[Essential mixed cryoglobulinemia with kidney involvement].

Essential mixed cryoglobulin associated with rapidly progressive glomerulonephritis has been observed in a 64-year-old patient. The cryoglobulin consisted of monoclonal IgM-kappa and polyclonal IgG and displayed rheumatoid-factor activity. The antinuclear antibody test was negative. In the immunofluorescent method renal biopsy showed positive staining for IgG; IgM and the early and late complement components. The rheumatoid factor activity of the renal IgM deposits was demonstrated by binding of fluorescent aggregated IgG. There is thus evidence that the vasculitis observed in essential mixed cryoglobulin is caused by IgM-IgG complexes. The differentiation of essential mixed cryoglobulin from lupus nephritis with cryoglobulins is discussed.

Arteritis

[Goodpasture's syndrome (author's transl)].

Goodpasture's syndrome occurred in six patients (four men and two women) aged 18-32 years. Haemoptysis, anaemia and abnormal urinary findings with erythrocyturia were the first signs of the disease in five instances, in one the renal signs predominated. In five patients the disease took a malignant course with uraemia developing within six weeks. One patient is in a spontaneous remission eleven months after the onset of the disease. Three patients died of recurrent haemoptysis (non-nephrectomised), two nephrectomised patients are still alive half a year and three and a half years later, respectively. A typical linear IgG-, beta1A, alpha2D and beta1E deposition along the glomerular capillaries was demonstrated immunohistologically in all subjects. Of three lung biopsies one also had typical IgG and beta1A-positive linear fluorescence along the alveolar basal membrane. Immunoglobulins with basal membrane antibody activity were eluted from renal homogenates in two patients, basal membrane antibody activity in serum was demonstrated only in one of three patients.

[Quantitative Cytochemical and Morphometric Investigations on Uterine Angiomatosis (Endolymphatis Stromal Myosis) and Stromal Sarcoma].

The angiomatosis uteri shows a low scattering of nuclear DNA content; an euploid DNA stem line suggests a benign behavior in these tumors. This evidence is supported by clinical experiences. The endometrial stromal sarcoma and the angioblastic sarcoma are characterized by a high scattering of nuclear DNA content, an aneuploid DNA stemline and a substantial irregularity of the nuclear area. These difference, suggest the malignancy of stromal and angioblastic sarcoma.

An electron microscope study of "Pacinian neurofibroma".

An electron microscope study has been made of the "Pacinian neurofibroma". Unlike the usual neurofibroma the "Pacinian neurofibroma" is characterized by a proliferation of the so-called perineurial cells. Groups of surface vesicles, the absence of mesoaxons and a fragmented basallamina differentiate these perineurial cells from Schwann cells. The formation of the perineurial cells can be traced continuously from small and wide submicroscopic cellbands and clubshaped thickenings to ribbonlike cell complexes as well as to tactile-like structures. The most developed complexes and structures are visible with the light microscope. These formations do not correspond with real tactile corpuscles, rather they can be considered as neoplastic structures of the perineurium.

Effect of glucose load on synthesis of plasma glucose in lactating cows.

We assess the effect of an intravenous glucose load on production of glucose in lactating dairy cows. Isotope dilution techniques were utilized to measure transfer rate of plasma glucose (with tritiated glucose) and gluconeogenesis (with carbon-14 labeled propionate or carbon dioxide). A glucose load equivalent to the cow's normal transfer rate decreased endogenous glucose output two-thirds and decreased gluconeogenesis by half. Glucose concentration and transfer rate were higher within 2 h after glucose load started while carbon-14 transfer to plasma glucose became increasingly depressed and persisted for at least 1 h after loading terminated. Reduced plasma glucose synthesis from propionate during loading was associated with increased transfer of propionate carbon to plasma phospholipids, triglycerides, and cholesterol esters, and with increased plasma insulin concentration. It appears that glucogenic precursors are utilized for greater synthesis of lipid (and presumably glycogen) and that loading leads to metabolic changes causing net reduction in gluconeogenic capacity. These changes could have important implications for use of intravenous glucose in treating hypoglycemic metabolic conditions.

[Advantages of the initial therapy of acute schizophrenia with large doses of droperidol/A comparative study (author's transl)].

The treatment of acute schizophrenic disorders with large doses of droperidol was compared to clozapine and clopenthixol in a special study. Apart from the clinical protocol, psychometric test for the assessment of social behaviour and thought disorders were used. The advantages of the treatment with droperidol, especially in highly disturbed acute schizophrenic patients, are shown and discussed by means of the results of psychological tests and clinical experience.
