[Modification of the risk profile of obese essential hypertensive patients by the group "hypertension discussion" program].

The programme "Discussions on Hypertension" is a programme for group treatment of obese essential hypertensives in medical practices on the basis of behavioural therapy. A co-operation model was developed between the Department for General Medicine of the "Medizinische Hochschule Hannover", the Institute for Medical Psychology of the "Philipps-Universität Marburg" and the company Galenus Mannheim and was offered to doctors in general practice. The effects of the programme on the modifying of the risk profile for coronary heart disease was investigated in a controlled study in 137 patients by nine general practitioners. 81 patients were enrolled in the experimental group and 56 patients were assigned to a control group that was not using the programme. The experimental and control groups were comparable with respect to sociodemographic, psychological and medical parameters. Three months after completing the programme, the results of the study was shown in specific effect both on the psychological criteria of success (well-being, health knowledge, healthy behaviour, salt use and compliance) and in a reduction of the diastolic blood pressure in spite of a reduction of the antihypertensive drug treatment. The blood lipid levels (cholesterol and triglycerides) and the blood glucose level were also favourably affected. Patients in the experimental group had reduced their weight by an average of 5.6 kg. The patients in the control group only by 0.8 kg. Overall, there was a marked reduction in the proportion of patients with several risk factors in the experimental group, so that the programme can be attributed a preventive effect.

[Experimental combined impression trauma of the brain and spinal cord].

Experimental investigations in guinea pigs after impression trauma by means of the weight fall technique showed in the brain and the spinal cord cellular changes resembling as such in man. No significant differences were presented between pure craniocerebral and combined traumas in the animal brain. The impression traumas of the spinal cord produced exclusively hemorrhages in the central grey matter of the spinal cord and in one half of the cases supplementary reactionary changes of the brain. Edematous changes in the spinal cord were observed in 70% of the animals in case of pure craniocerebral traumas.

[Comparative implant analysis in the operative treatment of unstable per- and subtrochanteric femoral fractures and results of 300 osteosyntheses with the 130 degree double T plate].

We have found that the 130 degrees-Double-T-Plate, when used to repair unstable intertrochanteric femoral fractures in patients in the eighth decade, where high morbidity (92%), osteoporosis and immobility is common, lowers the postoperative complication rate (8,5%), mortality rate (12%) hospital course (means = 24 days) and, last but not least, the total cost of medical care. The biomechanical superiority of these implants over others was shown by comparing moments of inertia, elasticity, bending strength, load capacity and spring constant. In 300 cases of unstable intertrochanteric femoral fractures treated with the 130 degrees-Double-T-Plate, 79% yielded very good to good results, 11% satisfactory results and 10% poor results.

[Efficacy of experimental renal embolization with microspheres. Additionally, a critical report on the complication rate of intra-arterial embolization with particles (author's transl)].

Renal embolization was performed in 12 dogs using the Seldinger technique and microspheres 250 +/- 40 micrometer in diameter. Periodical arteriography, microangiographies and histological analysis was carried out to control the efficacy of embolization. As a result, renal arteries could be embolized wi. th microspheres, however, because of statistical distribution there were histologically anemic and normal parenchyma situated closely together. In more than 50% there was a reflux of microspheres which could only be detected by histological examination, while angiographies showed a totally normal vascular pattern. Microspheres should not be used in clinical embolization. Experimental investigation of embolization materials must almost embrace histological analysis.

[Permanent stop of renal circulation by embolisation with microspheres. Preliminary report (author's transl)].

The risk of tumor cell dislodgement during operative removal of carcinoma of the kidney is reduced by preoperative occlusion of the renal arteries. After the intraarterial injection of 250 mu microspheres a complete blockage of the intrarenal circulation was found in all three experimental animals for at least 2 hrs. In the other two dogs the blockage remained unchanged over 24 hrs. Subsequent experiments shall demonstrate that the simple embolisation technique using microspheres is more stable than other methods.
