Percutaneous transluminal coronary angioplasty. Its success rate and affecting factors.

From December 1987 to October 1983, percutaneous transluminal coronary angioplasty (PTCA) was performed in 302 patients with 392 diseased vessels and 440 lesions. The success rate was 93.71% in 302 patients, 94.90% in 392 diseased vessels and 95.00% in 440 lesions. The success rate was 98.92% in Type A lesion, 95.71% in Type B lesion, and 86.57% in Type C lesion (A vs B P = NS, A vs C P < 0.01). There were 55 lesions with total or subtotal occlusion, the success rate was 89.09% (93.10% in Type B lesion and 84.62% in Type C lesion). As to the diseased vessels, the success rate was 95.65% in LAD, 94.38% in LCX and 93.75% in RCA. The results showed no significant difference. In this series, acute ischemic complications were found in 6.59% (29/440). Of these 29 lesions, 23 were treated successfully, 5 developed Q wave myocardial infarction and 1 died.

Prediction of life-threatening arrhythmia in patients after myocardial infarction by late potentials, ejection fraction and Holter monitoring.

In order to compare the prognostic significance of late potentials (LPs) on signal-averaged electrocardiogram (SA-ECG), left ventricular ejection fraction (EF) and 24-hour Holter monitoring (HM) following myocardial infarction, a prospective study on 60 patients (age 61.7 +/- 8.02 years old) just after acute myocardial infarction (AMI) was done. LPs, EF and HM were performed in all patients. Coronary arteriography had been done in 25 patients. The results showed that LPs were associated with a slightly higher incidence of life-threatening arrhythmia (34.8%) than HM (28.6%) and EF (25%). During the follow-up period (10 +/- 6 months), 9 patients had serious ventricular arrhythmic events, among whom 3 had sudden death due to ventricular fibrillation. The event rate in patients with abnormal LPs was higher than in patients with normal LPs (p = 0.01, odds ratio = 19.2). The study showed that there was no correlation between abnormal LPs and sex, age, number of narrowed coronary arteries, ventricular aneurysm, location of myocardial infarction, or EF alone. But there was a correlation between abnormal LPs and high grade ventricular ectopic activity detected by HM (r = 0.62899, p = 0.024). In addition, the combination of abnormal values of LPs, EF and HM could predict sustained ventricular tachycardia or sudden death in the first year after myocardial infarction with very high sensitivity (100%) as well as high specificity (p = 0.0009, odds ratio = 19).

[Effects of microiontophoretically applied OMF and U-50488 on spontaneous discharges of respiration related units in the region of nucleus of solitary tract of rat].

It was observed that ohmefentanyl (OMF) and U-50488, when applied microiontophoretically, affected the spontaneous discharges of respiration related units (RRUs) in the nucleus of solitary tract of rat. Of the 44 RRUs recorded, OMF produced depressive effect in 21, excitatory in 7, biphasic effect in 5 while 11 units were not affected. Naloxone administered iontophoretically blocked both the depressive (6 out of 7) and the excitatory units (2 out of 3). Of the 45 RRUs recorded, U-50488 produced depressive effect in 15, excitatory in 5, biphasic effect in 3, while 22 units were not affected. Naloxone blocked both the U-50488 depression (7 out of 9) and excitation (1 out of 2) effect. U-50488 antagonized OMF-induced depression in 4 out of 6 units. The results suggest that depression and excitation produced by OMF and U-50488 may be mediated by different subtypes of opiate receptor.

[Effects of microiontophoretically applied U-50488 on the spontaneous discharges, of respiration related units (RRUs) in the region of nucleus ambiguus or rat].

In the present study, the effects of microiontophoretically applied U-50488 on the spontaneous discharge of respitation related units (RRUs) in nucleus ambiguus or rats were observed. Of 96 RRUs recorded, U-50488 produced depressant effect on 51 units, excitative effect on 11 units, excitation followed by depression on 6 units, depression followed by excitation on 3 units. The remaining 25 units were not influenced. Naloxone administered iontophoretically not only blocked U-50488-induced depression in 12 out of 16 units, but also blocked U-50488-induced excitation in 2 units. Duration of the depression produced by U-50488 was remarkably longer than that of excitation. The result suggested that kappa receptor might be involved in respiratory regulation and that the subtype of opiate receptors which mediates depression and excitation effect might not be the same.

[Effects of microiontophoretically applied OMF on spontaneous discharges of respiration related units (RRUs) in the region of nucleus ambigus of rat].

In the present study, it was observed that ohmefentanyl (OMF) affected microiontophoretically applied spontaneous discharges of respiration related units (RRUs) in the nucleus ambigus of rats. Of 96 RRUS recorded, 48 units were depressed following OMF application, 14 units increased in discharges, 14 units increased and then decreased, 9 units decreased and then increased. The remaining 11 units were not influenced. Naloxone administered iontophoretically not only blocked OMF-induced depression in 11 out of 15 units, but blocked OMF-induced facilitation in 5 out of 6 units. U-50488 administered microiontophoretically antagonized OMF-induced depression in 9 out of 13 units. Duration of the depression produced by OMF was much longer than that of the facilitation. The result suggests that the subtype of opiate receptors mediating the depression seen to be different from that responsible for the facilitation.

[Diagnosis of myocarditis with endomyocardial biopsy].

Endomyocardial biopsy (EMB) was performed in 24 cases suspected of having myocarditis and 42 cases with cardiac symptoms and signs of unknown etiology. There was an obvious correlation between the clinical manifestations and the pathological findings in cases suspected of having myocarditis (r = 0.5296, P less than 0.005). The ratio of correspondence was 78.8%. It is shown that EMB is more sensitive than 201TI myocardial imaging in diagnosing myocarditis and serial EMB can follow its clinical process closely. Moreover, EMB can discriminate myocarditis from myocardiopathy.

Biopsy

[Basic research on endomyocardial biopsy].

To determine the risk of biopsy in different parts of heart and the artifacts of histomorphology, right heart endomyocardial biopsy in eight dogs were done. The result showed no tamponade occurred even 53 pieces of heart muscles were got from right ventricular free wall. In contrast, biopsies on right atrial free wall had higher risk in causing tamponade. It was confirmed by autopsy and histomorphology that endomyocardial biopsy could cause heart muscle damage. The artifacts caused by biopsy of normal beating heart muscle include: contraction bands (21.8%), widen of intracellular space (100%), interstitial edema (78%) and interstitial bleeding (53%). The results of the patients of right ventricular endomyocardial biopsy may provide practical value in the application of endomyocardial biopsy.

[Comparison of thallium-201 images in dilated cardiomyopathy and ischemic cardiomyopathy].

Comparison of thallium-201 image between 21 patients with dilated cardiomyopathy (DCM) and 12 patients with ischemic cardiomyopathy (ICM) was made in this study. All cases had abnormal thallium images. Multiple myocardial segments presenting with patchy pattern were seen in all 21 patients with DCM. 75.0% of patients with ICM also had patchy pattern on the thallium image, but the number of patchy pattern segments on image was significantly less in ICM than in DCM (P less than 0.001); complete defects on thallium image were noted in 75.0% of patients with ICM, however, only one case with DCM had complete defect on thallium scan (P less than 0.001). When the thallium image characterized by multiple patchy pattern segments without complete defects was used as a diagnostic criterion for DCM. 95.2% of patients with DCM coincide with the criterion. When the thallium image showing complete defect was used as a diagnostic criterion for ICM, 75.0% of patients with ICM coincide with the criterion. The characteristics of thallium-201 image are useful either for diagnosis of DCM and ICM or in differentiating DCM from ICM.
