Thyroid peroxidase (TPO) as a tumor marker in the follow-up of differentiated thyroid carcinomas with surgical and ablative radioiodine therapy. An assessment after evaluation.

The clinical significance of serum thyroid peroxidase (TPO) for differentiated thyroid carcinomas(DTA) is estimated differently. In our preliminary studies it was found that TPO presented information extending those that from hTG. For further clarification a prospective follow-up study was performed including 66 female and 14 male total thyroidectomized patients with DTA for the time course of TPO and human thyroglobulin (hTg) in relation to the ablative radioidine therapy (ART). In 34/50 evaluable cases TPO levels showed an approximately analogous time course with hTg. In relation to the extension of residues, some cases presented increasing of TPO and hTG after radioiodine treatment. 6/7 patients suffering from extended postoperative residues with high anti hTg levels but without elevated TPO concentrations showed distinctly elevated TPO values. Therefore, TPO seems to be an indicator for the destruction of normal thyroid cells or thyroid tumor cells. The clinical value of TPO seems to be in the time being limited to DTA due to false negative hTg values. However, it should be possible that TPO can did the evaluation of thyroid specific therapy.

Chromogranin A: an additional tumor marker for postoperative recurrence and metastases of medullary thyroid carcinomas?

In this study, plasma concentrations of chromogranin A, calcitonin and carcinoembryonic antigen (CEA) were measured in 40 healthy volunteers as well as in 129 patients with recurrences and/or metastases of neuroendocrine tumors and of medullary thyroid carcinomas (MTCs). A double antibody assay was employed using polyclonal rabbit antibodies to a C-terminal fragment of the protein for detection of human chromogranin A. Using ROC analysis, a cutoff at 22 U/l chromogranin A was calculated. In patients with neuroendocrine tumours, much higher serum concentrations of chromogranin A than for patients with MTC (80% vs. 46%) were measured. The following sensitivities were found: chromogranin A; 46%, calcitonin 100%, CEA 52%. Furthermore, the mean values of chromogranin A concentrations correlated with the tumour mass and/or number of metastases in MTC and neuroendocrine tumours. Evaluation of follow-up studies remains to be completed; however, preliminary results showed similarities regarding the behaviour of chromogranin A and calcitonin. Despite the findings of this study, the observations could not confirm chromogranin A as a reliable marker for metastazing or recurrent MTC.

[The relationship between the chemical structure and anti-arrhythmia action of a number of omega-aminoacyl-3-carbalcoxyaminodibenzazepines].

The antiarrhythmic activity of 20 derivatives of dibenzazepine (the effects on the maximal effective rabbit heart auricle contraction rate, aconitine-induced arrhythmia in rats under or without anesthesia) was studied. It was shown that the most active compounds are those with carbethoxyamine group in position 3 in combination with dimethylamino- or diethylamino-acetyl groups in position 5 of dibenzazepine ring. 5-dimethyl-aminoacetyl-10,11-dihydro-5H-dibenz b, f azepine (GS-015, bonnecor) was selected for the further detailed investigation.

Two ways to establish potential At-211 radiopharmaceuticals.

The special radiophysical characteristics of the alpha emitting cyclotron radionuclide At-211 favour its therapeutic application. Studies for therapeutic use have been carried out over the past 15 years. Labelling reactions with this heavy halogen and characterization of the products are important because of the low concentrations used in treatment (non carrier added level). One successful way to produce At-211 radiopharmaceuticals is the labelling of aromatic iodine compounds with At-211 via isotope exchange or reaction of organostannyl precursors with astatine. These products enter the tumour by its metabolism. To a certain extent, the intraarterial application of At-211 labelled particles seems to be a promising alternative to intravenous application, causing selective accumulation of therapeutic At-211 doses as shown by the first alpha endoradiotherapy of a incurable recurrent lingual carcinoma.

Can thyroid peroxidase be used as a complementary tumor marker besides thyroglobulin? Preliminary experience with determination of TPO in differentiated thyroid carcinomas.

Serum TPO concentrations were studied compared with the hTG levels in 47 patients suffering from different thyroid carcinomas (before and several times after the first, second or third radioiodine therapy respectively. An immunoluminometric coated tube assay was applied recognizing this enzyme (Brahms Diagnostica GmbH). Elevated TPO levels could be seen in 18/47 pts. All of them suffered from large postoperative remnants or metastases. An increase of TPO concentration was seen frequently at days 2 and 3 following the first or second radioidine application. Although the hTG response seems to be more sensitive, TPO may be helpful to reduce the number of false positive hTG findings.
