Surgical approach to the third area of the internal carotid artery through vertical osteotomy of the mandibular ramus: case report.

Internal carotid artery (ICA) reparative techniques are nowadays widely standardized in traumatic as well as degenerative pathological conditions; therefore, the greatest difficulty encountered in the treatment of lesions at the expense of the most distal segment of the ICA is not a result of the application of these techniques as much as the difficulty in obtaining adequate exposure of the vessel and a sufficiently wide surgical field. In the past, lesions localized more toward the skull were considered to be surgically unreachable. During the last 20 years, however, various techniques have been suggested and attempted for the exposure of this difficult area. This article suggests a type of approach already carried out by the Department of Maxillofacial Surgery of the University of Rome "La Sapienza" in collaboration with the II Department of Vascular Surgery of the same university, which offers the advantage of being of simple and rapid execution with minimum repercussions on the intrasurgical time required and guarantees an excellent exposure of the most distal tract of the ICA with an adequate widening of the surgical field.

Cyclopia: a radiological and anatomical craniofacial post mortem study.

Cyclopia is a rare foetal malformation characterized by a single palpebral fissure and a proboscis associated with severe brain malformation. Approximately 1.05 in 100 000 births including stillbirths are identified as cyclopian. Cyclopia is not compatible with life. The authors present an anatomical and histological study of the fronto-orbito-maxillary region carried out after 3-D CT reconstruction in a 21 week-old foetus with cyclopia. Anatomical and histological observations suggest that the integrity of the trigeminal nerves is very important for the normal development of the embryological structures of the face. Fusion of the facial processes in the midline takes place even if central prosencephalic structures are absent. For this reason the face in cyclopia, in both its positive and negative aspects, constitutes a model for the study of the normal development of this region. Delayed and inadequately treated malar fractures: evolution in the treatment, presentation of 77 cases, and review of the literature.

Seventy-seven cases of delayed or inadequately treated zygoma fractures, treated between 1977 and 1999 in the Maxillofacial Surgery Division of the La Sapienza University in Rome, are presented. The functional and aesthetic problems caused by this pathology are analyzed. The results achieved resolve some of the various aspects of inadequately treated malar fractures. These are possible because of the development of great biomaterials and the introduction of rigid internal fixation. As a consequence the surgical techniques have been revolutionized. Cases, with functional-aesthetic deficits or only aesthetic deficits, are studied separately. An accurate preoperative study permits the operation suitable for the specific features of the individual case to be carried out.
