Medical complications during stroke rehabilitation.

BACKGROUND AND PURPOSE: We sought to evaluate the effect of setting on the rate of medical complications during stroke rehabilitation. METHODS: A study of the frequency and nature of medical complications in stroke rehabilitation was undertaken in 245 patients managed either on a stroke rehabilitation unit (n = 124) or on general medical wards (n = 121). The stroke unit setting was characterized by established protocols for prevention, early diagnosis, and management of complications (eg, aspiration, infections, thromboembolism, pressure sores, depression, stroke progression). Similar protocols did not exist on general medical wards except for thromboembolism, pressure sores, and secondary stroke prevention. RESULTS: Medical complications were documented in 147 patients (60%) and were more common in patients with severe strokes (97%). The frequency of reported complications was similar in both settings. Aspiration (33% versus 20%; P < .01) and musculoskeletal pain (38% versus 23%; P < .05) were more commonly documented on the stroke unit, whereas urinary problems (18% versus 7%; P < .01) and infections (49% versus 25%; P < .01) were more commonly seen on general medical wards. The reported frequency of deep vein thrombi, pressure sores, and stroke progression was similar in both settings. Although depression was reported equally in both settings (34% on the stroke unit versus 27% on general wards), patients on the stroke unit were more likely to be treated compared with general wards (67% versus 36%; P < .05). CONCLUSIONS: The study shows that inpatient stroke rehabilitation is a medically active service. Management on specialist units is associated with earlier detection and management of stroke-related problems and prevention of potentially life-threatening complications.

[The diagnosis and treatment of the epithelial salivary gland tumors in children and adolescents].

Sixty-six cases with epithelial salivary gland tumors in children and adolescents, who were less than 16 years old, were analized. Those cases accounted for 3.6% of all epithelial salivary gland tumors in our school. Twenty-five cases (37.9%) were benign tumors, among which pleomorphic adenoma was the most common. Forty-one cases (62.1%) were malignant tumors, in which mucoepidermoid carcinoma was the most common one. Three quarters of cases were in the group over 10 years old. Six cases were in the group less than 5 years old. Among them 5 cases were malignant tumors. The younger the children is, the more possible the malignant tumor is. Two cases with malignant tumors had the history of receiving radiotherapy because of the hemangiomas at the same locations. It indicates that the occurrence of the malignant tumors was related to radiotherapy before. The diagnosis, differential diagnosis and treatment of the tumors were discussed. The initial radical resection of the malignant tumors was emphasized.

An association study between essential hypertension and HLA-DRB1 alleles.

It is well established that genetic and environmental factors are involved in the etiology of essential hypertension (EH). Previous studies have suggested that at least one of the HLA genes is responsible for the genetic susceptibility to EH. Our aim in the present study was to investigate this issue in China by the PCR-SSP HLA-DRB1 typing method. The results showed an increased frequency of HLA-DR2 and a decreased frequency of HLA-DR7 with EH patients compared with controls. We consider that HLA-DR2 may represent a marker for susceptibility to EH in the North Chinese population.

Are dihydropyridine receptors downregulated in the ischemic myocardium?

OBJECTIVE: We investigated the effect of ischemia on cardiac dihydropyridine receptors, which correspond to L-type sarcolemmal calcium channels. METHODS: Isolated working rat hearts were perfused aerobically for 10 min, and then subjected to 10-60 min of global ischemia. Control hearts were perfused aerobically for 30 min. [3H]PN 200-110 binding was measured in the unfractionated homogenate, in a crude membrane preparation and in a microsomal fraction. RESULTS: In the homogenate obtained from control hearts, the Kd and Bmax averaged 0.23 +/- 0.05 nM and 84 +/- 4 fmol/mg protein, respectively, and ischemia did not produce any significant change in these variables. Similar results were obtained in the crude membrane preparation (Kd = 0.29 +/- 0.08 nM, Bmax = 113 +/- 7 fmol/mg, yield of binding sites = 98 +/- 6%, no significant change in these variables during ischemia). On the contrary, in the microsomal fraction, the Bmax for [3H]PN 200-110 decreased after ischemia (115 +/- 15 fmol/mg after 20 min of ischemia vs. 190 +/- 34 fmol/mg in the control condition, P < 0.05), without any change in the Kd. In this fraction, the yield for PN 200-110 binding sites was 4.7 +/- 0.6% in the control condition and 2.8 +/- 0.5% after ischemia (P < 0.05). The yield of other sarcolemmal markers such as [3H]quinuclidinyl benzylate and [3H]ouabain binding sites was not reduced in the microsomal fraction obtained ischemic hearts. CONCLUSIONS: The total number of cardiac dihydropyridine binding sites was not downregulated during ischemia, although their distribution after tissue fractionation was slightly modified, possibly reflecting receptor redistribution between different subcellular pools.

