Optimization of atrioventricular delay by surface electrocardiography during dual chamber pacing.

BACKGROUND: Nominal atrioventricular (AV) interval in dual chamber pacemaker (DDD) is not the best AV delay in the majority of patients with atrioventricular block. To find a simple method for optimizing AV delay adjustment, we assessed surface electrocardiography (ECG) for optimizing AV delay during dual chamber pacing. METHODS: DDD pacemakers were implanted in 46 patients with complete, or almost complete, AV block. Optimal AV delay was achieved by programming an additional delay of 100 ms, to the width of intrinsic P wave or to the interval between pacing spike to the end of P wave on surface ECG. Left ventricular (LV) end diastolic and end systolic volumes, ejection fraction and diastolic parameters were measured by Doppler echocardiography during both nominal and optimal AV delay pacing. RESULTS: Compared to nominal AV delay setting, LV end diastolic volume increased [to (53.2 +/- 11.3) ml from (50.2 +/- 10.2) ml, P < 0.05], end systolic volume decreased [to (26.1 +/- 9.0) ml from (27.9 +/- 8.2) ml, P < 0.05] during adjusted AV delay pacing, resulting in an increase in LV ejection fraction [to (68.2 +/- 5.3)% from (64.5 +/- 4.3)%, P < 0.05]. LV diastolic filling and isovolumic relaxation time were not significantly changed. CONCLUSION: Optimization of AV delay by surface ECG is a simple method to improve LV systolic function during dual chamber pacing.

Toward integrated environmental management for challenges in water environmental protection of Lake Taihu basin in China.

Lake Taihu is the third largest freshwater lake in China. It serves many social, economic, and ecological purposes in the drainage basin. Unfortunately, the water has been heavily polluted due to rapid industrialization and urbanization during the last two decades. Notwithstanding great efforts made so far to improve the water quality, the environmental situation is still far from being optimistic. The basin and the lake are facing a range of severe environmental challenges: rapid socio-economic development continues to place great pressures on the environment, current pollution control projects have many problems from the viewpoint of effectiveness and efficiency of their implementations, and the non-point sources of pollution such as agricultural fields, for which control is more difficult than for industrial point sources, have become the main contributors to serious eutrophication of the lake. Considering the characteristics of the environmental challenges and problems confronting the basin and the lake, we focus on integrated environmental management (IEM) as a promising and effective approach to overcome these predicaments. Current practices and problems of environmental management in the basin are examined, and potential future developments are discussed. Three aspects of the IEM are emphasized: institutional cooperation, public participation, and internalization of environmental externalities. We think these are the most critical for not only the basin but also for the whole of China to achieve a sustainable society.

V2O5 nanofibre sheet actuators.

Vanadium oxides, such as V2O5, are promising for lithium-ion batteries, catalysis, electrochromic devices and sensors. Vanadium oxides were proposed more than a decade ago for another redox-dependent application: the direct conversion of electrical energy to mechanical energy in actuators (artificial muscles). Although related conducting polymer and carbon nanotube actuators have been demonstrated, electromechanical actuators based on vanadium oxides have not be realized. V2O5 nanofibres and nanotubes provide the potential advantages of low-cost synthesis by sol-gel routes and high charging capacity and long cycle life. Here, we demonstrate electromechanical actuation for obtained high modulus V2O5 sheets comprising entangled V2O5 nanofibres. The high surface area of these V2O5 sheets facilitates electrochemical charge injection and intercalation that causes the electromechanical actuation. We show that the V2O5 sheets provide high Young's modulus, high actuator-generated stress, and high actuator stroke at low applied voltage.
