diameter of the examined region were recorded. Dose measurements were statistically evaluated and the following data determined: Frequency distribution, mean, median, 25%- and 75 percentiles as well as correlations with sex, body weight, and diameters. RESULTS: Median DAP was 587 (men: 540; women: 636) cGycm2 with venography of the lower extremities. DAP/min was better correlated with body weight (r = 0.663) than with diameters of the thigh (r = 0.611). Mean DAP was 14,869 (18,070; 10,980) cGycm2 with abdominal arteriography. DAP/min was badly correlated both with body weight (r = 0.285) and diameters of the abdomen (r = 0.249). Mean DAP was 4527 (5096; 4114) cGycm2 with enteroclysis. DAP/min was well correlated with both body weight (r = 0.621) and diameters of the abdomen (r = 0.628). CONCLUSION: DAP of venography of the lower extremity and enteroclysis is greatly influenced by the constitution of the patient. The differences between men and women are, however, not as striking as with static X-ray procedures. By contrast, in abdominal arteriography body weight and diameter play a limited role for radiation exposure.

Suitability of children for ENT day case procedures.

This prospective study evaluates the suitability of 500 children under 12 years of age using both medical and social criteria, to determine the proportion of children suitable for increasing the scope of ENT operations performed as day cases. Co-existing medical conditions that could interfere with both surgery and anaesthesia were found in 39 (8%). A combination of adverse social and domestic conditions, including the number of adults and children resident, number of bedrooms and individuals sleeping in the child's bedroom, the number of cars, and the presence of a telephone, were present in 171 (34%) of children's homes and were almost exclusively found in National Health Service (NHS) patients. Private patients, 31 (6%) were more suitable for day surgery. Guidelines need to be followed to minimize the risk if the number of procedures is increased, if more patients were treated privately, the proportionate number of patients requiring in patient care would rise in the National Health Service (NHS).

Inhibition of lymphoma growth in vivo by combined treatment with hydroxyethyl starch deferoxamine conjugate and IgG monoclonal antibodies against the transferrin receptor.

Synergistic inhibition of hematopoietic tumor growth can be observed in vitro when the iron chelator deferoxamine (DFO) is used in combination with an IgG mAb against the anti-transferrin receptor antibody (ATRA). Our goal was to ascertain whether similar findings could be seen in vivo. A high molecular weight conjugate of deferoxamine, known as hydroxyethyl starch (HES) DFO or HES-DFO, was tested in conjunction with C2, a well-defined rat antimouse transferrin receptor mAb, against the 38C13 tumor in C3H/HeN mice. It was shown that while neither HES-DFO alone nor C2 alone produced consistent, significant inhibition of tumor growth, the combination of HES-DFO and C2 produced virtually complete inhibition of initial tumor outgrowth. The latter combination failed, however, to inhibit the growth of established tumors. It was then found that when C2 was used in conjunction with RL34, another IgG ATRA, the two ATRAS were themselves capable of causing synergistic inhibition of the growth of 38C13 in vitro. When the two IgG ATRAS were used together in vivo, regressions of established tumors were observed. Moreover, the addition of HES-DFO to the IgG ATRA pair then caused more frequent regressions. Although there was never any obvious toxicity seen with a single IgG ATRA, the use of the IgG ATRA pair was associated with sporadic mortality. In addition, although HES-DFO by itself was also not associated with any obvious toxicity, combined treatment with HES-DFO and a single ATRA resulted in death due to bacterial infection in about half of the mice after 10-15 days. Combined treatment with HES-DFO and the ATRA pair resulted in death attributed to infection in nearly all of the mice after 6 days. Thus, an iron deprivation treatment protocol with HES-DFO and IgG ATRAS produced both a significant antitumor effect and an increased risk of infection in a murine model system.

Effect of ankle position and a plaster cast on intramuscular pressure in the human leg.