[Smoking and the development of Warthin tumor of the parotid gland].

The smoking history was surveyed in 128 cases with Warthin tumor of parotid gland in order to investigate the relationship between smoking and the development of the tumor. Two hundred cases of middle or old age normal persons and 136 cases with pleomorphic adenoma in parotid gland served as the controls. The percentage of smoker in patients with Warthin tumor (96.9%) was much higher than that of middle or old age normal persons (25.5%) and patients with pleomorphic adenoma (24.3%). Morever, the amount of smoking was greater and the smoking history was longer in patients with Warthin tumor. When the male and female patients were analysed respectively, the same results were shown. This case--control study suggests that smoking may be one of the etiologic factors associated with the development of Warthin tumor.

The Schizosaccharomyces pombe pka1 gene, encoding a homolog of cAMP-dependent protein kinase.

We have isolated 16 independent Schizosaccharomyces pombe cDNA clones that suppress the temperature-sensitive (ts) phenotype of a Saccharomyces cerevisiae strain containing the dominant-negative RAS2val19ala22 allele. Fourteen of these cDNAs encode Sz. pombe Ras1. The other two clones encode the C-terminal region of a protein we have named Pka1. We have cloned the pka1 gene from a Sz. pombe genomic library. It contains an uninterrupted open reading frame encoding a 512-amino-acid (aa) protein. The C-terminal region (aa 200-512) of Pka1 is 51-63% identical to cAMP-dependent protein kinase (Pka) catalytic subunits from other eukaryotes. Production of Pka1 suppresses the ts phenotypes exhibited by Sa. cerevisiae ras1-ras2ts or cyr1ts strains. Furthermore, overproduction of Pka1 in Sz. pombe results in a sterile phenotype and an abnormal morphology similar to that exhibited by cells in which the cAMP pathway is constitutively activated. These observations suggest that pka1 encodes the Sz. pombe Pka catalytic subunit.

Effect of ischemia and reperfusion on cardiac ryanodine receptors--sarcoplasmic reticulum Ca2+ channels.

We investigated the effect of ischemia and reperfusion on the cardiac ryanodine receptor, which corresponds to the sarcoplasmic reticulum Ca2+ channel. Isolated working rat hearts were subjected to 10 to 30 minutes of global ischemia, followed or not by reperfusion. Ischemia produced significant reduction in the density of high-affinity 3H-ryanodine binding sites, determined either in whole-heart homogenate (Bmax, 220 +/- 22, 203 +/- 12, and 228 +/- 14 fmol/mg protein after 10, 20, and 30 minutes of ischemia versus 298 +/- 18 fmol/mg protein in the control condition; P < .01) or in a fraction enriched in sarcoplasmic reticulum (Bmax, 1.08 +/- 0.15 pmol/mg protein after 20 minutes of ischemia versus 1.69 +/- 0.08 pmol/mg protein in the control condition; P < .01). The Kd (1.5 +/- 0.1 nmol/L) and the Ca2+ dependence of high-affinity 3H-ryanodine binding were not affected by ischemia. The density of low-affinity 3H-ryanodine binding sites was also reduced after 20 minutes of ischemia (14.0 +/- 2.3 versus 34.0 +/- 8.2 pmol/mg protein in the sarcoplasmic reticulum fraction, P < .05), without significant changes in Kd (4.7 +/- 1.2 versus 2.4 +/- 1.0 mumol/L). All these changes persisted after 20 minutes of reperfusion. Analysis of tissue fractions showed that 55% of the ryanodine binding sites were retained in the pellet of a low-speed centrifugation ("nuclear pellet") and that the effects of ischemia concerned only the receptors released in the supernatant ("postnuclear supernatant"). In parallel experiments, we evaluated the effect of ryanodine on oxalate-supported Ca2+ uptake, which represents sarcoplasmic reticulum Ca2+ uptake. As expected, we found that high concentrations of ryanodine stimulated Ca2+ uptake, owing to channel blockade. The response to 900 mumol/L ryanodine was slightly reduced in crude homogenate and significantly reduced in postnuclear supernatant obtained from ischemic hearts. In conclusion, the number of ryanodine receptors is reduced after ischemia; this effect concerns a subpopulation of the receptors, persists after reperfusion, and might contribute to modify sarcoplasmic reticulum function.

