[Follow-up studies and special cardiologic diagnosis in patients with suspected heart sarcoidosis].

During a retrospective analysis of 1236 patients with pulmonary sarcoidosis who showed electrocardiographic disturbances or cardiomegaly the existence of a possible heart sarcoidosis was suspected. It was tried to confirm this suspected diagnosis in subsequent examinations of these patients by support of modern non-invasive and invasive cardiological methods. Thereby the differential diagnosis to the coronary heart disease is especially difficult in elderly. This problem represents a permanent diagnostic challenge because of the therapeutic chance in case of heart sarcoidosis.

[The renal pelvic tumor in the sonogram].

A prospective study evaluates screening investigations of the kidneys of 2350 patients. Sonographic criteria of the renal pelvic tumour are established. So-called "indirect signs" are only recognized by the compound scanner. By these ultrasonic symptoms a pelvic tumor could be detected with 15 patients which is a frequency of 6,5%. Part of the patients were preselected. The possibility of false-negative findings remains unconsidered. The study proves the great relevancy of ultrasonography in this disease. When operations of renal pelvic carcinomas were retrospectively checked our diagnostic results were underlined.

[Immunoadsorption using Staphylococcus aureus, COWAN I strain].

Immunoadsorbents bind immunoglobulins, immunocomplexes or immunocytes. Their clinical use could become a new therapeutic principle for diseases in the pathogenesis of which antibodies or immunocomplexes play an important part. Immunoadsorbents are also being tested in the hope that they can at least partially replace the plasmapheresis-plasma exchange therapy at present practised. A report is given on in-vitro studies using protein-A-bearing staphylococci, strain COWAN I. The bacteria bind IgG selectively, IgM to a small extent, IgA, IgD and IgE hardly. The majority of the other plasma proteins tested (e.g. albumin, transferrin, ceruloplasmin, alpha-2-macroglobulin, beta-lipoprotein, alpha-2-glycoprotein, alpha-1-antitrypsin) are bound only non-specifically and in small quantities. The staphylococci can react with acid solutions (glycin-HCl buffer, acetic acid) in several ways. The activation of the complement system by IgG binding to protein A is one of the problems to be solved before immunoadsorbents an be used clinically.

[Impact of synthetic sexual steroids on blood volume (author's transl)].

The isotope dilution technique was used to study the oestrogen component of "Deposiston", a long-acting contraceptive, and its effects on blood volume. Blood volume increased by about ten per cent within 15 days from administration of 3 mg of ethinyloestradiol-sulphonate (1 mg/week). Haematocrit was reduced, at the same time. This haemodynamic initial effect should be always taken into account for any assessment of biochemical parameters in response to hormonal contraception.
