Technical devices in otoplasty to obtain a natural appearance.

This article outlines the importance given by plastic surgeons to correcting the external ear malformations. The objective is to create ears with a normal appearance. Despite more than 200 methods proposed by different authors, a procedure for otoplasty that gives constant and lasting surgical results has not yet been found. Following some critical remarks, both anatomical and surgical, the author proposes a method that, in his experience, gives good results, more natural and reliable, and is free from failures.

[Filling of the residual depression after parotidectomy with a composite arterial subcutaneous flap].

Parotidectomy causes isolation of the facial nerve and an antiaesthetic hollow of the preauricular region. In order to avoid these two sequelae, the author proposes raising of an island subcutaneous flap, as large as required by the area to be filled, from above the resected area, which is then rotated onto the tissue defect. It is an arterial flap, with a superficial temporal artery pedicle and is composed by three superimposed layers: the epicranial aponeurosis (galea capitis); a layer of sub-aponeurotic connective tissue; the periosteum of the cranial bones. The rich blood supply and the fibrous nature of the flap limit its reabsorption and since the flap is drawn from the hair area, the scar is completely concealed.

Corrective surgery for hollow cheeks.

A new method is reported for correction of hollow cheeks. A dermal and submucosal graft removed from the buttock is transferred to a subcutaneous pocket made in the endoral wall of the cheek on the other side of the hollow area. The dental arches push the soft tissues of the cheek outward, thus causing the hollow to disappear.