Intramuscular pressure was measured with transducer-tipped catheters that had been inserted into the anterior and deep posterior compartments of the leg in seven healthy adults. Intramuscular pressure increased three to sevenfold (depending on the position of the ankle) in both compartments after the application of a plaster cast from the proximal part of the thigh to the malleoli. While the cast was in place, the baseline intramuscular pressure was elevated by the inflation of a tourniquet, which was located on the proximal part of the thigh, to a pressure of sixty millimeters of mercury (8.00 kilopascals). The intramuscular pressure in both the anterior and the deep posterior compartments was found to be lowest when the ankle joint was between the neutral and the resting positions (between 0 and 37 degrees of flexion). After the cast was bivalved and the opening on each side was spread approximately one-half centimeter, there was a significant decrease in intramuscular pressure of 47 per cent in the anterior compartment and of 33 per cent in the deep posterior compartment (p < 0.05 for both).

Trochlear nerve schwannomas: a case report and literature review.

Trochlear nerve sheath tumours are extremely uncommon, only six cases diagnosed during life having been presented previously. In none of these earlier cases were magnetic resonance imaging studies obtained. We report here upon the clinical presentation, surgical management and post-operative course of a case where the diagnosis was suspected pre-operatively from MRI studies. The radiological appearances are described, together with a review of all previously published accounts of this rare tumour.

Staining and histomorphometry of microcracks in the human femoral head.

We developed staining techniques that permit identification and histomorphometric analysis of microcracks in the human femoral head 1) from thick, ground bone sections (100 microns) by prestaining with the Villanueva mineralized bone stain (MIBS), and 2) from plastic embedded, undecalcified thin bone sections (5-15 microns) by staining in gallocyanin chrome alum-Villanueva blood stain methods. Both methods represent a significant improvement in the stainability of the microcracks, cellular and tissue elements, and the simultaneous assessment of osteoid seams and tetracycline markers by histomorphometry. Shrinkage and other artifacts were minimized, which helped to clarify some of the uncertainties arising from artifacts resulting from some bone staining methods. Histomorphometric analyses of microcracks were conducted on thick, ground sections of subchondral and trabecular bone. Microcracks were more prevalent in the subchondral bone and osteochondral junction than in the more distant trabeculae. We have consistently localized microcrack areas in bone tissues prepared in these ways.

Splinting in rheumatoid arthritis: I. Factors affecting patient compliance.

Fifty-four patients with rheumatoid arthritis were fitted with either unilateral or bilateral hand and wrist splints. Of 36 patients available for evaluation, 29 (greater than 80%) reported wearing their splints more frequently for activities requiring greater strength than dexterity. Patients also reported pain relief from wearing their splints, but did not report donning them for this purpose. We feel that a vigorous educational effort should be promoted to encourage patients to wear splints for diminution of inflammation and relief of pain as well as for support during heavier activities.

Splinting in rheumatoid arthritis: II. Evaluation of Lightcast fiberglass polymer splints.

Fifty-four patients with rheumatoid arthritis were fitted with fiberglass polymer Lightcast II hand and wrist splints. When patients were questioned after having worn the splints for 9 to 12 months, the majority responded that the splints were durable, cosmetically acceptable, and for the most part comfortable. The splints were staff manufactured at 1 sitting in less than an hour each.

Inhibitors of phenylethanolamine N-methyltransferase and epinephrine biosynthesis. 2. 1,2,3,4-Tetrahydroisoquinoline-7-sulfonanilides.

1,2,3,4-Tetrahydroisoquinoline-7-sulfonanilides (1-14) related to 1,2,3,4-tetrahydroisoquinoline-7-sulfonamide (21,SK&F 29661) were prepared and studied for their ability to inhibit phenylethanolamine N-methyltransferase (PNMT) in vitro. The choice of substituents on the 7-phenyl group of the sulfonanilides was based on the Topliss approach to structure-activity relationship studies. Information about the importance of an acidic hydrogen atom on the sulfonamide nitrogen atom was obtained from the preparation and testing of a tertiary N-methylsulfonanilide (15). Other THIQ's (1,2,3,4-tetrahydroisoquinolines) containing sulfur substituents in the 7 position were prepared and tested and consisted of 7-N-benzyl and 7-N-phenethyl derivatives of SK&F 29661 (16-18) and 7-(phenacylthio)-and 7-(phenacylsulfonyl)-THIQ (19 and 20). The two most potent inhibitors were the 7-p-bromo- and -chlorosulfonanilides, 2 and 6. However, neither was an effective inhibitor of norepinephrine to epinephrine conversion when tested in an in vivo mouse assay at unit doses of 25 or 100 mg/kg.