Development of a new laser-Doppler microvelocimetering and its application in patients with coronary artery stenosis.

A new Laser-Doppler microvelocimetering (LDMV) was developed to study blood flow in individual vessels in patients with coronary artery stenosis. The LDMV measures steady blood flow velocity, diameter of vessels and pulsation flow. In addition, the effects of China's traditional herbs--Salvia Mitiorrhyza and other drugs--on the blood flow in the patients were investigated. LDMV constitutes a new quantitative procedure and is a promising tool for rapid monitoring of tissue perfusion in cardiovascular disease and estimating the action of drugs on microcirculation.

Comparison of human CAP and CAP2, homologs of the yeast adenylyl cyclase-associated proteins.

We previously reported the identification of human CAP, a protein that is related to the Saccharomyces cerevisiae and Schizosaccharomyces pombe adenylyl cyclase-associated CAP proteins. The two yeast CAP proteins have similar functions: the N-terminal domains are required for the normal function of adenylyl cyclase, while loss of the C-terminal domains result in morphological and nutritional defects that are unrelated to the cAMP pathways. We have amplified and cloned cDNAs from a human glioblastoma library that encode a second CAP-related protein, CAP2. The human CAP and CAP2 proteins are 64% identical. Expression of either human CAP or CAP2 in S. cerevisiae cap- strains suppresses phenotypes associated with deletion of the C-terminal domain of CAP, but does not restore hyper-activation of adenylyl cyclase by RAS2val19. Similarly, expression of either human CAP or CAP2 in S. pombe cap- strains suppresses the morphological and temperature-sensitive phenotypes associated with deletion of the C-terminal domain of CAP in this yeast. In addition, expression of human CAP, but not CAP2, suppresses the propensity to sporulate due to deletion of the N-terminal domain of CAP in S. pombe. This latter observation suggests that human CAP restores normal adenylyl cyclase activity in S. pombe cap- cells. Thus, functional properties of both N-terminal and C-terminal domains are conserved between the human and S. pombe CAP proteins.

Free radical mechanism in enhancement of radiosensitization by SRSBR.

The free radical mechanism of enhancing radiosensibility by the Synergic Recipe to Strengthen Body Resistance (SRSBR) consisting of 10 Chinese drugs was studied by electron spin resonance (ESR) with spin trapping reagent--Nitroso-tert-butane (NtB) following irradiation of the Deoxy thymidine (dT) solution system by 60Co 3.7 PBq. The results showed SRSBR cannot only enhance the generation of e- aq but also enhance the production of . OH and H . after irradiation with gamma ray in dT-NtB-SRSBR aqueous solution system. These result in damage to biological molecules, attacking and killing tumor cells radiosensitized by SRSBR. The characteristics of SRSBR for radiosensitization are concluded to be increasing production of . OH and H. in comparison with the well-known radiosensitizer-Misonidazole (Miso).

Tb(III) and Eu(III) as fluorescent probes to investigate the metal-binding sites of trichosanthin.

Tb(III) and Eu(III) ions were used as fluorescent probes in the study of the trichosanthin (TCS). The fluorescence of Tb(III) was increased considerably when bound to TCS to replace the Ca(II) ions. The nonradiative energy transfer from fluorescent tryptophan (Trp) residues to the bound Tb(III) or Eu(III) took place. From a Foster d-d nonradiative energy transfer mechanism, it was obtained that the average distance between the bound Tb(III) and the Trp residue is 1.27nm. The results indicate that the major groups in TCS bound to Ca(II) ions should be the carboxylic side groups of the glutamic acid and/or aspartic acid. The fluorescent quenching of Ca(II)-free TCS by adding Tb(III) or Eu(III) into TCS from which the Ca(II) ions had been removed was discussed as well.
